Efficacy and Safety Pro Once-Daily Vemlidy Demonstrated in Children Six Years of Age (Weighing at Least 25kg) and Older FOSTER CITY, Calif.--(BUSINESS WIRE)-- Gilead Sciences, Inc. (Nasdaq: GILD) today announced that the U.S. Food and Drug Administration (FDA) has approved the supplemental new drug application (sNDA) for Vemlidy® (tenofovir alafenamide) 25 mg tablets as a once-daily treatment for chronic hepatitis B virus (HBV) infection in pediatric patients six years of age and older and weighing at least 25 kg with compensated liver disease. Vemlidy is a targeted prodrug of tenofovir that was approved by the FDA in 2016 as a once-daily treatment for adults with chronic HBV infection with compensated liver disease. In 2022, the FDA approved Vemlidy for the treatment of chronic HBV infection in pediatric patients 12 years of age and older with compensated liver disease. Vemlidy is recommended as a preferred or first-line treatment for adults with chronic HBV with compensated liver disease by the American Association for the Study of Liver Diseases (AASLD) and European Association for the Study of the Liver (EASL) guidelines. “Chronic hepatitis B can have a significant and lasting impact on the health of children. If left untreated, hepatitis B can lead to liver cirrhosis and liver cancer,” said Chaun-Hao Lin, MD, Associate Professor of Clinical Pediatrics Krek School of Medicine of USC. “As a clinician, I am well aware of the critical importance of promptly treating this disease to avoid possible complications and liver damage. The clinical trial demonstrated that tenofovir alafenamide may represent an effective treatment option for children as young as six years old affected by this chronic disease.” Vemlidy’s approval in this pediatric patient population is supported by Week 96 data from a Phase 2 clinical trial (Trial 1092) comparing treatment with Vemlidy 25 mg to placebo among 18 treatment-naïve and treatment-experienced patients aged 6 to less than 12 years weighing at least 25 kg (Cohort 2, Group 1). U.S. IMPORTANT SAFETY INFORMATION AND INDICATION FOR THE USE OF VEMLIDY BOXED WARNING: POSTTREATMENT SEVERE ACUTE EXACERBATION OF HEPATITIS B Discontinuation of anti-hepatitis B therapy, including VEMLIDY, may result in severe acute exacerbations of hepatitis B. Hepatic function should be monitored closely with both clinical and laboratory follow-up for at least several months in patients who discontinue anti-hepatitis B therapy, including VEMLIDY. If appropriate, resumption of anti-hepatitis B therapy may be warranted. Warnings and Precautions Risk of Development of HIV-1 Resistance in HBV/HIV-1 Coinfected Patients: Due to this risk, VEMLIDY alone should not be used for the treatment of HIV-1 infection. Safety and efficacy of VEMLIDY have not been established in HBV/HIV-1 coinfected patients. HIV antibody testing should be offered to all HBV-infected patients before initiating therapy with VEMLIDY, and if positive, an appropriate antiretroviral combination regimen that is recommended for HBV/HIV-1 coinfected patients should be used. New Onset or Worsening Renal Impairment: Postmarketing cases of renal impairment, including acute renal failure, proximal renal tubulopathy (PRT), and Fanconi syndrome, have been reported with TAF-containing products. Patients with impaired renal function and/or taking nephrotoxic agents (including NSAIDs) are at increased risk of renal-related adverse reactions. Discontinue VEMLIDY in patients who develop clinically significant decreases in renal function or evidence of Fanconi syndrome. Monitor renal function in all patients – See Dosage and Administration. Lactic Acidosis and Severe Hepatomegaly with Steatosis: Fatal cases have been reported with the use of nucleoside analogs, including tenofovir disoproxil fumarate (TDF). Discontinue VEMLIDY if clinical or laboratory findings suggestive of lactic acidosis or pronounced hepatotoxicity develop, including hepatomegaly and steatosis in the absence of marked transaminase elevations. Adverse Reactions Most common adverse events in the week 96 pediatric population reported in ≥5% were: nasopharyngitis, headache, COVID-19, pyrexia, diarrhea, upper respiratory tract infection, cough, respiratory tract infection viral, abdominal pain upper. Overall, abdominal pain upper and metabolic nephropathy were the only study drug–related adverse events, which occurred in > 1 participant, reported in 2.3% (2/88 participants) each. Drug Interactions Coadministration of VEMLIDY with drugs that reduce renal function or compete for active tubular secretion may increase concentrations of tenofovir and the risk of adverse reactions. Coadministration of VEMLIDY is not recommended with the following: oxcarbazepine, phenobarbital, phenytoin, rifabutin, rifampin, rifapentine, or St. John’s wort. Such coadministration is expected to decrease the concentration of tenofovir alafenamide, reducing the therapeutic effect of VEMLIDY. Drugs that strongly affect P-glycoprotein (P-gp) and breast cancer resistance protein (BCRP) activity may lead to changes in VEMLIDY absorption. Coadministration of VEMLIDY with carbamazepine, the tenofovir alafenamide dose should be increased to two tablets once daily. Dosage and Administration Testing Prior to Initiation: HIV infection. Prior to or When Initiating, and During Treatment: On a clinically appropriate