Surgical management of foregut duplication cyst existing with a congenital diaphragmatic hernia: A case report

INTRODUCTIONThere have been two cases of congenital diaphragmatic hernia associated with a gastroesophageal duplication cyst documented in the literature, both presented symptomatically in the newborn period. This case is unique given the patient presented in adolescence asymptomatically.PRESENTATION OF CASEWe present a case of a 15-year-old female who initially presented with lower abdominal pain. A CT chest and abdomen was concerning for a moderate to large hiatal vs diaphragmatic hernia and an adjacent 4.3 cm lower mediastinal cyst. Though asymptomatic, the patient underwent diagnostic thoracoscopy with cyst excision. On post operative day one, the patient endorsed severe uncontrolled abdominal pain with multiple bouts of non-bilious emesis. Chest X-ray and upper GI were positive for herniation of the stomach through the diaphragm. The patient was taken back to the operating room emergently for diagnostic thoracoscopy where the stomach was found herniated through a diaphragmatic defect with no evidence of necrosis. The stomach was reduced into the abdominal cavity. The patient tolerated this procedure well, recovered appropriately and was seen two-weeks post-operatively fully recovered.DISCUSSIONBetter visualization and thorough tactile check of the diaphragmatic defect during the initial operation would increase the chance of detecting a defect in the diaphragm immediately and avoid subsequent herniation of abdominal contents.CONCLUSIONWhen Foregut duplication cysts exist near the diaphragm it is critical to get a thorough view of the anatomy and ensure no diaphragmatic defect exits.

Unusually severe neonatal presentation of mediastinal bronchogenic cyst

AbstractBronchogenic cysts are rare congenital malformations that occur in adults and children, with differences in distribution and presentation. We present the case of a newborn who initiated respiratory distress from the first minutes of life, presenting with hypoventilation and rightward displacement of the cardiac impulse, requiring oxygen therapy and intubation. The first radiograph shows a left pulmonary emphysema. The computerized axial tomography revealed a large mediastinal mass causing an obstructive syndrome of the left bronchus. The mass was successfully excised, resulting in a favorable clinical evolution. Although the presentation of our case is exceptional, it is worth noting that while most bronchogenic cysts are asymptomatic, they can occur in childhood, even from birth, as in our case.

A 70-Year-Old Man With an Encapsulated Mediastinal Fluid Collection

CASE PRESENTATIONA 70-year-old man was diagnosed with mid-thoracic esophageal squamous cell carcinoma (distance from incisors, 27-30 cm) because of progressive dysphagia and underwent thoracic laparoscopic esophagectomy at a local hospital. He was transferred from the ICU 4 days after surgery; however, a large amount of purulent fluid exuded from the neck incision after oral drinking, which was consistent with cervical anastomotic leakage. Later, the patient experienced difficulty breathing and expelling sputum; he was then transferred back to the ICU for treatment. A CT scan showed massive fluid collection in the mediastinum and left pleural cavity. Thoracentesis yielded yellowish fluid, and the patient's general condition gradually improved after placement of a closed chest drainage system. The patient's cervical anastomotic fistula persisted and did not heal, and he was subsequently transferred to our medical center with the closed chest drainage system left in place.
