A Phase IIa Randomized, Double-Blind Trial of Erlotinib in Inhibiting EGF Receptor Signaling in Aberrant Crypt Foci of the Colon

This randomized phase II trial is studying how well erlotinib hydrochloride works in treating patients with stage I-III colorectal cancer or adenoma. Matrix metalloproteinase 9 implication during colorectal carcinogenesis. Effect of doxycycline

AbstractBackgroundMatrix metalloproteinases (MMPs), including MMP9, play a significant role in colorectal cancer (CRC) progression, mainly by extracellular matrix remodeling. However, little is known about MMP9 role in aberrant crypt foci (ACF) cluster formation, the earliest colon preneoplastic lesions.Aims and methodsWe conducted a bioinformatics analysis of MMPs expression in CRC using Gene Expression Profiling Interactive Analysis2 (GEPIA2). Subsequently, we investigated MMP9 expression during the early stage of colon carcinogenesis in mice and assessed the effect of doxycycline (DOX), a global inhibitor of MMPs, on ACF cluster formation. Thus, NMRI mice received two weekly injections of 1,2‐Dimethylhydrazine (DMH, 20 mg/kg, subcutaneously), followed or not by DOX (100 mg/kg, orally, from the 4th to the 6th week).ResultsGEPIA2 analysis indicated that among the 28 identified MMPs with collagenase and doxycycline‐sensitive activities, MMPs 1, 3, 7, 9, and 13 were overexpressed in CRC tissues. Moreover, only MMP1 and MMP9 correlated well with collagen expression in colorectal tumors. In vivo, methylene blue‐stained DMH‐treated colons revealed multiple ACF clusters at week 6, associated with mucosa remodeling and sustained nitrosative stress as attested by enhanced collagen fibers, malondialdehyde level, and nitrotyrosine deposits. Pyrosequencing showed increased methylation at the tenth CpG site of the MMP9 promoter, which was associated with increased MMP9 expression. Interestingly, DOX attenuated the number and size of ACF clusters and mucosa remodeling without rebalancing nitrosative stress.ConclusionOverexpression of MMP9 occurs early during colorectal carcinogenesis, and doxycycline may control the pathological remodeling of colon mucosa into ACF clusters by attenuating MMP9 activity.

Vitamin C and D do not increase the chemopreventive effect of aspirin on colon carcinogenesis in a mouse model

Aspirin has significant antineoplastic effects on the colon, whereas the outcomes of Vitamin C and D supplementation have been inconsistent. This study addressed the isolated and combined effects of vitamins C and D with aspirin on chemically induced colon carcinogenesis in mice, focusing on redox balance and DNA damage. Adult male and female mice were divided into seven groups: a control group; a group treated with 1,2-dimethylhydrazine (DMH, 40 mg/kg twice per week during weeks 4 and 5); and groups treated with DMH plus the following: aspirin (6 mg/kg in water); Vitamin C (500 mg/L in water); Vitamin D (1500 IU by oral gavage); a combination of Vitamin C and aspirin; and a combination of Vitamin D and aspirin. The treatments were administered continuously for 12 weeks. The treatments, isolated or combined, reduced aberrant crypt formations. Aspirin alone reduced the formation of aberrant crypt foci (ACF) by 65 %, accompanied by a systemic reduction in oxidative stress and DNA damage prevention. However, adding vitamins C and D to aspirin did not enhance these effects. Vitamin D alone suppressed ACF formation and DNA damage in the liver, whereas vitamin C had a limited effect on colon carcinogenesis, despite reducing oxidative stress in the liver and colon. In summary, we found no evidence that vitamins C and D supplementation enhanced the chemopreventive effects of aspirin on colon cancer.

BACKGROUNDColorectal carcinogenesis induces changes in the colon, such as the appearance of aberrant crypt foci (ACF). This process is influenced by genetic and environmental factors, such as diet. Hibiscus sabdariffa L. is a considerable source of phenolic compounds, such as anthocyanins, and dietary fibers that may exert anti-inflammatory, antioxidant and prebiotic properties, attenuating the appearance of ACFs.OBJECTIVESTo investigate whether supplementation with 5% or 10% of dehydrated calyces of Hibiscus sabadariffa (DHSC) influences the composition of the intestinal microbiota and the expression of genes related to colorectal carcinogenesis in BALB/c mice.METHODSThe in vivo experiment lasted 12 weeks and the animals were divided into 3 experimental groups: the control group and the supplemented groups (5% or 10% DCHS) and induced pre-neoplastic lesions with the drug Dimethylhydrazine. Serum aspartate aminotransferase and alanine aminotransferase markers, liver cytokine profile, gut microbiota composition and tumor protein 53, cellular myelocytomatosis oncogene, caspase-3 and Proliferating Cell Nuclear Antigen gene expression were determined.RESULTSSupplementation with 5% or 10% of DCHS altered the composition of the intestinal microbiota, increasing the abundance of the families Lachnospiraceae, Ruminococcaceae, Clostridiaceae and of the genus Clostridum, important producers of butyrate. Furthermore, 5% and 10% DCHS supplementation increased caspase-3 and c-Myc expression, respectively, which may suggest apoptotic events.CONCLUSIONSTherefore, the effects of DHSC, rich in anthocyanins and dietary fiber, on the composition of the intestinal microbiota and on the expression of genes associated with cell apoptosis may contribute to reducing the risk of developing preneoplastic lesions.
