The Phase 2/3 TEBE-AM trial was designed to evaluate KIMMTRAK® (tebentafusp-tebn), as monotherapy and in combination with pembrolizumab, versus a control arm, for the treatment of patients with previously treated advanced cutaneous melanoma. The trial was originally designed as an adaptive Phase 2/3 trial with the optionality to review Phase 2 data and drop an arm. Following consultation with the FDA, the Company has decided to conduct the trial solely as a Phase 3 with the primary endpoint of overall survival. As a result of the recent rapid accrual, the Company projects that the Phase 3 trial would be mostly enrolled by the time the Phase 2 overall survival would have matured. In addition, the three arm Phase 3 will allow more robust testing of KIMMTRAK as monotherapy and in combination versus a control arm. Finally, with all patients randomized to date to be included in the intent-to-treat population, the time to final analysis of the Phase 3 trial will be accelerated. The Phase 1b data showed that the maximum target doses of tebentafusp (68 mcg) plus durvalumab (20 mg/kg) were well tolerated, and that, in mCM patients who progressed on prior anti-PD(L)1, the combination demonstrated a one-year overall survival rate of ~75%. About Cutaneous Melanoma Cutaneous melanoma (CM) is the most common form of melanoma. It is the most aggressive skin carcinoma and is associated with the vast majority of skin cancer-related mortality. The majority of patients with CM are diagnosed before metastasis and survival remains poor for the large proportion of patients with metastatic disease. Despite recent progress in advanced melanoma therapy, there is still an unmet need for new therapies that improve first-line response rates and duration of response as well as for patients who are refractory to first-line treatments. About Uveal Melanoma Uveal melanoma is a rare and aggressive form of melanoma, which affects the eye. Although it is the most common primary intraocular malignancy in adults, the diagnosis is rare, and up to 50% of people with uveal melanoma will eventually develop metastatic disease. Unresectable or metastatic uveal melanoma typically has a poor prognosis and had no approved treatment until KIMMTRAK. About KIMMTRAK® KIMMTRAK is a novel bispecific protein comprised of a soluble T cell receptor fused to an anti-CD3 immune-effector function. IMPORTANT SAFETY INFORMATION Cytokine Release Syndrome (CRS), which may be serious or life-threatening, occurred in patients receiving KIMMTRAK. Monitor for at least 16 hours following first three infusions and then as clinically indicated. Manifestations of CRS may include fever, hypotension, hypoxia, chills, nausea, vomiting, rash, elevated transaminases, fatigue, and headache. CRS occurred in 89% of patients who received KIMMTRAK with 0.8% being grade 3 or 4. Ensure immediate access to medications and resuscitative equipment to manage CRS. Ensure patients are euvolemic prior to initiating the infusions. Closely monitor patients for signs or symptoms of CRS following infusions of KIMMTRAK. Monitor fluid status, vital signs, and oxygenation level and provide appropriate therapy. Withhold or discontinue KIMMTRAK depending on persistence and severity of CRS. Skin Reactions Skin reactions, including rash, pruritus, and cutaneous edema occurred in 91% of patients treated with KIMMTRAK. Monitor patients for skin reactions. If skin reactions occur, treat with antihistamine and topical or systemic steroids based on persistence and severity of symptoms. Withhold or permanently discontinue KIMMTRAK depending on the severity of skin reactions. Elevated Liver Enzymes Elevations in liver enzymes occurred in 65% of patients treated with KIMMTRAK. Monitor alanine aminotransferase (ALT), aspartate aminotransferase (AST), and total blood bilirubin prior to the start of and during treatment with KIMMTRAK. Withhold KIMMTRAK according to severity. Embryo-Fetal Toxicity KIMMTRAK may cause fetal harm. Advise pregnant patients of potential risk to the fetus and patients of reproductive potential to use effective contraception during treatment with KIMMTRAK and 1 week after the last dose. The most common adverse reactions (≥30%) in patients who received KIMMTRAK were cytokine release syndrome, rash, pyrexia, pruritus, fatigue, nausea, chills, abdominal pain, edema, hypotension, dry skin, headache, and vomiting. In a Phase 1b trial of tebentafusp in combination with checkpoint inhibitors in mCM, the maximum target doses of tebentafusp (68 mcg) plus durvalumab (20 mg/kg) were well tolerated. In mCM patients who progressed on prior anti-PD(L)1, tebentafusp with durvalumab continues to demonstrate promising overall survival (OS) (1-yr ~75%) compared to recent benchmarks (1-yr ~55%). Cutaneous and uveal melanoma both overexpress gp100, with greater than 80% of cutaneous melanoma tumor cells expressing gp100 by immunohistochemistry (IHC). The pattern of tumor shrinkage and its association with OS observed in mUM are replicated in this mCM study including 37% of patients with any tumor shrinkage of whom 89% lived at least 1-year. 58% of patients with