Introduction

A rare developmental defect during embryogenesis with characteristics of variable upper limb reduction defects and renal anomalies. Patients typically present absence/hypoplasia of digits, radii and/or ulnae, short stature and mild external ear malformation, as well as kidney agenesis or ectopia. There have been no further descriptions in the literature since 1983.

[The prevention of denhong injection on contrast-induced renal impairment after percutaneous coronary intervention].

OBJECTIVETo investigate the prevention of Danhong Injection (DHI) on contrast-induced renal impairment after percutaneous coronary intervention (PCI).METHODSEighty patients receiving PCI were randomly assigned to the control group and the treatment group, 40 in each. All patients used loperamide injection as the contrast media, and received routine medicines such as enteric coated aspirin and Betaloc, as well as routine rehydration therapy. As for patients in the treatment group, 20 mL DHI was intravenously dripped by adding in 250 mL 0.9% sodium chloride injection from 2 -3 days before PCI to 3 days after PCI, once daily. The levels of serum creatinine (SCr), cystatin C (CysC), urine micro-albumin (mAlb), and beta2-microglobulin (beta2-MG) were measured before PCI, and 24, 48, 72 h after PCI. The occurrence of radio contrast-induced nephropathy (RCIN) of the two groups was observed.RESULTSThe serum SCr and CysC levels of the two groups reached the peak 24 h after PCI (P < 0.05, P < 0.01). But they respectively restored to the pre-PCI levels at 48 and 72 h after PCI in the treatment group. In the control group the serum SCr level basically restored to the pre-PCI level at 72 h after PCI. The urinary mAlb and beta2-MG levels of the two groups reached the peak at 24 and 48 h after PCI (P < 0.05, P < 0.01), and basically restored to the pre-PCI level at 72 h after PCI. But they did not restore in the control group (P < 0.05). Seven patients suffered from RCIN in the two groups, of them 5 (12.5%) in the control group and 2 (5.0%) in the treatment group, with no statistical difference (P > 0.05).CONCLUSIONSDHI could effectively prevent contrast-induced renal impairment and shorten the recovery time of renal impairment. It was worth further studies.

[Acute kidney injury and drug-induced nephropathies].

All types of acute kidney injury (AKI) (functional /pre-renal, parenchymal/intra-renal, obstructive/post-renal) result in a sharp drop of the glomerular filtration rate, with variable reversibility according to the initial cause. In one case out of five, drug intake can be related to the onset of AKI. Antibiotics, analgesics and nonsteroidal anti-inflammatory drugs, angiotensin converting enzyme inhibitors and angiotensin II receptor antagonists are the agents mostly involved, as well as iodinated radio-contrast agents. Mechanisms are often complex: toxic cellular effect directed on a nephron segment (tubular necrosis) associated or not with intraglomerular hemodynamic changes, or immune process leading to acute tubule-interstitial nephritis. Each underlying risk factor (age > 60 year, cardiac or hepatic failure, hypertension, diabetes, intra-vascular volume depletion, preexisting or unknown chronic kidney disease) must be taken into consideration by the prescribing physician because it reduces the chance of functional recovery and worsens the renal and the overall prognosis. A pre-renal additional component is often present and avoidable thanks to a strict hemodynamic monitoring. The present article summarizes some recent physiopathological aspects of AKI and makes the link between clinical situations and currently prescribed drugs. Lessons from the radio-contrast induced nephropathy are examined by taking into account prevention aspects and risk factors screening. An effective collaboration between the general practitioner and the nephrologist would benefit in optimizing the treatment of difficult cases.

[Whole abdomino-pelvic moving strip irradiation in ovarian cancer].

From 1982 to 1987, 45 patients with ovarian cancer (21 Stage I, 8 Stage II, 5 Stage III-IV and 11 recurrent or metastatic lesions) were treated in our hospital. All received postoperative 60Co whole abdomino-pelvic moving strip irradiation. Tumor dose ranged from 20 to 26 Gy, in addition, a booster dose of 10-20 Gy was given to the pelvis. Serious gastrointestinal reaction and myelosuppression were rare, and no radio-nephritis or hepatitis was found. The 1, 3 and 5 year survival rates were 100%, 92% and 83% for Stage I; 100%, 100% and 100% for Stage II; 100%, 66.7% and 66.7% for Stage III-IV and 100%, 50% and 20% for recurrent or metastatic lesions, respectively. The author believes that the whole abdomino-pelvic moving strip irradiation is only indicated for patients with dysgerminoma, granulosa cell tumor and resected adenocarcinoma without residual tumor or with residual tumor smaller than 0.5 cm in diameter in the abdomen or smaller than 2 cm in the pelvis. The causes of treatment failure are discussed.
