Pneumocystis Pneumonia is increasing in Immunocompromised Non-HIV Infected Patients. The effects and safety of caspofungin and corticosteroids is not certain in this population. All Immunocompromised Non-HIV patients with respiratory failure were randomized into caspofungin and non-caspofungin group and corticosteroids and non-steroids group. The major outcome is 28 day mortality, the second outcome are time of respiratory rate decreases to less than 25 breath per minute, body temperature lower than 37.3℃.

A novel Escherichia coli cell–based bioreporter for quantification of salicylic acid in cosmetics

Abstract:

Transcription factor–based bioreporters have been extensively studied for monitoring and detecting environmental toxicants. In Escherichia coli, the multiple antibiotic resistance regulator (MarR) induces transcription upon binding to salicylic acid (SA). We generated SA-specific E. coli cell–based bioreporters utilizing the operator region of the mar operon and MarR as components of the reporter and sensing domains, respectively. Although bioreporters based on endogenous MarR and wild-type E. coli cells responded to SA, their sensitivity and selectivity were insufficient for practical sample monitoring. To improve these parameters, we genetically engineered host strains for optimal MarR expression, which enhanced the sensitivity of the biosensor to micromolar quantities of SA with increased selectivity. Under the optimized experimental conditions, the biosensor could quantify SA in environmental samples. For validation, the SA concentration in artificially contaminated SA-containing cosmetic samples was determined using the developed biosensor. Reliability assessment by comparing the concentrations determined using LC–MS/MS revealed > 90% accuracy of the bioreporters. Although bioreporters are not considered standard tools for environmental monitoring, bacterial cell–based bioreporters may serve as alternative tools owing to their affordability and simplicity. The SA biosensor developed in this study can potentially be a valuable tool for monitoring SA in environmental systems.

Enhanced stabilization of multifunctional phenolic acids-grafted chitin nanofibers for Pickering emulsions

The objective of this study was to develop novel functional stabilizers for Pickering emulsions using phenolic acids-grafted chitin nanofibers (phenolic acids-g-ChNF), which were fabricated by grafting ferulic acid (FA), sinapic acid (SA) and caffeic acid (CA) onto ChNF via free radical-mediated method. The Fourier transform infrared spectrum and Proton nuclear magnetic resonance showed that graft copolymerization occurred between the amino groups of ChNF and the carbonyl of the phenolic acids. Further, it was revealed that CA-g-ChNF and SA-g-ChNF possessed stronger antioxidant and antibacterial properties than the original ChNF and FA-g-ChNF. Additionally, we applied phenolic acids-g-ChNF to develop Pickering emulsions and found that SA-g-ChNF- and CA-g-ChNF-stabilized emulsions displayed reduced droplet sizes compared to FA, the main reason for which was that SA and CA had a rather close bonding relationship with ChNF. Taken together, SA-g-ChNF and CA-g-ChNF as novel multi-functional particles can be employed for facilitating the stability of Pickering emulsions.

Composite starch films as green adsorbents for removing benzo[a]pyrene from smoked sausages

Benzo[a]pyrene (BaP), which is emitted during the processing of smoked sausages, accumulates in sausages and poses a serious threat to human health. This study focused on the removal of BaP from sausages and accompanying particulate matter (PM) during the smoking of sausages by films formed by combining corn starch (CS) with K-carrageenan (KC)/sodium alginate (SA). Initially, the effects of different additions of KC and SA on the rheological analysis, thermogravimetric analysis (TGA) and film-forming properties of the composite films were investigated. The BaP reduction capacities of CS-KC and CS-SA composite films in sausage were 41.1%-47.0% and 54.2%-56.5%, respectively, because the three-dimensional mesh structure of the composite films provided a large number of adsorption sites. Finally, kinetic studies demonstrated that BaP control in composite films is mainly achieved by intraparticle diffusion. Therefore, due to its excellent recyclability and biodegradability, composite starch film has a promising application in smoked meat products.

FDA warns Mexican glycerin manufacturer for refusing an inspection

A drug manufacturing facility in Mexico is drawing the ire of the FDA after it ignored the US regulator’s inspection requests and phone calls. According to the warning letter issued on June 13, Glicerinas Industriales refused a pre-announced inspection during a phone call with FDA prior to the inspection at the company’s facility in Zapopan, Mexico, a city next to Guadalajara, which was planned for May 16 to May 20. The company, which also goes by Glinsa, is a supplier of a wide range of drug ingredients, including glycerin, to manufacturers of finished drug products. These include manufacturers that distribute to the US. The letter made clear the importance of the inspection given that the use of glycerin contaminated with diethylene glycol (DEG) has resulted in various lethal poisoning incidents over the years in various countries. According to the FDA’s guidance on the subject from 2007, that regulator has received and continues to receive reports about fatal DEG poisoning of consumers who ingested medicinal syrups, such as cough syrup, that were manufactured with DEG-contaminated glycerin. According to the letter, the company’s drugs are now “adulterated” in that they have been manufactured, processed, packed or held in an establishment that delays, denies or limits an inspection, or refuses to permit entry or inspection. The FDA placed the company on Import Alert on June 9, and until the FDA is permitted to inspect the facility and enters compliance, they can withhold approval of any new applications or supplements listing it as a drug manufacturer. In addition, shipments of articles manufactured at Glicerinas Industriales into the US that appear to be adulterated are subject to being detained or refused admission. In 1937, an outbreak of DEG poisoning occurred in the US, which resulted from people ingesting an elixir of sulfanilamide that contained DEG as a solvent, resulting in 107 deaths, many of them children, which in part led to the Food Drug & Cosmetics Act to be passed. In late 1995 and early 1996, many children were admitted to hospitals in Port-au-Prince, Haiti, with sudden kidney failure, resulting in at least 80 fatalities, following similar poisoning.
