Introduction

The twisting of the SPERMATIC CORD due to an anatomical abnormality that left the TESTIS mobile and dangling in the SCROTUM. The initial effect of testicular torsion is obstruction of venous return. Depending on the duration and degree of cord rotation, testicular symptoms range from EDEMA to interrupted arterial flow and testicular pain. If blood flow to testis is absent for 4 to 6 h, SPERMATOGENESIS may be permanently lost.

"Comparison of Analgesic Effects of USG-Guided Caudal Versus Ilioinguinal/Iliohypogastric Nerve Block Techniques for Inguinal Surgeries in Children, a Randomized Controlled Trial."

This study is designed to compare the analgesic effectiveness of caudal analgesia to ilioinguinal regional analgesia techniques in children undergoing inguinal surgeries. Both techniques will be done under ultrasound gaudiness, using the same local anesthesia. 128 patients will be included in this study, 64 for each technique. This study aims to know the best regional analgesia technique in children undergoing inguinal surgeries, reduction in rescue analgesia postoperatively, postoperative length of stay, and early resumption of postoperative activity.
The patients will be followed up 30 minutes after the end of anesthesia in PACU, postoperatively in Ward at 2 Hours, 3 hours, 6 hours from the end of anesthesia, and or at the time of discharge from the ward & on days 2 and 7. Patient privacy and safety will be respected at all times. An interim analysis will be done after recruiting 25% of cases (16 cases) in both arms. Once the sample size is reached, the data will be sent for statistical analysis with coded patient identity.
Based on study findings, the practice will be optimized with the aim of improvement in pain relief, reduction in opioid requirements, and enhanced recovery after surgery (ERAS), reducing the bed occupancy time in the hospital.

Rare case of left epididymo-orchitis complicated by pampiniform plexus thrombosis: A case report

Acute scrotal pain includes urgent conditions in urology, such as testicular torsion, testicular rupture, epididymo-orchitis, and abscess. However, varicocele (pampiniform plexus) thrombosis is considered to be a rare cause of such pain. We herein report a case of a 27-year-old male patient with a history of epididymo-orchitis, who complained of painful scrotal swelling. Ultrasonography showed left-side pampiniform plexus thrombosis. This case highlights a rare condition, which should be included in the differential diagnosis of acute scrotal pain, indicating the need for further studies to elucidate its pathophysiology and provide proper treatment for such cases.

Inequities and Progress in Testicular Torsion Care Following a US News & World Report Metric: A Retrospective Cohort Study

BACKGROUND AND OBJECTIVESPediatric testicular torsion is a surgical emergency with known health disparities. The US News and World Report is an American media company that ranks colleges and hospitals based on annually published criteria. Hospitals that complied with the US News and World Report's (USNWR) Speed in Treating Testicular Torsion (SiTTT) metric saw improved surgical testicular salvage rates, but the metric's impact in mitigating inequities in testicular torsion is unstudied.METHODSA retrospective cohort study of patients 1-18 years of age presenting with testicular torsion was performed using the Pediatric Health Information System database. Hospitals were classified based on their SiTTT score at the metric's onset in 2015: they were labeled complete-scoring if they achieved the maximum score and partial-scoring otherwise. Surgical testicular salvage was defined as orchiopexy without concomitant orchiectomy. Testicular salvage rates were compared based on various socioeconomic variables including race between the pre-metric (2010-2015) and post-metric (2015-2019) periods with both univariate and adjusted, multivariate analyses.RESULTSWe identified 3950 cases of testicular torsion: 2335 in complete-scoring hospitals and 1615 in partial-scoring hospitals. Only partial-scoring hospitals improved significantly in salvage rate (pre-metric 61.0 % vs post-metric 67.5 %, p < 0.01). Subgroup analysis revealed the largest benefit in patients with economic or communicative barriers such as those with complex chronic conditions (pre-metric 35.3 % vs post-metric 61.1 %, p < 0.05). No minority races had statistically significant improvement after the metric, and salvage rate gap between White and all other races increased (pre-metric 4.3 % vs post-metric 10.0 %, p < 0.05).CONCLUSIONSPatients with communicative or economic barriers showed improved equity after efforts to comply with a third-party metric, but disparities persisted for minority races. OBJECTIVEOur aim was to examine the effectiveness of human breast milk exosomes in treating testicular torsion.METHODSRats were divided randomly into three equal groups. Group 1 was our control group. A torsion/detorsion model was created for the rats in Group 2. After the torsion/detorsion model, 1 mg/kg intraperitoneally human breast milk exosomes were given 30 min before detorsion in Group 3. All testicles were removed at the 6^th h of the experiment, for biochemical and histopathological examination.RESULTSThere was a statistically significant decrease in Group 3 MDA values compared to Group 2. When Group 3 and Group 2 SOD and CAT values were compared, it was seen that these values were higher in Group 3. Histopathologically a statistically significant difference was found in Group 3 compared to Group 1 and Group 2 in terms of ischemia/reperfusion injury.CONCLUSIONWe have shown that human breast milk exosomes in testicular torsion do not completely prevent reperfusion injury but significantly preserve testicular tissue.
