Prospective Aleatory Study of Non Invasive Mechanical Ventilation Versus Respiratory Rehabilitation in Hypercapnic Stable Severe COPD

Objective: To analyze the additional benefits of exercise training application by the non-invasive home mechanical ventilation in patients with stable COPD and hypercapnic respiratory failure. SUBJECT: moderate-severe COPD (FEV1 <60%) in chronic respiratory failure (hypoxemia and hypercapnia PaCO2> 45mmHg). GROUPS: 45 patients included prospectively and randomly into 3 groups of 15: a) training + NIPPV group, b) Group training, c) Group NIPPV. Hypothesis: A training program to the effort associated with treatment with NIPPV significantly increase the effects compared with each treatment. MAIN OBJETIVE: Effects on exercise capacity as measured by the test of endurance cycling and test 6-minute walk (distance). SECONDARY OBJECTIVES: Impact on quality of life and dyspnea, as measured by questionnaire and CRQ, systemic inflammatory response (CRP, IL-8, TNF-α), changes in peripheral muscle strength (1RM test, isometric) and effects score BODE index.

A unilateral increase in the occlusal vertical dimension of growing rats results in mandibular deviation

AbstractObjectiveThe effects of unilateral increased occlusal vertical dimension (iOVD) on bilateral craniofacial, mandibular and alveolar development in growing rats were investigated via cone‐beam computed tomography (CBCT). The role of Wnt/β‐catenin signalling in this process was examined.Materials and MethodsForty‐eight female Sprague–Dawley rats were randomly allocated into unilateral iOVD and sham groups. At 2, 4 and 8 weeks, the rats were scanned via CBCT to analyse cranial, maxillary, mandibular and dental morphology. Changes in temporomandibular joint (TMJ) cartilage histology and Wnt/β‐catenin signalling were assessed by histochemical and immunohistochemical staining and qRT–PCR.ResultsDorsal cephalograms revealed that the mandible in the iOVD group tilted approximately 4° to the right. Unilateral iOVD had little effect on cranial and maxillary growth but inhibited mandibular growth (mandibular length and ramal height), especially on the deviated side (DS). Moreover, unilateral iOVD increased the length of the lower incisors and decreased the height of the molars on the DS. Unilateral iOVD induced bilateral osteoarthritis‐like changes in the bilateral TMJ condylar cartilage and activated Wnt/β‐catenin signalling in the condylar cartilage, especially on the contralateral side (CLS).ConclusionOcclusion with unilateral iOVD induced mandibular deviation, significantly inhibited mandibular growth and produced compensatory changes in the alveolar bone. In the iOVD group, the mandibular body length and ramal height were greater on the CLS than on the DS. Moreover, the greater β‐catenin protein expression in the TMJ condylar cartilage on the CLS than on the DS may account for the difference in asymmetrical mandibular development.

Unilateral Bleomycin-induced Interstitial Pneumonitis Mouse Model With Both a Healthy and a Diseased Lung

BACKGROUND/AIMA standard mouse model of pulmonary fibrosis has been created by intratracheal or intraperitoneal administration of bleomycin. However, a difficulty presented by this traditional method is its high mortality rate of more than 50% after bleomycin administration. In this study, we aimed to establish a unilateral lung disease model and to assess its feasibility and usefulness.MATERIALS AND METHODSAfter 6-week-old C57BL/6 mice were anesthetized, a 1.7Fr microcatheter was advanced into the trachea using an otoscope. Then, 1.0 mg/kg of bleomycin was injected into bilateral lung at the trachea (n=13) or unilateral lung (n=14) after advancing the microcatheter to the left main bronchus under fluoroscopy. Body weight change and survival of bilateral and unilateral lung disease group mice at day 28 were compared using Mann-Whitney and log-rank tests. Lungs were extracted and evaluated using Masson trichrome staining.RESULTSBody weights decreased 75.7%±14.0% in the bilateral lung disease group, but were greater, 94.1%±11.4%, in the unilateral lung disease group (p=0.03). Overall survival rates at day 28 were 30.8% and 85.7% in the bilateral and unilateral lung disease groups, respectively. Survival was significantly better in the unilateral lung disease model (p=0.01). Histological evaluation confirmed collagen deposition only in the bleomycin injected lung in the unilateral lung disease model.CONCLUSIONEstablishing both a healthy and a diseased lung in the same individual model was feasible, achieving lessened body weight loss and more favorable survival. This technique allows for a more efficacious research design, where both the efficacy and adverse effects of a pharmaceutical agent can be evaluated in a single animal.

Oral Commissure Ulcer in an Extremely Preterm Infant: A Complication of Endotracheal Tube Fixation at Contralateral Oral Commissure During Lateral Positioning

Lip ulcers associated with endotracheal tube fixation are a known complication in adults, but their prevalence in neonates and preterm infants remains unclear. We report a case of a right oral commissure ulcer that developed during endotracheal tube fixation at the right oral commissure and left lateral decubitus positioning in an extremely preterm infant with unilateral pulmonary interstitial emphysema (PIE). A male infant was born at 24 weeks and four days of gestation, weighing 696 gm. He required mechanical ventilation for respiratory distress syndrome from birth. On day 66 of life, left lateral positioning and selective right main bronchial intubation were initiated to manage left-sided PIE. The endotracheal tube had initially been fixed at the center or left oral commissure, but on day 101 of life, it was switched to right oral commissure fixation. Eleven days later, an ulcer was observed at the right oral commissure. The ulcer was sutured under local anesthesia. The infant was extubated on day 122 of life and discharged on day 180 of life. The ulcer healed with minimal scarring. This case suggests that combining endotracheal tube fixation at the oral commissure with contralateral positioning may increase the risk of ulcer formation due to opposing forces from gravity and fixation, resulting in shear stress. Additionally, restricted positioning and continuous sedation may delay the early detection of ulcers. When using lateral positioning, we recommend avoiding contralateral oral commissure fixation of endotracheal tubes and securing the tube to the ipsilateral commissure or upper lip instead.
