Introduction

Inflammation of the periosteum. The condition is generally chronic, and is marked by tenderness and swelling of the bone and an aching pain. Acute periostitis is due to infection, is characterized by diffuse suppuration, severe pain, and constitutional symptoms, and usually results in necrosis. (Dorland, 27th ed)

Voriconazole-induced periostitis in the hand

Voriconazole, a triazole antifungal, has proven effective against invasive fungal infections, and is often selected due to its enhanced antifungal spectrum coverage. Despite its general tolerability, voriconazole usage is associated with drug-induced periostitis, which presents with diffuse bone pain. This case report details a 65-year-old male on chronic immunosuppressive and antimicrobial therapy following heart transplant who developed hand pain. Identification of periostitis on hand radiographs and an acute elevation of alkaline phosphatase on subsequent blood analysis prompted discontinuation of voriconazole, resulting in a symptomatic and laboratory improvement. This case underscores the importance of leveraging a thorough understanding of imaging features, clinical manifestations, and laboratory data to aid in the diagnosis of voriconazole-induced periostitis.

Generalized sarcoidosis associated with hypertrophic osteopathy in a Standardbred racehorse

A Standardbred racehorse was presented for exercise intolerance, weight loss, pyrexia and facial deformity. Radiography and ultrasonography revealed periostitis and regional soft tissue swelling of maxillary bones. Computed tomography excluded any dental or sinus origin of these abnormalities. Further deformities on distal limbs and skin lesions appeared during hospitalization. Radiography identified bilateral periostitis and soft tissue swelling in the distal radius and metatarsal bones, as observed in the head, suggestive of hypertrophic osteopathy (HO). Skin biopsies revealed granulomatous dermatitis, indicating equine sarcoidosis (ES). The horse was euthanized due to rapid deterioration of his condition. Post-mortem examination and histopathology revealed splenomegaly, several pulmonary nodules and disseminated granulomatous lesions. These findings confirmed the diagnosis of generalized ES, or equine idiopathic systemic granulomatous disease. The concurrent with HO was probably associated with the intrathoracic pathology. Polymerase chain reaction (PCR) on pooled tissues identified equine herpesvirus 5 (EHV-5) DNA.

An underdog under the mandible: Pantoea agglomerans abscess of the submandibular region- a case report

BACKGROUNDPantoea agglomerans is a gram negative, aerobic/facultative anaerobic, rod shaped bacilli commonly isolated from plants, soil, food and faeces.(1) It is a rare cause of opportunistic infections in humans acquired mainly via two major routes being, wound infection or hospital acquired.CASE REPORTHere, we encountered a landmark, first of its kind, head and neck manifestation of a cervical soft tissue abscess with Pantoea agglomerans being the miscreant. The patient presented with complaints of a left sided neck swelling, which was radiologically suggestive of a cold abscess, however clinical suscpicion encouraged us to perform an incision and drainage, culture of which revealed this notorious phytogenic bacterium.DISCUSSIONCommonly encountered Pantoea infected cases documented in literature have shown a clinical picture of endophthalmitis, acute unilateral dacryocystitis, periostitis, endocarditis, osteomyelitis and a tumour like muscle cyst of the thigh with many of them eventually leading to septicemia while a few also resolved with targeted antibiotics.(2) Remarkably, no ENT or head and neck presentations have been reported in literature till date. History of trauma by brushing against a mango tree was confirmed retrospectively, which was found to be the missing piece of the puzzle.
