Introduction

A granulomatous disease caused by KLEBSIELLA RHINOSCLEROMATIS infection. Despite its name, this disease is not limited to the nose and NASOPHARYNX but may affect any part of the RESPIRATORY TRACT, sometimes with extension to the lip and the skin.

Otoscleroma of the Middle Ear and Mastoid Cavity with Facial Palsy: A Case Report.

IntroductionScleroma is a chronic, specific granulomatous disease that affects the head and neck mucosa. Its common sites are the nose and larynx; however, it might affect other areas. One of the rare sites to be affected is the middle ear and mastoid cavity, for which the term otoscleroma was coined. We present such a rare case in this report.Case ReportA 47-year-old patient with a history of both old laryngoscleroma and recent Rhinoscleroma. He presented with symptoms of ear discharge and facial palsy. Examination revealed complete facial nerve palsy and an external auditory canal polyp. A decision was made to perform subtotal petrosectomy with facial nerve decompression and maximum debulking to be sent for histopathology. The result came typical of otoscleroma.ConclusionOtoscleroma is a fairly rare occurrence. It might be primary, with no evidence of Scleroma in other sites or following rhino and/or laryngoscleroma. It might be unilateral or bilateral. It might mimic the clinical picture of chronic suppurative otitis media and its complications. Tympano-mastoidectomy is recommended to stop the discharge, obtain a proper biopsy and decompress the facial nerve if needed. Otoscleroma should be suspected in the case of previous Scleroma in other sites, and otitis media or complications like manifestations.

Nonendemic rhinoscleroma: An unusual manifestation of the trachea

AbstractObjectivesRhinoscleroma is classically described as a chronic granulomatous disease caused by Klebsiella rhinoscleromatis which primarily affects the nose and nasopharynx. When present, tracheal manifestations will be seen late in the disease course rather than on initial presentation. We describe a rare case of nonendemic rhinoscleroma that presented with tracheal lesions as an initial manifestation of disease.MethodsCase report and literature review.ResultsAn 88‐year‐old male presented with longstanding dysphonia. Flexible laryngoscopy demonstrated a septal perforation and diffuse glottic lesions. CT neck demonstrated a nonobstructive polypoid tracheal lesion and mucosal thickening of the paranasal sinuses. Biopsy confirmed an atypical lympho‐histiocytic proliferation and microorganisms within macrophages on Grocott methenamine silver and Steiner stains consistent with rhinoscleroma. He was referred for rheumatology and pulmonology consultation.ConclusionSystemic diseases rarely affect the trachea, and even less frequently is a tracheal lesion identified as the initial manifestation of disease. The most common systemic diseases that affect the trachea include relapsing polychondritis, granulomatosis with polyangiitis, amyloidosis, and inflammatory bowel disease. The literature surrounding nasolaryngotracheal rhinoscleroma is limited, especially in nonendemic areas. It is necessary to include unusual etiologies of airway lesions in the differential diagnosis, which warrants comprehensive airway evaluation including biopsy.Level of Evidence4.