schedule, assess serum creatinine, estimated creatinine clearance, urine glucose, and urine protein in all patients. In patients with chronic kidney disease, also assess serum phosphorus. Dosage: 1 tablet taken once daily with food. Renal Impairment: Not recommended in patients with end-stage renal disease (ESRD; eCrCl <15 mL/min) who are not receiving chronic hemodialysis; in patients on chronic hemodialysis, on hemodialysis days, administer VEMLIDY after completion of hemodialysis treatment. Indication VEMLIDY is indicated for the treatment of chronic hepatitis B virus infection in adults and pediatric patients six years of age and older and weighing at least 25 kg with compensated liver disease. About the 1092 Study Study 1092 is an ongoing Phase 2 clinical trial that randomized 88 treatment-naïve and treatment-experienced patients with chronic hepatitis B infection between the ages of 12 to less than 18 years weighing at least 35 kg to receive either Vemlidy 25 mg (N=47) or placebo (N=23) and 6 to less than 12 years weighing at least 25 kg to receive either Vemlidy 25 mg (N = 12) or placebo (N=6) once daily. The study met its primary endpoint of percentage of patients with HBV DNA levels below 20 IU/mL at 24 weeks of therapy; overall, 19% (11/59) of patients treated with Vemlidy 25 mg achieved HBV DNA < 20 IU/mL at Week 24 compared to 0% (0/23) of patients treated with placebo. The overall percentage of patients with HBV DNA < 20 IU/mL progressively increased in the Vemlidy 25 mg group relative to the Week 24 time point at both Week 48 (37% [22/59 subjects]) and Week 96 (61.0% [36/59 subjects]). The overall median change from baseline in ALT values for the Vemlidy and placebo-Vemlidy treatment groups, respectively, was -39.5 U/L and -46.5 U/L at Week 96. At 96 weeks, ALT normalization (AASLD criteria) was achieved for 54% of Vemlidy-treated patients and 57% of patients who switched from placebo to Vemlidy. Serum ALT is an enzyme used to monitor liver damage. Importantly, the mean percent changes in bone mineral density from baseline to Week 24 were similar for Vemlidy-treated patients and placebo-treated patients (1.6% (N=48) and 1.9% (N=23) for lumbar spine, and 1.9% (N=50) and 2.0% (N=23) for whole body, respectively). At Week 24, mean changes from baseline BMD Z-scores were 0.01 and -0.07 for the lumbar spine, and -0.04 and -0.04 for whole body, for Vemlidy and placebo groups, respectively. Researchers found that the oral bacterium Streptococcus gordonii activates another bacterial species, Fusobacterium nucleatum, to produce large quantities of methyl mercaptan, a compound responsible for bad breath. Disrupting this interaction could therefore help treat halitosis, and possibly also help prevent the development of more serious tooth and gum disease. Having bad breath really stinks! Now, researchers from Japan report that a particular combination of bacteria in our mouths may be responsible for producing some very unpleasant smells. In a study published last month in mSystems, researchers from Osaka University revealed that the interaction between two common types of oral bacteria leads to the production of a chemical compound that is a major cause of smelly breath. Bad breath is caused by volatile compounds that are produced when bacteria in the mouth digest substances like blood and food particles. One of the smelliest of these compounds is methyl mercaptan (CH3SH), which is produced by microbes that live around the teeth and on the surface of the tongue. However, little is known about which specific bacterial species are involved in this process. "Most previous studies investigating CH3SH-producing oral bacteria have used isolated enzymes or relatively small culture volumes," explains lead author of the study Takeshi Hara. "In this study, we aimed to create a more realistic environment in which to investigate CH3SH production by major oral bacteria." To do this, the researchers developed a large-volume anaerobic co-culture system that enabled them to test interactions between multiple different types of bacteria that live in the mouth. This system was able to test both direct, physical interactions among the bacteria, as well as whether these species could affect each other from a distance, for example by secreting active substances. "The results were very intriguing," says Masae Kuboniwa, senior author. "We found that Fusobacterium nucleatum produces large quantities of CH3SH in response to Streptococcus gordonii, another oral bacterium." By using stable isotope tracers and analyzing gene expression, the researchers showed that S. gordonii releases a substance called ornithine that prompts F. nucleatum to produce more of a molecule called polyamine. Because F. nucleatum needs methionine to produce polyamine, this enhanced polyamine production activates its methionine salvage pathway, which in turn results in increased CH3SH production. "Taken together, these findings suggest that CH3SH production in the mouth is driven by the interaction between S. gordonii and F. nucleatum," says Hara. Understanding how these two bacterial species work together to cause bad breath could be helpful in developing ways to treat or even prevent bad breath. In addition, given that bad breath is often associated with periodontal disease, treating this symptom early could help prevent more serious damage in the future.

