IMPORTANT SAFETY INFORMATION CONTRAINDICATIONS Antepartum use of Pitocin is contraindicated in any of the following circumstances: Where there is significant cephalopelvic disproportion; In unfavorable fetal positions or presentations, such as transverse lies, which are undeliverable without conversion prior to delivery; In obstetrical emergencies where the benefit-to-risk ratio for either the fetus or the mother favors surgical intervention; In fetal distress where delivery is not imminent; Where adequate uterine activity fails to achieve satisfactory progress; Where the uterus is already hyperactive or hypertonic; In cases where vaginal delivery is contraindicated, such as invasive cervical carcinoma, active herpes genitalis, total placenta previa, vasa previa, and cord presentation or prolapse of the cord; In patients with hypersensitivity to the drug. WARNINGS Pitocin, when given for induction of labor or augmentation of uterine activity, should be administered only by the intravenous route and with adequate medical supervision in a hospital. PRECAUTIONS All patients receiving intravenous oxytocin must be under continuous observation by trained personnel who have a thorough knowledge of the drug and are qualified to identify complications. Electronic fetal monitoring provides the best means for early detection of overdosage. When properly administered, oxytocin should stimulate uterine contractions comparable to those seen in normal labor. Overstimulation of the uterus by improper administration can be hazardous to both mother and fetus. Even with proper administration and adequate supervision, hypertonic contractions can occur in patients whose uteri are hypersensitive to oxytocin. This fact must be considered by the physician in exercising his judgment regarding patient selection. Oxytocin should not be administered in the following conditions: fetal distress, hydramnios, partial placenta previa, prematurity, borderline cephalopelvic disproportion, and any condition in which there is a predisposition for uterine rupture, such as previous major surgery on the cervix or uterus including cesarean section, overdistention of the uterus, grand multiparity, or past history of uterine sepsis or of traumatic delivery. The decision can be made only by carefully weighing the potential benefits which oxytocin can provide in a given case against rare but definite potential for the drug to produce hypertonicity or tetanic spasm. Maternal deaths due to hypertensive episodes, subarachnoid hemorrhage, rupture of the uterus, and fetal deaths due to various causes have been reported associated with the use of parenteral oxytocic drugs for induction of labor or for augmentation in the first and second stages of labor. Oxytocin has been shown to have an intrinsic antidiuretic effect, acting to increase water reabsorption from the glomerular filtrate. Consideration should, therefore, be given to the possibility of water intoxication, particularly when oxytocin is administered continuously by infusion and the patient is receiving fluids by mouth. When oxytocin is used for induction or reinforcement of already existent labor, patients should be carefully selected. Pelvic adequacy must be considered and maternal and fetal conditions evaluated before use of the drug. ADVERSE REACTIONS The following adverse reactions have been reported in the mother: Anaphylactic reaction, postpartum hemorrhage, cardiac arrhythmia, fatal afibrinogenemia, nausea, vomiting, premature ventricular contractions, pelvic hematoma, subarachnoid hemorrhage, hypertensive episodes, and rupture of the uterus. The following adverse reactions have been reported in the fetus or neonate: Due to induced uterine motility: Bradycardia, premature ventricular contractions and other arrhythmias, permanent CNS or brain damage, fetal death, and neonatal seizures have been reported with the use of Pitocin. Due to use of oxytocin in the mother: Low Apgar scores at five minutes, neonatal jaundice, and neonatal retinal hemorrhage. DRUG INTERACTIONS Severe hypertension has been reported when oxytocin was given three to four hours following prophylactic administration of a vasoconstrictor in conjunction with caudal block anesthesia. Cyclopropane anesthesia may modify oxytocin's cardiovascular effects, so as to produce unexpected results such as hypotension. Maternal sinus bradycardia with abnormal atrioventricular rhythms has also been noted when oxytocin was used concomitantly with cyclopropane anesthesia. INDICATIONS AND USAGE Antepartum: Pitocin is indicated for the initiation or improvement of uterine contractions, where this is desirable and considered suitable for reasons of fetal or maternal concern, in order to achieve vaginal delivery. It is indicated for (1) induction of labor in patients with a medical indication for the initiation of labor, such as Rh problems, maternal diabetes, preeclampsia at or near term, when delivery is in the best interests of mother and fetus or when membranes are prematurely ruptured and delivery is indicated; (2) stimulation or reinforcement of labor, as in selected cases of uterine inertia; (3) as adjunctive therapy in the management of incomplete or inevitable abortion. In the first trimester, curettage is generally considered primary therapy. In second trimester