A rare case of dumbbell shaped ancient schwannoma found inside thoracic spine in a psychologically challenged patient

Introduction and importance:Ancient schwannoma undergoes a degenerative process leading to hemorrhage, cystic necrosis, and calcification. The head and neck, thorax, and retroperitoneum are the most common locations; the spinal location is extraordinary.Case presentation:A 50-year-old man presented with muscle weakness in his lower limbs following 2 years of lower back pain, tingling sensation and numbness. He suffers from moderate depression that was refractory to treatment and was put on selective serotonin reuptake inhibitor. MRI showed an intradural extramedullary dumbbell-shaped mass at the T11–T12 level with compression of the spinal cord. Furthermore, it was located on the right side of the spinal canal in close relation to the right T12 spinal root. He underwent a T11–T12 laminectomy with complete en bloc resection and the nerve root was sacrificed. Histopathological exam confirmed the final diagnosis. Follow-up showed a rapid improvement of the neurological and psychiatric symptoms.Clinical discussion:Shwannomas are slow growing tumors arising from Schwann cells of the peripheral nerve sheaths. Spinal ancient schwannomas are particularly rare because of the compression of the spinal cord leading to the early presentation of symptoms. The risk of developing depressive disorders is increased by physical diseases; moreover, chronic back pain is strongly associated with depression.Conclusion:Ancient schwannoma is a slow-growing tumor and best pre-operatively diagnosed by MRI. Diagnostic approach in patient with psychiatric disorders presenting with physical symptoms, especially neurological symptoms should be done carefully depending on objective tools such as imaging or detailed neurological examination.

Optimal size of Frozenix for true thoracic aneurysms: is downsizing an option?

OBJECTIVEDuring total arch replacement (TAR) using frozen elephant trunk (FET) technique with Frozenix for true thoracic aortic aneurysm (tTAA), oversized FET tends to be chosen similar to the endovascular devise selection. However, the oversized FET is considered a risk factor for intimal injury. The appropriate size selection of FET remains insufficiently understood.METHODSBetween October 2014 and March 2022, a total of 49 patients underwent TAR using Frozenix for tTAA. Out of 49 patients, four patients planned to staged surgery were excluded, 19 patients were operated on with an undersized Frozenix compared with the descending aorta (undersized FET group) and in 26 patients an equal or oversized Frozenix was used (oversized FET group). Clinical outcomes and postoperative diameter changes were investigated.RESULTSIn-hospital mortality was 0%. The mean diameter of Frozenix and the descending aorta was 30.7 mm and 28.8 mm, respectively, in the oversized FET group, and 26.7 mm and 30.1 mm in the undersized FET group. Postoperative computed tomography (CT) demonstrated no endoleaks not only in the oversized FET group but also in the undersized FET group. CT also revealed that undersized FET had expanded more than the original diameter in all cases except for two, with an average of 2.47 ± 1.53 mm. Additionally, the descending aorta covered with Frozenix shrank in 10 patients (53%). Postoperative adverse aortic events were not observed.CONCLUSIONSUndersized Frozenix tightly fit the descending aorta and resulted in complete sealing without endoleaks. Oversized FET is not strictly necessary considering the size-related adverse complications.

Apparent Diffusion Coefficient (ADC) and Magnetic Resonance Imaging (MRI) Nomogram for Differentiating a Solitary Fibrous Tumor (World Health Organization Grade II) From an Angiomatous Meningioma

INTRODUCTIONAccurate preoperative differentiation between intracranial solitary fibrous tumor (SFT, World Health Organization grade II) and angiomatous meningioma (AM) is crucial for surgical planning and prognosis prediction. While conventional magnetic resonance imaging (MRI) is widely used, distinguishing these tumors based on imaging alone remains challenging. This study aimed to evaluate clinical and MRI features to improve diagnostic accuracy between SFT and AM, focusing on the apparent diffusion coefficient (ADC) and conventional MRI parameters.METHODSA retrospective analysis was conducted on 51 patients (23 with SFT and 28 with AM) confirmed by pathology. Clinical and MRI characteristics were assessed using t-tests and chi-square tests. Logistic regression analysis was performed to identify independent predictors, and receiver operating characteristic (ROC) curve analysis evaluated diagnostic performance. A nomogram integrating ADC values with conventional MRI features was developed and validated using calibration curves.RESULTSSignificant differences in tumor shape, cystic necrosis, T1-weighted imaging and T2-weighted imaging signal intensities, and ADC values were observed between SFT and AM (p < 0.05). Logistic regression analysis confirmed these factors as independent predictors, with ADC demonstrating the highest diagnostic performance at an optimal cutoff value of 1.08 × 10^-³ mm²/second. The ROC analysis showed that combining ADC with conventional MRI features improved diagnostic accuracy. The calibration curve demonstrated strong agreement between nomogram predictions and actual outcomes.CONCLUSIONIntegrating ADC values with clinical and MRI features provides a reliable method for differentiating intracranial SFT from AM. This approach enhances diagnostic precision, aiding in optimized clinical decision-making and surgical planning.
