Family Supportive Supervisor Training and Workplace Assessment Tool

This study will evaluate the effectiveness of FSST 2.0 (family supportive supervisor training plus support for use of family and sick leaves) using a randomized control trial design. We expect the intervention will increase supervisors' family supportive and leave supportive behaviors, which in turn will increase employees' leave use and decrease employees' work-family conflict, emotional exhaustion, depressive symptoms, and turnover intentions. This study will enroll a minimum 200 groups of managers and their employees. Managers in the intervention condition will complete pre- and 2 post-intervention Workplace Assessments, 2 online training modules, 1 webinar where managers can review intervention content and ask questions and share reactions on the materials. Managers in the control group and employees in both groups will complete pre- and 2 post-intervention Workplace Assessments. The post-intervention Workplace Assessment will be completed 3 and 6 months after the intervention. The total duration of the study is 9 months as the wait list control group will be offered the intervention and 2nd follow up survey after the 6-month trial.

Impact of Genomics and Exposures on Disparities in Breast Cancer Radiosensitivity

RATIONALE: Radiation therapy uses high-energy x rays to kill tumor cells. Radiation therapy may cause skin reactions when patients are exposed to high-energy x rays. Studying the genetic pattern of patients before and after radiation therapy may help doctors prevent toxicity and plan the best treatment.
PURPOSE: This clinical trial studies genetic susceptibility to radiation-induced skin reactions in racial/ethnic groups of patients with breast cancer.

Experiences of women with disabilities in Lithuania when their gender, disability, domestic violence, and mental health services intertwine

BACKGROUNDWomen with disabilities are two to five times more likely to experience domestic violence (DV), including intimate partner violence (IPV), when compared with women without disabilities. Survivors of DV and IPV are around three times more likely than women without this life experience to develop mental health conditions or a psychosocial disability.OBJECTIVETo explore the perspectives of women with disabilities who are survivors of DV and IPV about their help-seeking experiences and their use of mental health services in Lithuania.METHODSA qualitative explorative study was implemented within an experiential framework. Semi-structured interviews (n = 15) were conducted with women-survivors of DV who have sensory, physical, psychosocial, and intellectual disabilities. Audio recordings were transcribed verbatim and analysed thematically using MAXQDA software.RESULTSThe key findings highlight the complex intersection between gender, disability, and DV, including IPV. Women with disabilities may be extremely dependent on their abusers for daily individual support and may also experience disability-based violence as a result of this situation. Societal stigma and victim-blaming attitudes, as well as the lack of community-based services, may prevent survivors from seeking help. However, those who do may benefit greatly from support provided by mental health services.CONCLUSIONSFurther qualitative and quantitative research concerning the interlink between the areas of gender, disability, DV and IPV, and the provision of mental health services is needed, especially about what interventions might be the most effective for this particular population.

Social inequalities in eligibility rates and use of the Australian National Disability Insurance Scheme, 2016–22: an administrative data analysis

AbstractObjectivesTo assess differences in eligibility rates and use of the National Disability Insurance Scheme (NDIS).Study designAnalysis of NDIS unit‐record administrative data.Setting, participantsApplicants for NDIS support aged 7 years or older, 1 July 2016 – 31 August 2022 (eligibility analysis); active NDIS participants aged 7 years or older on 31 August 2022 (plan size and spending analyses).Main outcome measuresDifferences in NDIS eligibility rates by broad age group (under 55 years v 55 years or older), gender (girls and women v other applicants), and residential socio‐economic status (three lowest deciles of the Index of Relative Socioeconomic Disadvantage v other areas); differences in NDIS personal plan size (allocation) and spending (use) by gender and residential socio‐economic status.ResultsDuring 2016–22, 705 594 people aged 7 years or older had applied for NDIS support; 485 676 applicants with recorded decisions were included in our analysis (393 152 eligible, 92 524 ineligible). Eligibility rates were highest for applicants with brain injury or stroke, intellectual disability, or autism (900 or more per 1000 applicants), and only minor inequalities by socio‐demographic group were evident. Eligibility rates were lower for applicants with physical disability, psychosocial disability, or unclassified (other) disability (60–75%). Eligibility inequalities were most marked for people with physical disability, with fewer approvals for women and girls than men and boys (145 [95% confidence interval {CI}, 138 − 152] fewer approvals per 1000 applicants), for people aged 55 years or older than for younger applicants (235 [95% CI, 227–242] fewer approvals per 1000 applicants), and for people from lower socio‐economic status areas than for those from other areas (86 [95% CI, 78–93] fewer approvals per 1000 applicants). The eligibility rate for applicants with psychosocial disability was lower for women and girls than men and boys (83 [95% CI, 77–89] fewer approvals per 1000 applicants). Inequalities in plan sizes and spending by socio‐economic group and gender for the 312 268 active participants at 31 August 2022 were smaller.ConclusionsWomen and girls and applicants over 55 years of age or living in socio‐economically disadvantaged areas with certain disability types are less likely to be deemed eligible for NDIS support than other applicants. Inequalities in plan allocation and use of personal NDIS budgets are less marked. Changes to NDIS eligibility processes could reduce these inequalities.

SummaryShort stature is a common complaint among pediatric visits and the differential diagnosis is extensive. Although some variations in growth are normal, deviation from normal growth is often the first symptom of chronic disease in children. This is true for hormone abnormalities including growth hormone deficiency, hypothyroidism and glucocorticoid excess. However, reduced growth velocity can also occur as the first sign of chronic anemia, malnutrition, deprivation (psychosocial dwarfism), chromosomal abnormalities, genetic syndromes and inflammatory bowel diseases. For the primary care provider, simple measures of standing height, sitting height, arm span, weight, body mass index (BMI) and bone age (BA) will lead to the correct diagnosis in most short children. Screening laboratory studies for endocrine disorders, a skeletal survey if skeletal disproportion is evident, a karyotype or microarray (microarray favored if developmental delay is also present) and genetic testing for monogenic disorders will lead to a specific diagnosis in an additional subset of short children. This case presented a diagnostic dilemma that spanned all these possibilities and served as a focal point for the review of normal growth and growth abnormalities.Learning points

Variations in growth can be normal variants (constitutional delay of growth and puberty or familial short stature) but deviation from normal growth can also be the first sign of an underlying pathological process.

Measures of standing height, sitting height, arm span, weight, body mass index (BMI) and bone age (BA) will lead to the correct diagnosis in 50–80% of short children.

Screening laboratory studies for endocrine disorders, a skeletal survey if skeletal disproportion is evident, a karyotype or microarray (microarray is favored if developmental delay is also present) and genetic testing will lead to a specific diagnosis in another 35% of short children.

Pseudohypoparathyroidism (PHP) type 1A is due to a mutation in the alpha subunit of the stimulatory G protein of the guanine nucleotide-binding protein gene. Multiple hormone resistance often affects thyroid-stimulating hormone and, when presenting in the newborn period, can be misdiagnosed as common forms of congenital hypothyroidism.

Molecular testing is an important component of confirming the diagnosis and PHP subtype, which can help guide management.
