Introduction

A common congenital midline defect of fusion of the vertebral arch without protrusion of the spinal cord or meninges. The lesion is also covered by skin. L5 and S1 are the most common vertebrae involved. The condition may be associated with an overlying area of hyperpigmented skin, a dermal sinus, or an abnormal patch of hair. The majority of individuals with this malformation are asymptomatic although there is an increased incidence of tethered cord syndrome and lumbar SPONDYLOSIS. Phase II Pilot Randomized-Controlled Trial for the Investigation of the Preliminary Efficacy of Surgical Sectioning of the Filum Terminale in Treating Occult Tethered Cord Syndrome Patients

The investigators hypothesize that surgical release of the filum terminale (strand of fibrous tissue at the end of the spinal cord) is a more efficacious treatment option for symptomatic relief than medical management in subjects with Occult Tethered Cord Syndrome (OTCS) and that the risks do not outweigh the benefit profile.

The Relationship Between Nocturnal Enuresis And Spina Bifida Occulta: A Prospective Controlled Trial

It is claimed that SBO may be responsible for bladder dysfunction in patients without known neurological disease. Subsequently, it was reported that the frequency of SBO in NE cases was higher than normal children. However, in controlled trials, SBO frequency was not different in NE patients compared to the normal population. Conversely, the incidence of dysfunctional bladder in the presence of SBO in NE patients was found to be higher and the response to treatment was worse than in non-SBO patients.
The present study aimed to determine whether the frequency of SBO in patients with NE was higher than in healthy subjects, the effect of SBO on the severity of LUTS and whether treatment response of primary NE patients changed in the presence of SBO.

Cerebrospinal fluid leakage following penetrating spinal trauma in a patient with congenital spinal dysraphism: A case report

INTRODUCTIONSpinal dysraphism, a congenital condition characterized by vertebral malformations like spina bifida occulta, may lead to cerebrospinal fluid (CSF) leaks following a minor trauma. We report a case of a 19-year-old male with congenital spinal dysraphism who developed a CSF leak after a stab wound to the lumbosacral region.PRESENTATION OF CASEThe patient was admitted four days post-injury with CSF leakage, vertigo, and nausea but no neurological deficits. Imaging revealed a dural tear and spina bifida occulta. The combined surgical intervention, including primary closure, dural repair, and medical intervention, was remarkably successful, significantly improving the patient's condition.DISCUSSIONThis case underscores the urgent need for early detection and treatment of CSF leaks in patients with congenital spinal dysraphism who experience trauma. Spinal dysraphism can significantly increase the risk of complications following traumatic injuries. The absence of immediate symptoms and underlying anomalies complicates diagnosis and treatment, emphasizing the need for thorough evaluation using imaging studies to identify potential dural defects after spinal trauma.CONCLUSIONThis case highlights the critical relationship between congenital spinal dysraphism, primarily spina bifida occulta, and spinal traumatic injuries. It also highlights the importance of accurate diagnosis and early intervention to improve patient outcomes and prevent complications.

Evaluation of MRI technologies for surgical spine planning and navigation

PURPOSECT is considered the gold standard to assess bone morphology, whereas MRI is the imaging modality of choice to assess soft tissue. To reduce the ionising radiation exposure of the patient from CT, new MRI technologies have been developed to obtain images of bone. Two promising methods include MRI-based synthetic CT (sCT) and T1-weighted isotropic MRI.METHODSA prospective study including twenty-four adult patients receiving lumbar or sacral spine stabilisation surgery was performed. For each patient, two scans were acquired: a 3D dual echo T1-weighted gradient image, from which a sCT was reconstructed, and a 3D isotropic T1-weighted MRI sequence. Three observers assessed the sCT images for adequate visualisation of relevant vertebral anatomies and confidence using sCT in preoperative planning compared to the isotropic MRI based on a series of statements scored using a Likert-scale. Summary statistics and intraclass correlation coefficients were calculated.RESULTSAll observers agreed that the sCT provided adequate visualization (94% of cases). Compared to the isotropic MRI, the sCT provided added value (89% of cases) and improved confidence (92% of cases) for the preoperative planning stage. No unexpected poor intraclass correlations were identified. The observers diagnosed patients with spondylolysis, scoliosis, arthrosis, spina bifida occulta, various Castellvi classifications, or without pathology.CONCLUSIONSsCT adequately visualised vertebral structures relevant for surgical spine planning with good confidence and added value for sCT compared to 3D T1-weighted isotropic MRI is shown. sCT could be a valuable method to reduce the radiation exposure associated with CT.

Novel Predictive Scoring System for Bone Union Rate After Conservative Management of Lumbar Spondylolysis

Study Design.A single-center retrospective cohort study.Objectives.To develop a predictive scoring system for bone union after conservative treatment of lumbar spondylolysis and assess its internal validity.Summary of Background Data.Lumbar spondylolysis, a common stress fracture in young athletes, is typically treated conservatively. Predicting bone union rates remains a challenge.Methods.This study included patients aged 18 years or younger with lumbar spondylolysis undergoing conservative treatment. A multivariable logistic regression analysis was used to develop a scoring system containing 6 factors: sex, age, lesion level, main side stage of the lesion, contralateral side stage of the lesion, and spina bifida occulta. The predictive scoring system was internally validated from the receiver operating characteristic (ROC) curve using bootstrap methods.Results.The final analysis included 301 patients with 416 lesions, with an overall bone union rate of 80%. On multivariable analysis, the main and contralateral stages were identified as factors associated with bone union. The predictive scoring system was developed from the main side stage score (prelysis, early=0, progressive stage=1) and the contralateral side stage score (none=0, prelysis, early, progressive stage=1, terminal stage=3). The area under the curve was 0.855 (95% confidence interval, 0.811–0.896) for the ROC curve, showing good internal validity. The predicted bone union rates were generally consistent with the actual rates.Conclusions.A simple predictive scoring system was developed for bone union after conservative treatment of lumbar spondylolysis, based on the stage of the lesion on the main and contralateral sides. The predicted bone union rate was ~90% for a total score of 0–1 and ≤30% for a score of 3–4. This system demonstrated good internal validity, suggesting its potential as a useful tool in clinical decision-making for the management of spondylolysis.

Celebrating 100 years of caring for childrenMONTREAL, Oct. 04, 2024 (GLOBE NEWSWIRE) -- The 36th edition of the Canada Shrine Bowl will take place on Saturday, October 5 at 1 p.m. at Concordia Stadium, 7200 Sherbrooke Street West in Montreal. Shriners Children’s Canada will be celebrating its 100th anniversary this year and has provided pediatric specialty care to thousands of patients over that time. This year’s game will also feature two patient ambassadors, Finley and Émerick, to proudly represent the hospital. This year's matchup is between the Concordia University Stingers against the Sherbrooke University Vert & Or. The public is invited to attend this annual event in support of Shriners Hospitals for Children Canada. Kevin Regimbald, head coach of the Vert & Or, and Brad Collinson, head coach of the Concordia Stingers, are all set for this superb sporting match between the two teams.
