Cornual Wedge Resection for Interstitial Pregnancy and Postoperative Outcome

Cornual pregnancy is a rare form of ectopic pregnancy that accounts for 2% to 4% of ectopic pregnancies, with a mortality rate between 2.0% and 2.5%. Traditionally, interstitial pregnancies were treated by laparotomy with cornial resection or hysterectomy. However, nowadays, increasingly interstitial pregnancies are treated by laparoscopic cranial resection.
Although this technique can potentially protect the fertility of the patient, it carries a serious risk of bleeding and requires a surgical experience.

Interstitial pregnancy: A retrospective case series of surgically managed patients over a ten‐year period

AimsTo describe the ultrasound diagnostic features and surgical management procedures for patients with an interstitial ectopic pregnancy in our tertiary institution and associated peripheral hospital over a ten‐year period.MethodsA retrospective audit of all surgically managed cases of interstitial pregnancies over a ten‐year period at a tertiary hospital and one associated peripheral hospital in New South Wales.ResultsSixteen cases of surgically managed interstitial pregnancy were identified. In 43.8% of these cases, patients had previously undergone an ipsilateral salpingectomy. No cases required hysterectomy, post‐operative methotrexate or return to theatre. Ten patients underwent diagnostic ultrasound prior to operative management, seven of which were correctly identified to be an interstitial ectopic pregnancy at the time. The proportion of cornuostomies being performed for interstitial pregnancy compared to wedge resection has increased over the period of this review from 33 to 60% between the two five‐year periods.ConclusionThe combination of expert ultrasound and sophisticated laparoscopic techniques at our institution has facilitated earlier diagnosis and greater use of minimally invasive management of interstitial pregnancy.

Surgical Management of Interstitial Ectopic Pregnancy Using Intramyometrial Vasopressin and Cornuostomy

Interstitial pregnancy is a rare entity with a high risk of maternal mortality. It is difficult to diagnose in early pregnancy and can result in uterine rupture at the cornual end leading to catastrophic heamorrhage. The key to its management lies in early diagnosis by detailed history history, early radiological diagnosis, and management by medical or surgical means as indicated. Surgical management forms the mainstay of management in case of ruptured interstitial pregnancy or if the gestational age is advanced and the HCG levels are high. There is an inclination towards laproscopic management rather than laparotomy, and more conservative techniques involving cornuostomy rather than cornuectomy. Caution should be exercised in the next pregnancy as there is a risk of repeat intestitial pregnancy in the next conception. An elective cesarean section should be done in the next pregnancy to avoid the risk of uterine rupture at the cornuostomy site.
