Establishment and Evaluation of Clinical Classification and Surgical Decision-making System for Biliary Dilatation：a National Special Disease Cohort Study in China

This study is a multicenter, bidirectional cohort study aimed at continuously enrolling patients with biliary dilatation from 25 medical centers in China. It will collect comprehensive life-cycle data from the cohort to establish a Chinese cohort for bile duct dilatation. Based on this cohort, the study seeks to clarify the epidemiological characteristics, pathological features, standard classification, disease progression, cancer risk, and optimal timing for surgical intervention in patients with bile duct dilatation. Additionally, it will compare the perioperative risks, long-term outcomes, and quality of life among different types of patients (including Todani types I, IVa, and V) following surgical treatment, to establish standardized surgical treatment strategies for each type.

Clinicopathologic features of 6 cases of segmental atrophy of liver and review of the literature

Segmental atrophy (SA) of the liver is a rare and frequently under-recognized condition that can lead to the formation of pseudotumors, posing significant challenges in both clinical and imaging diagnoses. To date, this disease has been infrequently reported in the literature. To elucidate the clinicopathological characteristics of SA and enhance clinicians' understanding, we collected clinical and pathological data from 6 cases of SA and summarized the clinical information and pathological features of 22 previously reported SA patients. Among all cases, 19 patients were female and 9 were male, with a median age of 60 years. Lesion sizes varied widely, ranging from 1.5 to 10.5 cm. 17 patients exhibited notable right upper quadrant abdominal pain. Histological examination revealed a sequence of changes: early-stage lesions exhibited varying degrees of hepatic parenchyma collapse, brisk bile ductular proliferation, and extensive infiltration of chronic inflammatory cells, accompanied by very mild elastosis. In subsequent stages, the lesions exhibited reduced bile duct proliferation and inflammatory response alongside a notable increase in elastic fibers. Advanced-stage lesions presented with extensive elastic fiber deposition, occasionally interspersed with scattered islands of hepatocytes, and the terminal phase manifested as dense fibrotic nodules. Abnormally thick-walled blood vessels were identified in nearly all cases (n = 25), and bile duct cysts were observed in some cases (n = 11). In summary, SA primarily affects middle-aged individuals, with a female predominance, and presents with nonspecific clinical symptoms, predominantly right upper quadrant pain. Pathologically, it displays a continuous histomorphological spectrum. Elastic fiber staining has proven beneficial for both diagnosis and differential diagnosis of this condition. Proficiency in recognizing its diverse morphological features is essential for accurate diagnosis.

Comparison of robotic versus laparoscopic cyst excision and hepaticojejunostomy for choledochal cyst in children: a propensity score-matched study

BACKGROUNDMinimally invasive surgery (MIS) has become a standard approach in pediatric surgery, including for the treatment of choledochal cysts (CC). This study compared the long-term outcomes of laparoscopic (LA) and robot-assisted (RA) surgeries for pediatric CC. Propensity score matching (PSM) was used to control for potential confounding variables that could influence surgical outcomes.METHODSA retrospective review of 159 pediatric patients who underwent MIS for CC at our tertiary referral center between June 2008 and December 2020 was conducted. PSM was used to minimize selection bias, resulting in 63 matched pairs of patients in the RA and LA groups. Key outcomes, including operative time, complication rates, and post-operative recovery, were compared between the two groups.RESULTSAfter PSM, the RA group had a longer mean operative time compared to the LA group (359 vs. 319 min, p = 0.006). However, the RA group had a significantly lower incidence of anastomotic leaks (0% vs. 7.9%, p = 0.023). There were no statistically significant differences between the groups in terms of conversion to open surgery, length of hospital stay, or severe complications. The RA group had a shorter time to start feeding and to achieve full feeding postoperatively.CONCLUSIONRA significantly enhances the quality of anastomosis, contributing to more secure anastomoses compared to LA, and provides the benefit of faster bowel movement recovery in pediatric choledochal cysts.