abortion, oxytocin infusion will often be successful in emptying the uterus. Other means of therapy, however, may be required in such cases. THURSDAY, June 1, 2023 -- The number of American women who have diabetes when they become pregnant has increased dramatically over five years, health officials reported Wednesday. Between 2016 and 2021, the rate of pregnancy among diabetic women has risen 27%, from about 9 per 1,000 births to 11 per 1,000 births, according to the report from the U.S. Centers for Disease Control and Prevention. Primary reasons for this increase are most likely the ongoing obesity epidemic and the fact that more older women are having children. "The findings are consistent with the increased risk of type 2 diabetes among people who are overweight or obese," said lead researcher Elizabeth Gregory , a health scientist at CDC's National Center for Health Statistics (NCHS). "And when we looked at other studies, there was an increasing rate of pre-pregnancy diabetes with advancing maternal age," Gregory said. One expert said these trends do indeed appear to be driving the increase in pre-pregnancy diabetes cases. "The study illustrates that the same trends noted in the U.S. in prior years are ongoing, with diabetes being a major public concern," said Dr. Eran Bornstein , director of obstetrics and gynecology at Lenox Hill Hospital in New York City. "Despite knowledge of these data and associated risk factors, public health interventions to date have not been sufficient in halting this concerning trend," Bornstein said. "These data call for increased public health efforts to target interventions such as education on nutrition and physical activity, as well as facilitation of healthy lifestyle with specific targeting of the populations that are at risk." In 2021, the rate of diabetic women getting pregnant varied from a low of 9 per 1,000 births among white women to a high of nearly 29 per 1,000 births among American Indian or Alaska Native women, the study found. The rate also rose among women aged 40 and older and obese women, the researchers reported. And it varied by state, from less than 9 per 1,000 births in Hawaii, Wyoming, Utah and Colorado to a high of 15 per 1,000 births or more in Tennessee, Vermont and New Mexico. For the study, the investigators used data on U.S. birth certificates from 2016 to 2021. The findings were published May 31 as an NCHS data brief, the National Vital Statistics Reports . "These findings are worrisome, as being diabetic before pregnancy can cause complications for both mother and baby," said Dr. Sarah Pachtman , a maternal/fetal physician at Katz Women’s Hospital of LIJ Medical Center in New Hyde Park, N.Y. "This is concerning because we know that a diagnosis of diabetes before pregnancy puts pregnancies at increased risk including a higher chance of preeclampsia, fetal growth restriction and stillbirth on one spectrum, and large-for-gestational-age neonates [newborns] and polyhydramnios [excess of amniotic fluid] on the other spectrum," she said. Moreover, poorly controlled diabetes before pregnancy is a risk factor for birth defects, especially of the heart, brain and spinal cord, Pachtman added. "Pre-pregnancy diabetes is one of the most important diagnoses to get under control before getting pregnant to ensure the healthiest possible pregnancy," she said. Researchers at the National Institutes of Health (NIH) have developed a prototype device that could potentially diagnose pregnancy complications by monitoring the oxygen level of the placenta. The device sends near-infrared light through the abdomen to measure oxygen levels in the arterial and venous network in the placenta. The method was used to study the anterior placenta, which is attached to the front wall of the uterus. The researchers described their results as promising but added that further study is needed before the device could be used routinely, according to an NIH news release. The small study was conducted by Amir Gandjbakhche, Ph.D., of the Section on Translational Biophotonics at NIH’s Eunice Kennedy Shriver National Institute of Child Health and Human Development (NICHD), and colleagues. It appears in Biomedical Optics Express. The researchers devised mathematical methods to study the passage of light through the skin, abdominal wall and uterine tissue to reach the placenta and calculate its oxygen levels. Currently, the device cannot monitor oxygen in women with a posterior placenta, which is attached to the back wall of the uterus, because the distance is too far for the light to travel. However, an anterior placenta is associated with a higher rate of complications than a posterior placenta, such as postpartum hemorrhage and an increased need for labor induction or cesarean delivery. The researchers enrolled 12 pregnant women with an anterior placenta in the study. Of these, five had a pregnancy complication, including hypertension, a short cervix and polyhydramnios (excess amniotic fluid). On average, the women with complications had a placental oxygen level of 69.6%, a difference statistically significant from the 75.3% seen in the healthy pregnancies in the study. The authors see their results as a first step in continuously monitoring placental oxygen levels to assess maternal and fetal health, the agency said.
