Remineralization Potential Of Egg Shell Hydrogel Versus Fluoride Varnish On Early Carious White Spot Lesions In Anterior Primary Teeth: A Randomized Controlled Trial

The goal of this clinical trial is to to assess the remineralization potential of Egg shell hydrogel versus fluoride varnish in early carious white spot lesions in anterior primary teeth.
The main question it aims to answer is:What is the remineralization potential of egg shell hydrogel on early carious white spot lesions compared to fluoride varnish in anterior primary teeth? Researcher will compare egg shell hydrogel with fluoride varnish to see if it is able to promote lesion remineralization.

Effect of Artificial Intelligence-Generated WhatsApp Reminders Compared to No Reminders on Oral Health Among Young Adults: A Randomized Controlled Trial

This randomized controlled trial aims to evaluate the effectiveness of artificial intelligence-generated WhatsApp reminders on improving oral hygiene practices among individuals aged 20-40 years visiting the periodontal clinic at El-Qasr El-Ainy Dental Hospital. Participants will be randomly assigned to either receive health education followed by AI-generated WhatsApp reminders over a 3-month period or to a control group receiving health education only. The primary outcome is the presence of white spot lesions, assessed using the modified Gorelick White Spot Lesion (WSL) index. Secondary outcomes include the presence of dental plaque, measured by the Plaque Index (PI). This study seeks to determine whether AI-generated reminders can enhance oral hygiene adherence compared to standard health education alone.

Enamel conservation orthodontic paradigm via treatment with newly developed remineralizing calcium-phosphate etchant pastes

OBJECTIVETo evaluate recently developed acidic calcium-phosphate (CaP) pastes as conservative enamel-conditioning systems for bracket bonding and investigate their effects on the shear bond strength, adhesive residues, and enamel damage in comparison to a conventional 37% phosphoric acid (PA) gel.MATERIALS AND METHODSTwo experimental etchant pastes consisting of hydroxyapatite and monocalcium phosphate monohydrate were prepared by mixing them with 37% and 10% PA solutions, respectively. These were characterized using X-ray diffraction. Metal brackets were bonded to 90 freshly extracted human premolars and assigned to three groups, depending on etchant type: 37% PA-gel (control) and the two experimental (HPA and MPA) pastes. Shear bond strengths (SBS), adhesive remnant index scores, and enamel damage for the three groups were compared at 24 h water storage and after 5000 thermocycles. Enamel etch pattern, surface damage, and CaP precipitation were evaluated through FE-SEM. Surface Vickers micro-hardness (VHN) following etching procedures was compared to the unetched enamel surface.RESULTSHPA and MPA pastes yielded significantly lower SBS values than 37% PA-gel (p < .001), induced a milder etch pattern, unblemished enamel surfaces and CaP re-precipitation with no or minimal adhesive residues post-bracket debonding as compared to 37% PA-gel. Moreover, the HPA paste-etched enamel revealed the highest (p < .001) micro-hardness value (232 VHN).CONCLUSIONSEnamel conditioning using the CaP pastes, particularly the HPA, may evoke CaP re-precipitation and cause minimal enamel damage, with greater efficiency at bracket debonding/enamel cleaning procedures. Furthermore, it yields low adhesive residues and potentially adequate bond strength for clinical performance in comparison to conventional PA gel.CLINICAL SIGNIFICANCEEnamel etching with the developed paste prior to orthodontic bonding might lessen the development of white spot lesions during treatment, with the potential of reducing the prolonged chair time for clean-up and polishing usually required whenever PA is used.

Prognostic factors and prognostic models for white spot lesions: A systematic review and meta-analysis

OBJECTIVESTo summarize the existing evidence on prognostic factors (PFs) and prognostic models (PMs) for white spot lesions (WSLs) in orthodontic patients.SOURCESElectronic searches were conducted across PubMed, Embase, Cochrane Library, Web of Science, and Scopus (last search date: December 19th, 2024), supplemented by grey sources (Google Scholar and ProQuest). Manual searches were performed by checking the reference lists of key relevant publications.STUDY SELECTIONCohort and case-control studies reporting PFs and PMs for WSLs in orthodontic patients were included. The risk of bias was assessed with QUIPS and PROBAST tools for PF and PM studies, respectively. Prognostic estimates were pooled with risk ratio (RR) using the random effects model. The certainty of the evidence was assessed with an adapted GRADE system.DATAThirty-two PF and one PM studies were included, involving 5101 participants and investigating 31 PFs and 1 PM. Pretreatment lesions [RR=3.87 (95 % CI, 2.06 to 7.27)] and pretreatment oral hygiene condition [RR=1.86 (95 % CI 1.20 to 2.88)] were significant PFs for WSL incidence, whereas there is insufficient evidence for gender and treatment duration. Only 1 PM was identified, which was developed via Fisher's discriminatory analysis without adjustment and validation. The certainty of evidence was rated as very low or low.CONCLUSIONS/CLINICAL SIGNIFICANCEBased on low and very low certainty evidence, the existing lesions and oral hygiene condition before the commencement of orthodontic treatment are significant PFs for WSL incidence. Comprehensive pretreatment assessments are vital for preventing WSLs in orthodontic patients. The development of a novel, validated PM for WSL risk assessment is warranted.

Evaluation of NaOCl application prior to resin infiltrant or fluoride-containing resin varnish in the treatment of white spot lesions: An in vitro study

OBJECTIVEThe aim of this in vitro study is to investigate the effect of NaOCl (5.25 %) prior to resin infiltration and resin varnish on microhardness, surface roughness, and DIAGNOdent Pen values in the treatment of white spot lesions (WSLs).METHODSThe artificial initial caries lesions were created in 160 human enamel samples. The demineralizated samples were divided into five groups: resin infiltration (ICON; DMG, Hamburg, Germany), NaOCl + ICON, resin varnish (Clinpro XT; 3M ESPE, Seefeld, Germany), NaOCl + Clinpro XT, and a negative control group. Enamel samples that underwent thermocycling aging were subjected to 5.000 cycles between 5-55 °C (N = 32, n = 16 for aging subgroups). Surface roughness (Ra), Vicker's microhardness (VHN), and DIAGNOdent Pen (Kavo, Biberach, Germany) (DDP) values were recorded at the baseline, after demineralization, after treatment, and after the thermal aging stages. The micro-CT analysis, microleakage, and Scanning Electron Microscope images were evaluated after treatment procedures and thermal aging. Robust ANOVA, Pearson's chi-square, and McNemar tests were used for statistical analysis. The significance level was p < 0.05.RESULTSThe main effect of both group and stage was statistically significant for DDP, Ra, VHN, and micro-CT values. All treatment procedures improved the demineralized enamel compared to the negative control group (p < 0.001). The application of NaOCl (5.25 %) before resin infiltration increased microhardness but also in roughness. The resin-infiltrated groups showed less microleakage than the resin-varnish-treated groups (p < 0.001).CONCLUSIONThe application of NaOCl prior to resin infiltration or resin varnish did not result in any important alterations in the examined parameter values.
