Using regression tree analysis to examine demographic and geographic characteristics of COVID-19 vaccination trends over time, United States, May 2021–April 2022, National Immunization Survey Adult COVID Module

Using data from the nationally representative National Immunization Survey (NIS), we applied conditional linear regression tree methodology to examine relationships between demographic and geographic factors and propensity of receiving various doses of COVID-19 vaccine over time; these analyses identified temporal changes in these relationships that heretofore had not been identified using conventional logistical regression methodologies. Three regression tree models were built using an R package, Recursive Partitioning for Modeling Survey (rpms), to examine propensities over time of receiving a (1) first dose of a two-dose COVID-19 mRNA primary vaccination series or single dose of the Janssen vaccine (vaccine initiation), (2) primary series completion, and (3) monovalent booster dose, using a conditional linear effect model. Persons ≥50 years were more likely to complete a primary series and receive a first booster dose; persons reporting having received non-COVID-19 vaccines recently were more likely to initiate vaccination, complete the primary series, and get a first booster dose; persons reporting having work or school requirements were more likely to complete the primary series. Persons not reporting having received non-COVID-19 vaccines in 2 years but reporting having work or school vaccination requirements were more likely to initiate vaccination than those without work/school requirements. Among persons not reporting having received non-COVID-19 vaccines in 2 years and not reporting having work or school vaccination requirements, those aged ≥50 years were more likely to initiate vaccination than were younger adults. Propensity of receiving various doses was correlated with age, having recently received non-COVID 19 vaccines, and having vaccination requirements at work or school. Regression tree methodology enabled modeling of different COVID-19 vaccination dose propensities as a linear effect of time, revealed changes in relationships over time between demographic factors and propensity of receipt of different doses, and identified populations that may benefit from vaccination outreach efforts.

A vaccination-based COVID-19 model: Analysis and prediction using Hamiltonian Monte Carlo

Compartmental models have emerged as robust computational frameworks and have yielded remarkable success in the fight against COVID-19. This study proposes a vaccination-based compartmental model for COVID-19 transmission dynamics. The model reflects the specific stages of COVID-19 infection and integrates a vaccination strategy, allowing for a comprehensive analysis of how vaccination rates influence the disease spread. We fit this model to daily confirmed COVID-19 cases in Tennessee, United States of America (USA), from June 4 to November 26, 2021, in a Bayesian inference approach using the Hamiltonian Monte Carlo (HMC) algorithm. First, excluding vaccination dynamics from the model, we estimated key epidemiological parameters like infection, recovery, and disease-induced death rates. This analysis yielded a basic reproduction number ( __-mml:math xmlns:mml="http://www.w3.org/1998/Math/MathML"-__ R 0 ) of 1.5. Second, we incorporated vaccination dynamics and estimated the vaccination rate for three vaccines: 0.0051 per day for both Pfizer and Moderna and 0.0059 per day for Janssen. The fitted curves show reductions in the epidemic peak for all three vaccines. Pfizer and Moderna vaccines bring the peak down from 8,029 infected cases to 5,616 infected cases, while the Janssen vaccine reduces it, to 6,493 infected cases. Simulations of the model by varying the vaccination rate and vaccine efficacy were performed. A highly effective vaccine (95% efficacy) with a daily vaccination rate of 0.006 halved COVID-19 infections, reducing cases from 8,029 to around 4,000. The results also show that the model's prediction accuracy for new observations improves with the number of observed data used to train the model.

Mid-term follow-up after COVID-19 vaccination in adults with CHD: a prospective study

AbstractBackground:Long-term data on COVID-19 vaccine safety, immunogenicity, and acceptance in adults with CHD are lacking.Methods:This is a prospective study including adults with CHD patients undergoing COVID-19 vaccination from January 2021 to June 2022. Data on adverse events, antispike IgG titre, previous or subsequent COVID-19 infection, booster doses, and patients’ attitude towards vaccination were collected.Results:Four hundred and ninety CHD patients (36 ± 13 years, 53% male, 94% with moderate/complex defects) were prospectively included: 433 (88%) received a Pfizer–BioNTech mRNA vaccine, 31 (6%) Moderna mRNA vaccine, 23 (5%) AstraZeneca–Oxford ChAdOx1 nCov-19 vaccine, and 3 (0.6%) Janssen Vaccine; 310 (63%) received a booster dose. Median follow-up after vaccination was 1.53 [1.41–1.58] years. No major adverse event was reported. Eighty-two fully vaccinated patients contracted COVID-19 during follow-up after a median of 5.4 [4.3–6.5] months from the last dose. One patient with Ebstein’s disease died from severe COVID-19. Symptoms’ duration in patients who tested positive after vaccination was significantly shorter than in the group tested positive before vaccination (5.5 [3–8] versus 9 [2.2–15] days, p = 0.04). Median antispike IgG titre measured in 280 individuals (57%) at a median of 1.4 [0.7–3.3] months from the last dose was 2381 [901–8307] BAU/ml. Sixty patients (12%) also showed positive antinucleocapsid antibodies, demonstrating previous SARS-COV2 exposure. Twenty-nine percent appeared to have concerns regarding vaccine safety and 42% reported fearing potential effects of the vaccine on their cardiac disease before discussing with their CHD cardiologist.Conclusion:COVID-19 vaccines appear safe in the mid-term follow-up in adults with CHD with satisfactory immunogenicity and reduction of symptoms’ duration in case of infection.
