Joint association of remnant cholesterol and lipoprotein‐associated phospholipase A2 with composite adverse events: A 12‐year follow‐up study from Asymptomatic Polyvascular Abnormalities Community study

AbstractAimsTo explore the association of remnant cholesterol (RC) and lipoprotein‐associated phospholipase A2 (Lp‐PLA2) with composite adverse events in a large‐scale prospective study.MethodsAll data were collected from the Asymptomatic Polyvascular Abnormalities Community study between 2010 and 2022. Serum cholesterol levels and Lp‐PLA2 were determined by enzyme‐linked immunosorbent assay. The participants were categorized into four groups based on their RC and Lp‐PLA2 levels: low‐RC/Lp‐PLA2−, high‐RC/Lp‐PLA2−, low‐RC/Lp‐PLA2+ and high‐RC/Lp‐PLA2+. The composite endpoint was a combination of first‐ever stroke, myocardial infarction or all‐cause mortality. Cox regression analyses were performed to evaluate associations of RC and Lp‐PLA2 with composite adverse events.ResultsOf the 1864 eligible participants, the average age was 60.6 years, and 74.3% were male. Over a follow‐up of 12 years, we identified 500 composite adverse events, including 210 major adverse cardiovascular events and 342 all‐cause deaths. When compared with the group of low‐RC/Lp‐PLA2−, the hazard ratios with 95% confidence intervals in the group of high‐RC/Lp‐PLA2+ for stroke, myocardial infarction, major adverse cardiovascular event, all‐cause death and composite endpoints were 1.37 (0.87–2.16), 0.72 (0.28–1.82), 1.29 (0.85–1.95), 1.61 (1.10–2.38) and 1.43 (1.07–1.91), respectively. A significant interaction between RC and Lp‐PLA2 status has been found for all‐cause death and composite endpoint (p for interaction <0.05). In addition, joint association of RC and Lp‐PLA2 with all‐cause death was modified by sex and age of <60 versus ≥60 years (p for interaction: 0.035 and 0.01, respectively).ConclusionsElevated RC and Lp‐PLA2 levels were associated with an increased risk of composite adverse events, with these associations significantly influenced by sex and age. Our study highlights the synergistic effect of RC and Lp‐PLA2 on the composite adverse events.

Association of remnant cholesterol with coronary heart disease and severity of coronary artery stenosis in non-dialysis patients with stage 3–5 chronic kidney disease

BACKGROUND AND AIMRemnant cholesterol (RC) is a risk factor for coronary heart disease (CHD). This study aimed to investigate the association between RC, CHD, and the severity of coronary artery stenosis in patients with stage 3-5 chronic kidney disease (CKD).METHODS AND RESULTSWe conducted a retrospective study of non-dialysis patients diagnosed with stage 3-5 CKD and suspected CHD who underwent coronary angiography (CAG) at our center between August 2015 and December 2017. CAG was used to diagnose CHD, and the Gensini score was used to quantify the severity of coronary artery stenosis. Of 870 patients included in this study, 659 were diagnosed with CHD. There was no significant difference in the RC concentration between the CHD and non-CHD groups (P = 0.129). Multivariate logistic regression analysis demonstrated that RC was ssociated with CHD (OR 2.733, 95 % CI 1.094-6.825; P = 0.031). Further analysis of the CHD group showed that the Gensini score for severe coronary artery stenosis in the third tertile of the RC concentration (44 [20-86]) was higher than that in the first tertile (36 [15-68]; P = 0.046). After adjusting for potential confounders, the RC level was independently associated with severe coronary artery stenosis (tertile 2 vs. tertile 1: adjusted OR 3.659, 95 % CI 1.231-10.878; P = 0.020; and tertile 3 vs. tertile 1: adjusted OR 5.597, 95 % CI 1.835-17.070; P = 0.002).CONCLUSIONSHigher RC levels were independently associated with CHD and severe coronary artery stenosis among non-dialysis patients with stage 3-5 CKD.

SMP30 Alleviates Oxidative Stress and Regulates Ca ²⁺ -ATPase Activity in UVR-B-Induced Cataracts in Rats

PURPOSEOxidative stress, ultraviolet radiation, and calcium imbalance are key components in the onset and advancement of cataract, which continue to be the leading cause of blindness globally. An important newly discovered aging maker, Senescence marker protein 30 (SMP30) regulates calcium and participates in mitigating oxidative stress damage. Here, we examined the beneficial role of SMP30 in protecting against ultraviolet radiation type B (UVR-B)-induced cataract in rats.METHODSWistar rats (2 months) were arbitrarily assigned into 4 groups of 10 rats. These groups included the Control group, UVR-B group, adeno-associated virus 2 vectors negative control (AAV2-NC) group, and adeno-associated virus 2-mediated overexpression of SMP30 (AAV2-SMP30) group. The control group received Phosphate-buffered saline (PBS) via injection, while the AAV2-NC group and AAV-SMP30 group were separately injected with AAV2-NC and AAV2-SMP30 vectors. In addition to the control group, the remaining three experimental groups were subjected to ultraviolet light exposure 4 weeks post-injection. The lens opacity was examined by stereoscopic microscope, and the lenses were separated to measure oxidative damage parameters particularly superoxide dismutase (SOD), glutathione peroxidase (GPX), and Ca²⁺-ATPase activity. The localization and expression of SMP30 and Ca²⁺-ATPase in the lenses were determined using immunohistochemistry and RT-qPCR.RESULTSAfter UVR-B irradiation, the AAV2-SMP30 group exhibited a substantial decrease in lens opacity compared to the UVR-B group. The results revealed a notable downregulation of SMP30 expression and the activities of SOD, GPX, and Ca²⁺-ATPase of rat lens following exposure to UVR-B radiation. However, SMP30 overexpression partially reversed these effects. In vivo experiments demonstrated SMP30 overexpression attenuated the UVR-B-induced decline in SOD, GPX, and Ca²⁺-ATPase activities.CONCLUSIONThis study demonstrates that SMP30 has the potential to reduce lens opacity caused by UVR-B by increasing antioxidant stress and regulating Ca²⁺-ATPase activity. SMP30 might be a cutting-edge target for the treatment of cataracts.
