Purpose:This study aims to evaluate the performance of dual-energy window (DEW) and triple-energy window (TEW) scatter correction methods in cardiac SPECT imaging with technetium-99m (Tc-99m) and thallium-201 (Tl-201) radioisotopes.Materials and Methods:The SIMIND Monte Carlo program was used to simulate the imaging system and produce the required projections. Two phantoms, including the simple cardiac phantom and the NCAT phantom, were used to evaluate the scatter correction methods. The simulations were repeated 5 times for each phantom and finally, the mean values obtained from these 5 tests were used in the analysis of the results.Results:The obtained results from this study show that in the case of both investigated phantoms, the use of correction methods leads to improve the contrast of the images obtained from Tc-99m and Tl-201 radioisotopes. In the case of the simple cardiac phantom, the use of DEW and TEW correction methods leads to a relative increase in image contrast of about 23.88% and 12.23% for 99mTc radioisotope and about 29.19% and 20.98% for 201Tl radioisotope, respectively. This relative increase in the case of the NCAT phantom is about 22.48% and 19.43% for 99mTc radioisotope and about 27.74% and 24.74% for 201Tl radioisotope, respectively.Conclusion:According to the obtained results, despite the higher contrast of the noncorrected images of 99mTc radioisotope, the relative increase in contrast of the corrected images of 201Tl radioisotope is more than that of 99mTc radioisotope. Furthermore, for both radioisotopes, the relative increase related to the DEW method is higher than the TEW method.

Lung-to-heart ratio analysis using virtual planar images obtained from myocardial perfusion SPECT data: A phantom and clinical studies

BACKGROUNDSThe lung-to-heart ratio (L/H ratio) in myocardial perfusion scintigraphy (MPS) is a useful marker that complements the sensitivity of ischemia detection. However, it requires planar imaging acquired following a separate protocol in addition to single-photon emission computed tomography (SPECT). We developed a novel method for constructing virtual planar image (VPI) from SPECT data.METHODSMyocardial phantoms using Tl-201 were built with different amounts of radioactivity in the lungs. SPECT data and conventional planar images of these phantoms were collected with an Anger-type gamma camera. VPIs were constructed by adding all coronal images reconstructed from SPECT data. The clinical utility of VPIs obtained from 52 patients who underwent MPS with Tc-99m sestamibi was evaluated.RESULTSThe radioactivity linearity of VPIs was satisfactory, with a correlation coefficient of r ≥ .99 between the measured amounts of radioactivity and image counts. The L/H ratios obtained from VPI analysis were strongly correlated with those of conventional planar images with a correlation coefficient of r ≥ .99 in the phantom study and r = .929 in clinical application.CONCLUSIONThe accuracy of VPI-based L/H ratio analysis was comparable to that of conventional planar image-based analysis. VPIs could be used as an alternative method of obtaining planar images in clinical settings.

Simulating dose reduction for myocardial perfusion SPECT using a Poisson resampling method

PURPOSEThe purpose of this study was to determine the lowest Tl-201 dose that does not reduce the image quality of myocardial perfusion SPECT (MPS) by Poisson resampling simulation.METHODSOne hundred and twelve consecutive MPS data from patients with suspected or known coronary artery disease were collected retrospectively. Stress and rest MPS data were resampled using the Poisson method with 33%, 50%, 67%, and 100% count settings. Two nuclear medicine physicians assessed the image quality of reconstructed data visually by giving grades from - 2 to + 2. The summed stress score (SSS), summed rest score (SRS), and summed difference score (SDS) were obtained on the workstation. Image quality grades and semi-quantitative scores were then compared among these resampled images.RESULTSThe proportions of "adequate" image quality were 0.48, 0.75, 0.92, and 0.96 for the groups of images with 33%, 50%, 67%, and 100% data, respectively. The quality of the resampled images was significantly degraded at 50% and 33% count settings, while the image quality was not different between 67 and 100% count settings. We also found that high body mass index further decreased image quality at 33% count setting. Among the semi-quantitative parameters, SSS and SRS showed a tendency to increase with a decline in count.CONCLUSIONBased on the simulation results, Tl-201 dose for MPS can be reduced to 74 MBq without significant loss of image quality. However, the SSS and SRS can be changed significantly, and it needs to be further verified under the different conditions.
