Introduction

A rare disorder of mineral absorption and transport characterised by hypocupraemia that manifests as failure to thrive, mild anaemia, repeated seizures, hypotonia and seborrhoeic skin. Spurring of the femur and tibia are also noted on radiographic imaging. Symptoms are reversible or improve with supplements of oral copper. There have been no further descriptions in the literature since 1982.

Copper deficiency-associated myelopathy in cryptogenic hyperzincemia: a case report.

Copper deficiency syndrome is an underestimated cause of posterior myelitis. We describe the case of a 41-year-old woman, who developed a subacute ataxic paraparesis associated with low back pain. Her 3T spine MRI showed a thin hyperintense FS-Echo T2 longitudinally extensive lesion involving the posterior columns of the cervical cord (from C2 to C6). An extensive diagnostic work-up excluded other causes of myelopathy and blood tests pointed out hypocupremia and mild hyperzincemia. Patients affected by this rare form of oligoelement deficiency typically develop progressive posterior column dysfunction with sensory ataxia and spasticity, sometimes associated with sensori-motor polyneuropathy. Clinical and radiological characteristics of posterior myelopathy due to copper deficiency are briefly reviewed. Physicians should be aware of this condition since a prompt introduction of copper supplementation can avoid progression of the neurological damage.

Fatal Copper Deficiency From Excessive Use of Zinc-Based Denture Adhesive

INTRODUCTIONHuman stores of zinc and copper exist in dynamic equilibrium; an increase in one causes a fall in the other, and clinical consequences of toxicity or deficiency of one or the other may result. Some of the most widely used denture adhesives are zinc based, creating a potential for zinc toxicity and corresponding copper deficiency. A case of denture adhesive-based zinc toxicity with corresponding copper deficiency leading to fatal ascending sensorimotor polyneuropathy was identified. The objectives of this study were to illustrate the evolution, and disparate response to treatment, of neurologic and hematologic abnormalities resulting from copper deficiency, and to discuss opportunities to mitigate denture adhesive-related zinc toxicity.METHODSDetailed clinical and laboratory data for the subject patient were compiled. The patient received copper supplementation. Copper and zinc levels were obtained posttreatment at varying intervals.RESULTSHematologic and neurologic abnormalities progressed, as excessive use of zinc-based denture adhesive persisted. Hematologic and neurologic consultants were initially considered purely hematologic or neurologic diagnoses. Eventual consideration of unifying hypotheses led to definitive diagnosis 10 months after presentation. Hematologic abnormalities responded to copper supplementation, but neurologic abnormalities did not. The patient died of aspiration likely due to severe sensorimotor polyneuropathy.CONCLUSIONSEarly recognition of copper deficiency and improved warnings regarding excessive use of zinc-based denture adhesives may be the routes to improved outcomes.

Copper Deficiency After Gastric Surgery: A Reason for Caution

BACKGROUND: Acquired copper deficiency in adults leads to hematological and neurological manifestations that mimic vitamin B12 deficiency. A significant number of patients with copper deficiency syndrome have a history of gastric surgery, often remote. We sought to determine whether copper deficiency is present in a population of individuals with longstanding partial gastric resection.METHODS: Serum copper, ceruloplasmin, and zinc levels were determined in 20 patients with a history of partial gastric resection and 50 controls, randomly selected from the Oklahoma City Veterans Affairs Medical Center electronic database.RESULTS: Hypocupremia and symptoms of copper deficiency were detected in patients with partial gastric resection in contrast to controls (3/20 versus 0/50, P = 0.02). Serum copper and ceruloplasmin levels were significantly lower in individuals with partial gastric resection than in controls (P = 0.04 and P = 0.001, respectively). The mean interval between gastric surgery and testing was 20.7 years.CONCLUSIONS: Our results indicate that a significant number of individuals with longstanding history of partial gastric resection have undiagnosed hypocupremia. Screening for copper deficiency after gastric surgery may prevent the development of hematological and neurological complications in these patients.
