Introduction

A rare genetic odontologic disease with characteristics of failure of eruption of non-ankylosed permanent teeth without evidence of obvious mechanical obstruction. Posterior teeth are preferentially affected (typically with involvement of all teeth distal to the most mesial non-erupted tooth), resulting in a posterior open bite. Non-ankylosed teeth tend to become ankylosed, and orthodontic treatment of affected teeth is generally unsuccessful.

Survival of Cast Restorations - Restorations Made of Precious Metal vs. Non-precious Metal

The aim of this study is to determine and compare the longevity of single-tooth cast restorations made of precious metal vs. non-precious metal (e.g. partial crowns, crowns) and to determine the influence of patient-, tooth-, treatment-, and restoration-specific parameters on longevity.
Patients who received single-tooth cast restorations made of precious metal or non-precious metal (e.g. partial crowns, crowns) between 1997 and 2022 will be retrospectively analyzed. Patient-, tooth-, treatment-, and restoration-specific parameters will be obtained from digital and paper-based dental records.
Survival and success of the restorations will be assessed using Kaplan-Meier statistics. Mean annual failure rates (mAFR) and median survival time will be calculated (Kaplan-Meier statistics). Potential predictive factors will be tested using log-rank tests and multi-variate Cox-regression analysis.

Missense variants weakening a SOX9 phosphodegron linked to odontogenesis defects, scoliosis, and other skeletal features

SOX9 encodes an SRY-related transcription factor critical for chondrogenesis and sex determination among other processes. Loss-of-function variants cause campomelic dysplasia and Pierre Robin sequence, while both gain- and loss-of-function variants cause disorders of sex development. SOX9 has also been linked to scoliosis and cancers, but variants are undetermined. It is highly expressed in tooth progenitor cells, but its odontogenic roles remain elusive, and tooth defects are unreported in SOX9-related conditions. Here, we performed whole-exome sequencing for nine unrelated children with tooth eruption delay and no known syndromes and identified a 7-year-old girl heterozygous for a SOX9 p.Thr239Pro variant and a 10-year-old boy heterozygous for presumably adjacent p.Thr239Pro and p.Thr240Pro variants. These variants were de novo and rare in control populations. Both cases had primary tooth eruption delay. Additionally, the boy had mesiodens blocking permanent central upper incisor eruption, severe scoliosis, and mild craniofacial and appendicular skeleton abnormalities. p.Thr239 and p.Thr240 occupy variable and obligatory positions, respectively, in a cell division control protein 4 (Cdc4)/FBXW7-targeted phosphodegron motif (CPD) fully conserved in SOX9 vertebrate orthologs and SOX8 and SOX10 paralogs, but functionally uncharacterized in vivo. Structural modeling predicted p.Thr240Pro and p.Thr239Pro/p.Thr240Pro but not p.Thr239Pro to strongly reduce SOX9/FBXW7 interaction. Accordingly, p.Thr240Pro and p.Thr239Pro/p.Thr240Pro but not p.Thr239Pro blocked FBXW7-induced SOX9 degradation in cultured cells. All variants increased SOX9-mediated reporter activation independently of protein stabilization, suggesting that CPD may also modulate the transactivation function of SOX9. Altogether, these findings concur that CPD has critical functions, that SOX9 decisively controls odontogenesis, and that gain-of-function variants may markedly perturb both this process and skeletogenesis.

Incidental Detection of a Non-valvular Papillary Fibroelastoma on Transthoracic Echocardiography in an Elderly Patient Undergoing Preoperative Assessment for Hip Repair

Papillary fibroelastoma (PFE) is a rare benign cardiac tumor that is often detected incidentally. This case report describes a 91-year-old Lebanese female who was admitted for a hip fracture and was found to have a nonvalvular cardiac PFE during preoperative evaluation. The patient had a history of progressive dyspnea, and transthoracic echocardiography revealed a pedunculated mass attached to the chordae of the posterior papillary muscle, causing mid-diastolic obstruction. Given the patient's clinical presentation and imaging findings, close postoperative monitoring in the cardiac care unit was advised. This case highlights the importance of echocardiographic evaluation in elderly patients with unexplained dyspnea and the need for individualized management strategies for incidentally discovered cardiac tumors.

Dental Neglect Among Children in Eastern Turkey.

PURPOSEDental neglect is defined as the failure of parents to take necessary measures to protect their child's oral health, prevent pain and infection, and provide essential dental treatment. This study aims to assess the level of dental neglect among children in Eastern Turkey and its relation to their oral health.MATERIALS AND METHODSThis cross-sectional study involved the 215 children and their parents. Dental neglect was measured using the Dental Neglect Scale (DNS), which consisted of seven Likert-scale questions (ranging from 1 to 5). A questionnaire also gathered demographic data. The DNS score was calculated by summing the responses, and the children's oral health was assessed using the Oral Hygiene Index and the Decayed, Missing, and Filled Teeth (DMFT) index. Data were statistically analysed with significance set at p 0.05.RESULTSThe average DMFT score was 7.24 ± 3.01, and the average DNS score was 16.24 ± 4.72. A statistically significant correlation was found between DMFT and the DNS score (correlation coefficient 0.162, p = 0.018). A statistically significant difference was noted between the mother's education level and the Dental Neglect Score (p = 0.006), but no statistically significant differences were observed concerning paternal education, family income, or frequency of dental visits.CONCLUSIONHigh DMFT scores and low maternal education levels are linked to higher dental neglect. Mothers play a crucial role in their children's oral health. Identifying mothers who do not provide adequate attention to their children as well as children in need of oral care is essential for implementing early, tailored interventions.
