Clinical and Radiographic Presentation and Surgical Outcomes of T-Box Pituitary Transcription Factor (TPIT) Silent Corticotroph Pituitary Neuroendocrine Tumors: A Multi-institutional Experience and Review of the Literature

OBJECTIVEThe aim of this study is to characterize the incidence, aggressiveness, and clinical outcomes of silent TPIT+ PitNETs, as well as treatment strategies in the event of recurrence/progression. We also review the current literature surrounding TPIT+ silent corticotrophs.METHODSAn institutional review board-approved retrospective study of prospectively acquired patients undergoing resection of PitNETs at the University of Washington and University of Southern California between 2011 and 2023 was performed. A prospectively maintained Research Electronic Data Capture database at each institution was queried for patients with tumors immunostaining positive for TPIT and included for study regardless of adrenocorticotropic hormone (ACTH) status. Exclusion criteria included patients with biochemically confirmed Cushing disease. Patient demographics, preoperative radiographic findings, and surgical outcomes were documented. Descriptive statistics were reported for the patient cohort and recurrence/progression free survival analysis was measured and visualized using Kaplan-Meier and Swimmer plots.RESULTSA total of 1475 patients underwent surgical resection of PitNET with a total of 107 TPIT-immunoreactive tumors. Of these, 37 (34.6%) patients were diagnosed with Cushing disease preoperatively and were excluded from the analysis, leaving 70 (65.4%) patients with TPIT+ silent corticotroph PitNETs. A total of 56 (80%) tumors were only TPIT+, while 14 (20%) stained positive for multiple transcription factors including steroidogenic factor-1, pituitary-specific positive transcription factor 1, or both. The cohort consisted of 45 (64.3%) ACTH+ tumors and 25 (35.7%) ACTH tumors. There were 19 (27.1%) men and 51 (72.9%) women, with mean age 51.3 years. Radiographically, growth beyond the sella into the suprasellar space 54 (77.1%), cavernous sinus 41 (51.4%), and clival/sphenoid 12 (17.1%) compartments was common. A total of 67 (95.7%) of cases were treated via an endoscopic endonasal approach. Gross total resection (GTR) was achieved in 47 (70.1%) of cases. Of those undergoing GTR, two (4.3%) experienced tumor recurrence. Of those undergoing subtotal resection, four (20%) experienced tumor progression (P = 0.06). The median recurrence/progression free survival of TPIT+ tumors was 51.3 months. When stratified by extent of resection, median recurrence free survival was 38.3 months for GTR versus median progression free survival of 51.3 months for subtotal resection (P = 0.88).CONCLUSIONSWith the addition of TPIT staining, the diagnosis of silent corticotroph PitNETs increased substantially versus those defined by ACTH immunostaining alone. Regardless of hormone status, these tumors continue to exhibit high rates of extrasellar growth and high rates of recurrence/progression.

Comparative population genomics analysis for chicken body sizes using genome-wide single nucleotide polymorphisms

Objective: This study aims to investigate the selection history, genome regions, and candidate genes associated with different chicken body sizes, thereby providing insights into the genetic basis of complex economic traits such as chicken body size and growth.Methods: In this study, a total of 217 individuals from eight breeds were selected. According to body size, they were divided into two groups: large chickens and bantam chickens, with four breeds in each group. Firstly, we investigate population structure by principal component analysis (PCA), phylogenetic tree, and ancestry component analysis. Next, we recognize runs of homozygosity (ROH) islands through calculating ROH. Finally, we carry out selection signatures analysis utilizing population differentiation index and nucleic acid diversity.Results: The population structure analysis show that large and bantam chickens are clearly separated. Large chickens are clustered together, the bantam chickens are relatively dispersed. The results of ROH island analysis show that 48 and 56 ROH islands were identified in large and bantam chickens respectively. Among the interesting ROH islands, a total of eight candidate genes were identified. In selection signatures analysis, a total of 322 selected genes were annotated in large chickens, such as POU1F1, BMP10, enrichment in 16 gene ontology (GO) terms. In bantam chickens, a total of 447 selected genes were annotated, such as IGF1, GRB10, enrichment in 20 GO terms and 2 Kyoto encyclopedia of genes and genomes pathways. The haplotype analysis results show that GRB10 has differences in chickens of different body sizes.Conclusion: By population structure, ROH islands, and selection signatures analysis, we have identified multiple genes associated with chicken body size, growth, and development (such as BMP10, IGF1, GRB10, etc). This provides a theoretical reference for the subsequent development of molecular markers for chicken body size and the analysis of the genetic mechanism of chicken body size.

Ablation of Leptin Receptor Signaling Alters Somatotrope Transcriptome Maturation in Female Mice

AbstractAnterior pituitary somatotropes respond to metabolic signals from the adipokine leptin to optimize functional responses to the body's nutritional state via growth hormone (GH) secretion. Molecular targets of leptin in pituitary somatotropes include GH, the GH-releasing hormone receptor (GHRHR), and, in females, the transcription factor POU1F1, all of which are dependent on leptin stimulation for expression. To identify the trophic mechanisms underlying leptin action upon somatotropes, we analyzed single-cell gene transcriptomes comparing pituitaries from a female mouse model bearing somatotropes lacking leptin receptors (LEPR-null mutants) and control pituitaries. Computational clustering of results identified all common pituitary cell types and differentially expressed genes. Mutant female somatotrope clusters showed decreased levels of Gh and Htatsf1 mRNA, which was also reduced in mutant pituitaries lacking Prop1 or POU1F1. Mutant somatotropes also showed increased expression of markers for pituitary stem and progenitor cells (eg, Sox9) and increased (1.73-6.7 fold) expression of nonsomatotrope hormones, Pomc, Lhb, Tshb, Cga, and Prl. Conversely, the mutant female Sox2-positive stem cell cluster showed decreased expression of markers for stem cells and increased expression of pituitary hormone genes. The data support a model in which the female pituitary somatotrope cell population's development and/or maintenance requires leptin trophic signals and also suggests that, in the absence of normal somatotrope maturation, pituitary stem cells are driven towards premature differentiation.
