Assessing the Value of 18F-Fluciclovine PET/CT in the Assessment of Pancreatic Transplants

Hypothesis 1: 18F-Fluciclovine PET/CT can correctly and easily identify the pancreatic allograft and determine its viability
Aim 1: Assess whether 18F-Fluciclovine can identify the pancreatic allograft accurately and assess its viability and visibility
Hypothesis 2: 18F-Fluciclovine PET/CT uptake in the pancreas (SUV) is related to total pancreatic function and therefore can indicate whether the pancreatic allograft is at risk of rejection

Clinical Comparison of MEBO Ointment and Hyaluronic Acid Gel in the Management of Pain After Free Gingival Graft Harvesting: A Randomized Clinical Trial

Postoperative pain as well as bleeding are the most common complications after palatal graft harvesting also postsurgical swelling have been reported , Although different agents were used to protect the denuded areas on the palate as gelatin sponge , platelet rich fibrin (PRF) , medicinal plant extract (MPE) and platelet concentrate , no gold standard can be specified for this purpose
PICO Format:
P: Patients with mucogingival defects that require free gingival graft. I:1. Hyaluronic acid gel 0.2% placed in the palatal donor site then covered with periodontal pack (test group I) 2.MEBO ointment placed in the palatal donor site then covered with periodontal pack (test group II) C: Periodontal pack only O: Post-operative pain T: day 3, 7, 14, 21 and 42 postoperative.
Aim of the study:
To compare the effect of MEBO ointment versus Hyaluronic acid gel 0.2% applied to palatal donor site in post-operative pain reduction after free gingival graft harvesting.
Research question:
Is MEBO ointment as effective as hyaluronic acid gel in the management of postoperative pain after free gingival graft harvesting in the management of mucogingival defects?

[A study of early graft healing after anterior cruciate ligament reconstruction in over-the-top position].

ObjectiveTo compare early graft healing between over-the-top (OTT) and anatomic single-bundle (SB) anterior cruciate ligament (ACL) reconstruction.MethodsA clinical data of 40 patients underwent ACL reconstruction, who admitted between June 2021 and October 2022 and met the selective criteria, was retrospectively analyzed. Among them, 20 patients were treated with OTT reconstruction (OTT group) and 20 with SB reconstruction (SB group). There was no significant difference between groups ( P>0.05) in the gender, age, affected side, disease duration, degree of meniscus injury, body mass index, and preoperative International Knee Documentation Committee (IKDC) score, Lysholm score, pain visual analogue scale (VAS) score, and KT-2000 measurement. At 3, 6, and 12 months, MRI was performed to measure the signal noise quotient (SNQ) of the proximal end, middle, and distal end of the graft in the two groups, as well as at the corner of the graft with lateral femoral condyle and 1 cm around the femoral fixation point in the OTT group, to observe the degree of graft healing. Before operation and at 3, 6, and 12 months, the knee function and pain were evaluated by IKDC score, Lysholm score, and VAS score. Before operation and at 12 months after operation, the KT-2000 measurement was taken to evaluation the knee joint stability.ResultsAll operations were successfully completed in both groups and the incisions healed by first intention. All patients were followed up 12-15 months (mean, 12.9 months), with no significant difference in the follow-up time between groups ( P>0.05). After operation, the IKDC score, VAS score, and Lysholm score improved gradually over time in both groups, with significant differences between different time points ( P<0.05). The differences between groups at 3, 6, and 12 months after operation were not significant ( P>0.05). The anterior and posterior stability of the knee joint improved significantly in both groups at 12 months after operation, and the difference in KT-2000 measurements was significant when compared with the preoperative value ( P<0.05), but the difference of pre- and post-operation between groups was not significant ( P>0.05). At 3, 6, and 12 months after operation, MRI showed that the differences in the SNQ of the proximal end and middle of the grafts between the two groups were not significant ( P>0.05), and the SNQ of distal end was significantly higher in the SB group than in the OTT group ( P<0.05). At each time point, grafts in the OTT group had the highest SNQ at the corner and the lowest at the fixation point, and the differences were significant compared to the other sites ( P<0.05). In the two groups, except for the fixation point, the SNQ of the remaining sites were highest at 6 months and lowest at 12 months ( P<0.05). In addition, there were significant differences in SNQ between the different sites of grafts ( P<0.05), and the SNQ was lowest at proximal end and highest at distal end. At last follow-up, the knee grafts in both groups were in good shape and no graft necrosis or loosening of the internal fixation was observed.ConclusionThe knee joint function and graft healing after OTT reconstruction of ACL are similar to those of SB reconstruction, but it should be noted that the healing at the corner of the graft is slower.