any tumor increase were alive at 1-year, compared to 64% in the Phase 3 study in mUM. These data provide a rationale for a randomized study of tebentafusp monotherapy and in combination with an anti-PD1 in mCM, which the Company plans to start by year end. “KIMMTRAK (tebentafusp-tebn) was designed to target gp100-positive melanoma, regardless of site of origin, tumor mutational burden or prior therapy. We remain encouraged by the similarities in clinical benefit for this Phase 1 study in metastatic cutaneous melanoma relative to our Phase 3 experience in uveal melanoma and look forward to starting a randomized trial in melanoma by year end,” stated David Berman, Head of Research and Development at Immunocore. “Additionally, we presented further analysis of KIMMTRAK, the first T cell receptor therapeutic to demonstrate an OS benefit in a randomized study versus investigator’s choice, on treatment beyond radiographic progression and the effect of the systemic corticosteroids on efficacy.” In an analysis of the Phase 3 trial of tebentafusp, an OS benefit observed for tebentafusp among mUM patients who have initial radiographic progression demonstrates that radiographic assessment underestimates the benefit. In a post-hoc analysis of OS following initial radiographic progression, continued treatment with tebentafusp was associated with numerically longer OS even after adjusting for the difference in key prognostic variables. Tebentafusp treatment beyond progression was tolerated without new safety signals and, in some patients, was associated with radiological stabilization of target lesions for ≥ 4 months following the initial progression. In another post hoc analysis of the Phase 3 trial, the vast majority patients treated with tebentafusp (84%) either did not require corticosteroids (74%) or only received them on a single day (10%). The most frequent reason for corticosteroid use was an emergent adverse event (AE), including CRS and rash. Corticosteroid use following the pre-specified AE guidelines was not associated with any significant impact on efficacy of tebentafusp. Under the deal, Sanofi will study its SAR444245 in combination with Immunocore’s KIMMTRAK in HLA-A*02:01 positive patients with advanced unresectable or metastatic skin cancers in an ongoing Phase I/II trial. SAR444245 is Sanofi’s precisely PEGylated, engineered form of Interleukin-2 (IL-2) which has specificity and selectivity toward CD8+ T cells and Natural Killer (NK) cells. It has a single, targeted PEG-moiety linked irreversibly to a novel amino acid inserted at a precise spot, which prevents the drug from binding to the CD25 alpha-submit while preferentially binding to the beta/gamma IL-2 receptor subunits. It is approved in the U.S. for metastatic uveal melanoma and was also approved for that indication in Europe on April 4. “Immunocore has demonstrated that in vitro, IL-2 can enhance the activity of the ImmTAC platform, particularly in the context of tumor-associated inhibitory macrophages, and that metastatic uveal melanoma (mUM) patients with higher expression of IL2-beta and gamma, but not alpha, receptor have better overall survival (OS) on KIMMTRAK (presented at SITC 2021),” Dr. David Berman, M.D., Ph.D., head of Research and Development at Immunocore stated. “We are pleased that Sanofi will test this hypothesis in their clinical trial in patients with metastatic cutaneous melanoma (mCM), a population with significant unmet medical need.” Under the deal, Sanofi will handle clinical development and all costs for the study except for expenses associated with manufacturing and supply of KIMMTRAK, which Immunocore will be responsible for. “We are excited to embark on this collaboration with Immunocore,” Dr. John Reed, M.D., Ph.D., executive vice president and global head of R&D for Sanofi, said. “The strong scientific rationale makes it compelling to investigate Immunocore’s KIMMTRAK, the first approved TCR therapeutic for a solid tumor, in combination with our engineered lymphokine, SAR444245.” He added, “While we are actively studying SAR444245 as monotherapy and in combination with anti-PD-1 class checkpoint inhibitors and with approved immuno-competent monoclonal antibodies, the collaboration with Immunocore represents Sanofi’s first exploration of combining SAR444245 with a T cell engager. We are therefore very eager to explore this high potential combination in our ongoing multi-arm Phase I/II clinical study.” In 2021, Sanofi presented interim, early-stage data on SAR444245 (previously THOR-707) as a monotherapy and in combination with an anti-PD-1 checkpoint inhibitor. The initial activity was observed, with three confirmed partial responses, including patients who had previous anti-PD-1 therapy. At the time, Reed said , “THOR-707 has a potentially best-in-class pro reinforces the promise of our Synthorin technology platform to overcome difficult targets with precision biology. The activity observed both as single agent and with an anti-PD-1 further strengthens our belief that as a unique not-alpha IL-2, THOR-707 could become a backbone of future immuno-oncology therapies.”
