以山东信谊制药有限公司研制的盐酸胺碘酮片（0.2 g）为受试制剂，Sanofi-Aventis France公司研发的盐酸胺碘酮片（200 mg，Cordarone®）为参比制剂，考察两制剂在空腹和餐后状态下单次给药的药代动力学参数及相对生物利用度，评价两制剂是否具有生物等效性。

Amiodarone hydrochloride tablets (0.2 g) developed by Shandong Xinyi Pharmaceutical Co., Ltd. were used as the test preparation, and amiodarone hydrochloride tablets (200 mg, Cordarone®) developed by Sanofi-Aventis France were used as the reference preparation. The pharmacokinetic parameters and relative bioavailability of the two preparations after single administration in the fasting and postprandial states were investigated to evaluate whether the two preparations were bioequivalent.

Impact of multimorbidity on risk stratification and prognosis in elderly patients after acute myocardial infarction

BackgroundElderly patients with first acute myocardial infarction (AMI) often have serious clinical features and worse prognoses. The purpose of the study was to evaluate the burden of comorbidity in predicting prognosis of elderly patients after AMI.MethodsA total of 479 elderly patients with AMI divided into three groups: group A (with zero or one comorbidity), group B (with two comorbidities), and group C (with three or more comorbidities). The clinical characteristics, medication, in-hospital complications, and one-year prognosis of these patients were collected and compared across the three groups.ResultsCompared to group A, the age, heart rate, creatinine and uric acid, glucose, left atrial diameter (LAd), relative number of multi-vessel and culprit vessels located in left main (LM) artery and thrombolysis in myocardial infarction (TIMI) grade 0-1 were significantly higher in groups B and C. Compared to group A, the incidences of in-hospital death, paroxysmal atrial flutter/atrial fibrillation, and acute heart failure were significantly higher in groups B and C and that of in-hospital cardiac rupture was remarkably higher in group C. Compared group A, the incidence of one-year major adverse cardiovascular events (MACEs), all-cause death, and attacks of acute heart failure were significantly higher in groups B and C. The incidences of one-year non-fatal ischemic stroke in group C and late revascularization in group B were dramatically higher.ConclusionsA higher comorbidity burden was associated with more serious clinical presentations, more severe in-hospital complications, and worse one-year prognosis of elderly AMI patients. These findings can help clinicians to identify high-risk elderly AMI patients, determine the prognostic impact of comorbidity, and provide better managements.

Hypertrophic cardiomyopathy in a dog: a systematic diagnostic approach

A seven-year-old female neutered Parson Russel terrier was referred for syncopal episodes. An electrocardiogram revealed paroxysmal atrial flutter followed by periods of sinus arrest, suggesting sick sinus syndrome. Echocardiography showed severe biventricular wall thickening (hypertrophic cardiomyopathy (HCM) phenotype) with no signs of fixed or dynamic left ventricular outflow tract obstruction. Blood pressure, abdominal ultrasound, serum total thyroxin and thyroid-stimulating hormone, and insulin-like growth factor-1 were all within normal limits. Cardiac troponin I was elevated (1.7 ng/mL, ref<0.07). Serological tests for common infectious diseases were negative. A 24-h Holter confirmed that the syncopal episodes were associated with asystolic pauses (sinus arrest after runs of atrial flutter) ranging between 8.5 and 9.6 s. Right ventricular endomyocardial biopsies (EMB) were performed at the time of pacemaker implantation to assess for storage or infiltrative diseases that mimic HCM in people. Histological analysis of the EMB revealed plurifocal inflammatory infiltrates with macrophages and lymphocytes (CD3+ > 7/mm²) associated with myocyte necrosis, but no evidence of myocyte vacuolisation or infiltrative myocardial disorders. These findings were compatible with myocardial ischaemic injury or acute lymphocytic myocarditis. Molecular analysis of canine cardiotropic viruses were negative. The dog developed refractory congestive heart failure and was euthanised 16 months later. Cardiac post-mortem examination revealed cardiomyocyte hypertrophy and disarray with diffuse interstitial and patchy replacement fibrosis, and small vessel disease, confirming HCM. We described a systemic diagnostic approach to an HCM phenotype in a dog, where a diagnosis of HCM was reached by excluding HCM phenocopies.

Atrial myxoma: A cause for concern in multiple cerebral infarctions in absence of cardiac symptoms

Ischaemic cerebral infarct is the most common neurological manifestation of myxomas. We present a mid-70s man who was brought to the stroke unit with sudden onset right hemiparesis and dysarthria. Although he had risk factors for developing stroke like hypertension, hypercholesterolaemia, history of prostate cancer and new paroxysmal atrial flutter on admission, clinical suspicion and detailed investigations in the absence of cardiac symptoms revealed a 9 cm myxoma causing severe mitral stenosis. He underwent successful surgical resection with good functional recovery. This also highlights the need for thorough clinical examination especially in the absence of cardiac symptoms.
