Comparison of Lung Ultrasound and Chest Radiography for Pleural and Pulmonary Diagnosis in Pediatric Intensive Care Unit

In pediatric intensive care unit of Strasbourg, two exams are easily used for pleural and pulmonary diagnosis : chest radiography, which is the "historic" and reference exam, and commonly systematically the lung ultrasound.
However, chest radiography produces radiation and is less accessible in our unit, because it depends on the radiology department.
On the opposite, the lung ultrasound doesn't produce radiation, is simple, reliable, easily accessible (the investigators have two ultrasound systems in the unit), and simply and quickly achievable at the patient's bedside, including for less experienced operator.
We will evaluate the diagnostic performances of lung ultrasound and chest radiography for pleural and pulmonary diagnosis in pediatric intensive care unit.

Challenging diagnosis in pulmonary nut carcinoma: A report of two cases with different histopathologic and molecular features and a novel NUTM1::SPECC1 gene fusion.

BACKGROUNDNUT carcinoma (NC), formerly known as NUT midline carcinoma, is a rare but highly aggressive cancer. It is a poorly differentiated carcinoma characterized by rearrangements of the NUTM1 (nuclear protein in Testis) gene with a member of the bromodomain-containing protein (BRD) family gene, usually BRD4. There is limited knowledge about primary pulmonary NC till now. It is probably underestimated or underdiagnosed because of its poorly differentiated character, misleading immunophenotype, and wide range of differential diagnoses.METHODWe report here two cases of pulmonary NC with different clinicopathological and molecular presentations to draw attention to some atypical clinicopathologic features that can help clinicians and pathologists consider this rare entity.RESULTSThe first case shows a nested pattern with small, uniform, blue epithelioid cells and aberrant expression of neuroendocrine markers, which has a known BRD3::NUTM1 fusion accompanied by a novel IGR (downstream ROR2)::NUTM1 fusion. The second case demonstrates solid sheets and cords of eosinophilic epithelioid-polygonal cells with a mucoid stroma and TTF1 expression, which has a novel SPECC1::NUTM1 gene fusion accompanied by TP53 and JAK1 gene oncogenic variants.CONCLUSIONAs a result, our study contributes to expanding the variant spectrum of the NUTM1 gene. NUT carcinoma with different fusion partners seems to have unique clinicopathological characteristics, yet more cases need to accumulate experience.

An isolated congenital absence of right pulmonary artery associated with coronary and multiple systemic collaterals: A case report and literature review

Unilateral pulmonary artery agenesis (UPAA) is the complete absence of either the right or left pulmonary artery. It is a rare congenital malformation, usually associated with other cardiac defects; however, in some cases, it may occur as an isolated lesion, typically affecting the right side. Patients with isolated right pulmonary artery agenesis are often diagnosed incidentally on chest radiographs in adulthood. Rare cases are associated with multiple systemic collaterals that supply the affected lung. We report the case of a 69-year-old man with the absence of a right pulmonary artery, arterial supply to the right lung from the celiac trunk, and communication of the collaterals with a marginal branch arising from the right coronary artery. UPAA is a rare congenital anomaly that may present with nonspecific symptoms, such as dyspnea and recurrent chest infections. Clinicians should consider UPAA in the differential diagnosis of respiratory symptoms, particularly in adult patients with unexplained symptoms.

Right lung agenesis with multi-drug-resistant Cepacia syndrome: A case report

ABSTRACTLung agenesis (LA) is a rare congenital anomaly that refers to an absence of lung tissue and bronchi. Burkholderia cepacia complex (BCC) causes a fatal acute necrotising pneumonia called Cepacia syndrome. The unique concomitant association of both these rare diseases has not been highlighted in the literature to the best of our knowledge. Here, we present a case of association of right-sided lung agenesis along with Cepacia syndrome. In our case, the Burkholderia cepacia complex was found to be a multi-drug-resistant organism. Debilitating deterioration in the oxygen saturation was observed and initiation of treatment after culture sensitivity report yielded in remission of the disease. However, due to underlying congenital agenesis, therapeutic intervention with steroids and bronchodilators led to stabilising the patient by developing oxygenation. Underlying pulmonary congenital defects predispose individuals to infections. The implementation of antimicrobial therapy must be aided with culture and sensitivity reports for better outcomes.