Graft Intolerance Syndrome Complicates Retransplantation

BACKGROUNDPatients with non-functioning renal grafts constitute approximately 4% of patients with incident dialysis. Complete withdrawal of immunosuppression has been associated with a higher risk of HLA sensitization and renal graft intolerance syndrome (GIS).METHODSWe conducted a retrospective observational study of 63 patients with renal graft failure (from January 2012 to December 2022). Immediate graft losses due to vascular thrombosis or technical complications were excluded. We analyzed demographic and clinical characteristics, as well as the evolution of GIS. A comparative study was performed with patients who did not develop GIS (non-GIS).RESULTSTwenty-three patients (36.5%) developed GIS. The most common presentation was anemia (73.9%), followed by renal graft pain (65.2%). Seventeen patients (74%) required renal graft embolization. GIS was associated with a higher degree of sensitization. We did not find differences in prior HLA sensitization, causes of graft failure, history of acute rejection, or doses/levels of immunosuppression after graft failure, although rejections were more severe in the GIS group. Patients with GIS had a lower rate of retransplantation after graft failure (4.3% vs 25%, P = .02). Graft failure within the first 2 years after transplantation was the main predictive factor for GIS (hazard ratio = 2.740, 95% confidence interval = 1.06-7.06, P = .03).CONCLUSIONSGIS was more prevalent in patients who experienced graft failure within the first 2 years. Despite increased immunosuppression, a significant percentage required graft embolization. GIS was associated with an increase in HLA sensitization and a lower likelihood of retransplantation.

The Extracellular Matrix Promotes Diabetic Oral Wound Healing by Modulating the Microenvironment

Oral wounds in diabetes mellitus (DM) often delay healing due to reduced angiogenesis and increased inflammatory response in the local microenvironment, even leading to graft necrosis and implant failure. Therefore, developing an effective program to promote healing is of great clinical value. Much of the current research is focused on promoting wound healing through surface adhesive materials that exert a pro-angiogenic, anti-inflammatory effect. However, the application of surface bonding materials in the oral cavity is very limited due to the humid and friction-prone environment. Decellularized extracellular adipose tissue (DAT) is an easily accessible and biocompatible material derived from adipose tissue. To further explore the potential of DAT, we used multi-omics to analyze its composition and possible mechanisms. Proteomic studies revealed that DAT contains anti-inflammatory, pro-angiogenic proteins that promote DM tissue regeneration. To adapt to the moist and chewing friction environment of the mouth, we modified DAT into a temperature-sensitive hydrogel material that can be injected intramucosally. DAT hydrogel has been verified to promote angiogenesis and exert anti-inflammatory effects through macrophage phenotypic transformation. Meanwhile, transcriptome analysis suggested that the inhibitory effect of DAT on the interleukin 17 signaling pathway might be a key factor in promoting DM oral wound healing. In conclusion, after multi-omic analysis, DAT hydrogel can exert good pro-angiogenic and anti-inflammatory effects through the interleukin 17 signaling pathway and can be adapted to the specific environment of the oral cavity. This provides a potential way to promote DM oral wound healing in a clinical setting.
