Abstract Objective One of the adverse effects of in-office bleaching is tooth hypersensitivity, which could be neutralized with a desensitizing agent. In-office bleaching and the application of desensitizing agents will affect the morphology of the enamel surface. These morphological changes have potential effects on the shear bond strength (SBS) and bonding of orthodontic brackets. This study analyzed the effects of fluoride and nonfluoride desensitizing agents after in-office bleaching on the SBS of metal brackets with resin composite cementation. Materials and Methods Twenty-seven postextraction premolars (n = 27) were bleached with 37% hydrogen peroxide and then divided into three groups: control group (group 1), fluoride-based desensitizing agent (group 2), and nonfluoride-based desensitizing agent (group 3). After treatment, the brackets were bonded using Transbond, and then the SBS test was performed using a universal testing machine. The adhesive remnant index (ARI) was analyzed after the SBS test, whereas enamel morphological changes were observed under a scanning electron microscope. After this assessment, energy-dispersive X-ray spectroscopy was conducted to determine calcium and phosphorus elements in the enamel surface after treatments. SBS data were analyzed using the one-way analysis of variance, followed by the Tukey test, whereas ARI scores were subjected to the Kruskal–Wallis test and the Mann–Whitney test with a significance level of 5%. Results The SBS increased significantly in group 2 compared with groups 1 and 3 (p < 0.05). In group 2, an ARI of 2 (55.56%) was frequent, whereas in group 3, an ARI of 3 (55.56%) appeared most frequently. Conclusion The application of the desensitizing agent fluoride after in-office bleaching increased the SBS and more adhesive remains on the tooth surface compared with that when nonfluoride desensitizing agents were applied on metal brackets with composite resin cementation.

Preemptive use of ibuprofen and desensitizer decreases immediate tooth sensitivity after in-office bleaching

BACKGROUNDThe authors evaluated the synergistic analgesic effect of preemptive administration of 400 mg of ibuprofen (IBU) and potassium fluoride 2% (KF2), both in isolation or combination, on the painful perception of tooth sensitivity after tooth bleaching.METHODSFifteen patients participated in this triple-blind, randomized, placebo-controlled clinical trial. The study used a crossover design for drug administration and a split-mouth design for desensitizer agent. Four paired groups were formed: IBU plus KF2 (IBU + KF2); IBU (IBU + placebo KF); KF2 (placebo IBU + KF2), and placebo (placebo IBU + placebo KF). The outcome measure was the perception of tooth sensitivity, assessed using a visual analog scale. Data were collected at 4 different times: immediately after tooth bleaching (baseline) and after 6, 30, and 54 hours. Statistical analysis was performed using the Friedman test and relative risk.RESULTSIBU plus KF2 was found to be more effective in reducing tooth sensitivity immediately after bleaching (baseline) compared with the placebo group (P < .05). The risk of experiencing moderate or severe tooth sensitivity was approximately 4 times higher in the placebo group than in the IBU plus KF2 group (relative risk, 4.00; 95% CI, 1.01 to 15.81; P = .025).CONCLUSIONSThe combined use of 400 mg of IBU and KF2 appears to be beneficial in managing postbleaching tooth sensitivity. It provides a superior analgesic effect compared with placebo.PRACTICAL IMPLICATIONSThe preemptive administration of IBU plus KF2 reduces tooth sensitivity after tooth whitening when compared with placebo. This clinical trial was registered in the Brazilian Clinical Trials Registry Platform. The registration number is U1111-1249-8191.

Effect of a calcium phosphate-containing desensitizing agent on postoperative sensitivity: A split-mouth, randomized controlled study.

PURPOSETo evaluate the effectiveness of a calcium phosphate-containing-desensitizer (Teethmate Desensitizer - TD), caries type, subject age, and preoperative hypersensitivity on postoperative sensitivity (POS) after composite restorations on deep or extremely deep lesions.METHODS50 subjects, having two teeth with deep or extremely deep caries, participated in this study. TD was applied randomly to one tooth of each participant, and all teeth were restored with composite resin (Filtek Z250). After 1 week, POS was evaluated according to NRS (numerical rating scale) and VAS (visual analogue scale) by using participant diaries. At 6 weeks, POS was assessed considering subjects' reports. The normality of data was analyzed with Shapiro-Wilk test. For analyses, Pearson's chi-squared test, Mann-Whitney U and the Wilcoxon Signed-Rank test were used, and the effect sizes (ES) were calculated (α= 0.05).RESULTS47 of the participants completed the 6-week study. There was a small effect size noted for TD for NRS and VAS (P> 0.05, ES < 0.30). Also, there was no statistically significant difference between POS and subject age (P= 0.294, ES= 0.161), type of caries (P= 0.680, ES= 0.042) and preoperative sensitivity (P= 1.000, ES= 0.138) after the first week.CLINICAL SIGNIFICANCETeethmate Desensitizer had no significant effect on postoperative sensitivity occurrence with respect to caries type, subject age, and existence of preoperative sensitivity. The application of Teethmate Desensitizer demonstrated no significant relieving effect on postoperative sensitivity in deep or extremely deep cavities.
