Introduction

Abdominal symptoms after removal of the GALLBLADDER. The common postoperative symptoms are often the same as those present before the operation, such as COLIC, bloating, NAUSEA, and VOMITING. There is pain on palpation of the right upper quadrant and sometimes JAUNDICE. The term is often used, inaccurately, to describe such postoperative symptoms not due to gallbladder removal.

Rationale Diet for Gallstone Disease (RADIGAL Trial): A Randomized Controlled Trial Comparing Fat-free Versus Balanced (WHO) Diet in Gallstone Disease

The goal of this trial is to assess the effect on the quality of life (QoL) of recommending a fat-free diet versus a balanced WHO diet in symptomatic patients with gallstones and in patients undergoing cholecystectomy for gallstone disease. The main question it aims to answer is:
• Is balanced WHO diet as good as low-fat diet in terms of patient reported outcomes when gallstone disease is managed non-operatively (RADIGAL-1) or cholecystectomy is performed (RADIGAL-2)?
A validated QoL questionnaire (Gastrointestinal Quality of Life Index - GIQLI) will be administered both at baseline and during follow-up. Analysis of covariance with ordinary least squares estimation will be applied to estimate between-group differences in GIQLI scores at 3 months (primary endpoint). Patient adherence and the disease-related symptoms occurrence or hospitalizations will also be appraised (secondary endpoint).
Participants presenting with biliary colic or acute cholecystitis (RADIGAL-1) or undergoing cholecystectomy for gallstones (RADIGAL-2) will:
be randomly assigned to recommending either a fat-free or a balanced WHO diet .
be contacted via phone call to complete the GIQLI questionnaire and document overall adherence to the diets and secondary outcome events.
This trial examines the impact of diet on adverse events in gallstone disease, representing the first randomized study to focus on this aspect.

The impact of vascular injuries on the management of bile duct injury following laparoscopic cholecystectomy- insights from a prospective study

BACKGROUNDOur study aimed to compare the clinical presentation and outcomes of post-cholecystectomy bile duct injuries (BDI) with and without arterial injuries.METHODSA prospective analysis of 123 patients with post-cholecystectomy BDI between July 2018 and January 2022 was performed. Multivariate logistic regression analysis was used to assess the impact of vascular injuries on perioperative complications and long-term outcomes after delayed repair.RESULTSOf 123 patients, 42 (34%) had associated vascular injuries, predominantly right hepatic artery disruptions. These patients experienced significantly higher perioperative complications after the index surgery (Cholangiolar abscess- 83.3% vs 32.1% ( p<0.001), recurrent cholangitis- 66.67 % vs 14.81 % ( p<0.001), blood transfusions ³2 - 89.74 % vs 28.57 % ( p<0.001), hospital admissions ³3 - 3.88 ± 1.99 vs 2.49 ± 0.74 ( p<0.001). Delayed Hepp-Couinaud biliary repair was performed in 112 patients. After a median follow-up, of 2 years, 85 (51.43 % vs. 88.16 % ), 12 (25.71 % vs. 3.89 %), 6 (11.43 % vs. 2.59 %), and 9 (11.43 % vs. 6.49 %) patients had excellent, good, fair, and poor outcomes.CONCLUSIONConcomitant vasculobiliary injuries were associated with increased morbidity after index surgery; however, the long-term outcomes following definitive biliary repair remained comparable.

Gallbladder stones are present in 10-20 % of the population of Western countries, and the incidence rate has almost tripled over the past 30 years. Asymptomatic cholecystitis is more common than it is mentioned in the literature. Approximately 80 % of the GI have no clinical manifestations. In most cases, the disease will remain asymptomatic throughout life. The problem of asymptomatic cholecystitis is often hushed up and ignored. Surgeons try to avoid examining this pathology. Works on asymptomatic cholecystitis are not only few, but also scattered. At the same time, there is no uniformity in the literature. Since the tactics of treating such patients still does not have an unambiguous approach. Many surgeons doubt the decision on surgical treatment, a smaller part chooses cholecystectomy at the same time, without substantiating the indications for it. That is why most cases of postcholecystectomy syndrome occur in patients operated on for asymptomatic cholecystitis. The unwillingness to discover the secret of stone formation and asymptomatic cholecystitis in humans when concretions in the gallbladder are detected without reference to surgery, to indicate the trajectory of the examination, leads to a number of tactical errors, preserving the risk of PES, and erasing practical recommendations for the management of such patients. The solution to this issue lies in a thorough study of each stage of the tactical search in patients with asymptomatic cholecystitis, which will determine a new strategy in biliary surgery.

Laparoscopic cholecystectomy for triple gallbladder malformation: A comprehensive case report and literature review

Rationale:Triple gallbladder is a rare congenital anatomical abnormality because of the incomplete regression of rudimentary bile ducts and is often not found until it is accidentally detected during imaging research.Patient concerns:We report a rare case of triple gallbladder malformation and review the English literature on biliary tract variation caused by gallbladder malformation. The diagnosis, treatment, and postoperative situation of the patients were summarized and analyzed.Diagnoses:The patient was diagnosed with triple gallbladder malformation.Interventions:We conducted laparoscopic cholecystectomy for the patient.Outcomes:We successfully removed all 3 gallbladders by laparoscopic cholecystectomy, and the patient had no postoperative complications such as bleeding or bile leakage and recovered well.Lessons:At present, the best treatment for triple gallbladder malformation is to remove all 3 gallbladders by laparoscopic cholecystectomy, which can effectively prevent the occurrence of “postcholecystectomy syndrome” caused by individual gallbladder residue. However, the diagnosis of triple gallbladder is very challenging and often requires a combination of advanced imaging methods. However, sometimes preoperative imaging does not fully reveal biliary tract variation. Therefore, we need to rely on the guidance of evidence-based medicine before surgery and the accurate evaluation of surgical plans to complete the operation without risk.
