Introduction

Solitary or multiple benign hepatic vascular tumors, usually occurring in women of 20-50 years of age. The nodule, poorly encapsulated, consists of a central stellate fibrous scar and normal liver elements such as HEPATOCYTES, small BILE DUCTS, and KUPFFER CELLS among the intervening fibrous septa. The pale colored central scar represents large blood vessels with hyperplastic fibromuscular layer and narrowing lumen.

"The Fingerprint: Spoke-wheel Sign" of Focal Nodular Hyperplasia: Assessment of Novel Ultrasound Approach Using Superb Microvascular Imaging Technique

Focal nodular hyperplasia (FNH) in liver is the second common benign hepatic tumor. It usually shows hypervascular mass on imaging studies and it is not easy to differentiate with other hypervascular malignant tumor. For diagnosis of FNH, contrast-enhanced ultrasonography (US) has been used to detect 'spoke-wheel sign', which can be typically seen in FNH. However, temporal window of vascular phase using contrast-enhanced US (CEUS) is very short (about 10 sec) and coordination of patient's respiration during US exam is absolutely needed. Thus, the investigators will use Superb-Microvascular imaging (SMI, Toshiba, Japan) for detection of 'spoke-wheel sign' in patients with proven FNH, which enable to detect slow micro vascular flow without using CEUS.
First, to compare the detection rate of 'spoke-wheel sign' between CEUS using sonazoid (Perfluorobutane, GE healthcare) and SMI.
Second, to compare the accuracy of size measurement between gray-scale US and SMI (reference standard: CEUS using sonazoid.)

EX vivo liver resection and auto transplantation for a giant hepatocellular carcinoma: A case report

INTRODUCTION AND IMPORTANCEThe curative options for locally advanced hepatocellular carcinomas involving the vessels are very limited. Ex vivo liver resection and auto transplantation (ERAT) is a novel surgical method, which has been used in the management of such complex liver space occupying lesions, which are rendered conventionally unresectable. This technique involves a complete hepatectomy, extracorporeal hepatic resection on the backbench, and then reimplantation of the remnant hepatic parenchyma. George et al. (2019) [1] In literature, Ex vivo liver resection and auto transplantation (ERAT) has been utilized in the management of both benign and malignant liver lesions, including hepatocellular carcinoma (HCC), cholangiocarcinoma, hepatic metastases, hepatic alveolar echinococcosis, focal nodular hyperplasia and hemangioma. Zawistowski et al. (2020) [2].CASE PRESENTATIONWe report this case of a 28 years old male with a giant hepatocellular carcinoma, that deemed unresectable as it was closely abutting all three hepatic veins. After appropriate preoperative surgical planning, he underwent ex vivo liver resection and autotransplantation (ERAT). Patient had gradual uneventful recovery and he was discharged home on 12th postoperative day.CLINICAL DISCUSSIONA radical (R0) resection is the main goal of the ex vivo liver resection and autotransplantation (ERAT) procedure, which can be used for curative intent in patients with liver lesions that are conventionally inoperable. It allows for precise extracorporeal resection, maximizes the residual liver volume and increases the likelihood of obtaining negative margins. Furthermore, the bench surgery in ice bath makes it easier to reconstruct the vasculature without the pressure of time constraints in situ, unlike conventional surgery, which can take a long time when done in a bleeding operative field. Zawistowski et al. (2020) [2].CONCLUSIONFor patients with complex liver lesions believed to be incurable with conventional surgical methods, ex vivo liver resection and autotransplantation (ERAT) provides an alternative.

Membranoproliferative glomerulonephritis in a child with congenital portosystemic shunt

Congenital portosystemic shunts (CPSS) are rare congenital vascular anomalies characterized by abnormal connections between the portal vein and systemic circulation, bypassing the liver. They can lead to complications such as recurrent encephalopathy, liver nodules, portopulmonary hypertension, and neurocognitive issues due to hyperammonemia and rarely kidney involvement. Hepatic hemodynamic changes can lead to liver nodules and hepatocellular carcinoma, particularly in extrahepatic shunts. We describe here an 11-year-old girl with type 1 intrahepatic portosystemic shunt with focal nodular hyperplasia in the liver, presenting with nephrotic syndrome that was diagnosed as membranoproliferative glomerulonephritis on kidney biopsy and that responded partially to therapy with immunosuppressants.