A new study has uncovered the word properties that make slogans effective, as the researchers found that the attributes that make a slogan easier to process lead to it being more likeable but less memorable, and vice versa. A new study from Bayes Business School (formerly Cass), the University of Missouri and the University of Arizona has uncovered the word properties that make slogans effective, as the researchers found that the attributes that make a slogan easier to process lead to it being more likeable but less memorable, and vice versa. Previous research has recommended that slogans should be creative or capture the soul of the brand, but the authors found that people preferred slogans that are shorter, omit the brand name, and use words that are linguistically frequent and abstract. In contrast, slogans are less liked, but better remembered, if they are long, include the brand name, and feature unusual and concrete words. The paper, co-authored by Professor Zachary Estes, Professor of Marketing at Bayes, sheds light for the first time on the trade-offs that brands face when crafting a new slogan. It also offers marketers practical advice on choosing appropriate words, as well as guidance on how to write slogans that are either likeable or memorable in line with their strategic goals. Words matter To explore the relationship between the length and composition of a slogan and how well liked it is, the researchers carried out a large multi-method study with 820 brand slogans and a variety of experiments to uncover the word properties that make slogans more effective. They asked around 1,000 students and online workers to tell them how much they liked, or disliked, a subset of real brand slogans. Later, they also gave them a surprise recognition test to see which slogans they remembered seeing earlier. Following on from this experiment, the researchers identified five linguistic properties that had opposing effects on whether a slogan was liked and remembered: length, brand name, word frequency, perceptual distinctiveness, and abstractness. Slogans that were longer and included the brand name (Like a good neighbor, State Farm is there vs. Like a neighbor) were more frequently remembered but liked less. Conversely, slogans that included words that are more frequently used (bad breath vs. halitosis) and abstract (disease vs. halitosis) were better liked but less well remembered. This is because consumers fixate less often and for less time on slogan words that are frequently used and more abstract. As a result, when consumers come across fluent slogans, they are more likely to like and click on the ads but remember them less accurately. Snag the sensation Using these findings, the researchers then tried to improve existing brand slogans by making disliked ones more fluent, and forgettable slogans less fluent. In one experiment with 243 students, they found that the slogans they had made more fluent (e.g., changing Listerine's slogan from 'Stops halitosis' to 'Kills bad breath') became better liked but also worse remembered. Conversely, slogans that they made less fluent (e.g., altering Toyota's slogan from 'Get the feeling' to 'Snag the sensation') became better remembered but less liked. Another experiment using eye-tracking technology revealed that these changes occurred because participants look longer and more often at disfluent words (e.g., sensation) compared to fluent words (e.g., feeling). The researchers also saw a 28% increase in the click-through rate on a Facebook ad when they improved the fluency of a slogan, as the rate increased from 1.3% to 1.7%, reducing the cost-per-click. As a result, the authors suggest that brands that need to gain recognition may consider using words that are difficult to process, i.e., rare and concrete words, while established brands may want to use words that are easy to process, i.e., those that are common and abstract. Our research identifies specific properties of words that can make a slogan better liked or better remembered, but importantly, the properties that make a slogan more likeable also make it less memorable, and vice versa. "To be memorable, slogans should be relatively long, include the brand name, and use rare and concrete words. For instance, BMW could make its slogan easier to remember by changing it from 'The ultimate driving machine' to 'BMW is the peak driving machine', but that would also make it harder to like. In fact, our research can be viewed as the ultimate slogan machine, and we hope that it will help marketers choose the best words for their brand."
