Investigation into the effects of acute heat stress on stress level, meat quality, myofibrillar proteins properties and serum metabolites release of Muscovy ducks (Cairina moschata)

This study systematically investigated the effects of acute heat stress (AHS) on Muscovy ducks, focusing on plasma stress indicators, meat quality, myofibrillar proteins (MPs) properties, and serum metabolite profiles. AHS significantly elevated stress level, impairing meat quality, as evidenced by higher L* value, shear force, drip loss, and cooking loss (P < 0.05). It also induced MPs oxidation and aggregation, reflected by elevated carbonyl content, turbidity, zeta potential, and particle size. MPs structural alterations were confirmed by fluorescence quenching and increased exposure of hydrophobic groups. Correlation analysis revealed strong associations between physicochemical changes and MPs oxidation. Metabolomic analysis identified 161 and 105 differential metabolites in the CON vs. LS and CON vs. SS comparisons, respectively, involving 25 metabolic pathways related to energy, amino acids, and fatty acids. These findings provide novel insights into the molecular pathways of AHS-induced meat quality deterioration and reveal potential intervention timing for the poultry industry.

Severe Neutropenia Due to Carbimazole and the Importance of Early Recognition and Management

Agranulocytosis is a rare but potentially life-threatening adverse effect of antithyroid medications, particularly carbimazole. This case report presents a well-structured and clinically relevant discussion of a rare but serious adverse effect of carbimazole therapy in a patient with Graves' disease. The case highlights the importance of early recognition and prompt discontinuation of the drug, which are crucial to preventing severe complications. We present the case of a 51-year-old female patient who was recently diagnosed with Graves' disease and developed agranulocytosis, leading to neutropenic sepsis after initiating carbimazole therapy. She initially presented to the medical same-day emergency care (SDEC) unit five weeks earlier with complaints of palpitations for two months, accompanied by weight loss, heat intolerance, irritability, and excessive sweating. Her thyroid function tests revealed a thyroxine (T4) level of 61.1 pmol/L and a thyroid-stimulating hormone (TSH) level of <0.01 mIU/L. An electrocardiogram (ECG) showed sinus tachycardia, while a thyroid ultrasound indicated an enlarged thyroid gland consistent with thyroiditis, along with a single U2 nodule in the right lobe. She was prescribed carbimazole and propranolol and was referred for outpatient endocrinology follow-up. After 4-5 weeks of treatment, she developed a fever, sore throat, and fatigue. A routine blood test ordered by her general practitioner (GP) revealed severe neutropenia, consistent with agranulocytosis. Carbimazole was immediately discontinued, and she was treated with broad-spectrum antibiotics and granulocyte colony-stimulating factor (G-CSF), leading to a gradual recovery. Carbimazole-induced agranulocytosis typically occurs within the first few months of therapy. Early discontinuation of the drug is associated with a favorable prognosis. Alternative treatment options, including radioactive iodine therapy and thyroidectomy, should be considered for long-term disease management.

Data-Driven Strategies for Carbimazole Titration: Exploring Machine Learning Solutions in Hyperthyroidism Control

AbstractBackgroundUniversity Hospitals Dorset (UHD) has more than 1000 thyroid patient contacts annually. These are primarily patients with autoimmune hyperthyroidism treated with carbimazole titration. Dose adjustments are made by a healthcare professional (HCP) based on the results of thyroid function tests, who then prescribes a dose and communicates this to the patient via letter. This is time consuming and introduces treatment delays. This study aimed to replace some time-intensive manual dose adjustments with a machine learning model to determine carbimazole dosing. This can in the future serve patients with rapid and safe dose determination and ease the pressures on HCPs.MethodsData from 421 hyperthyroidism patients at UHD were extracted and anonymized. A total of 353 patients (83.85%) were included in the study. Different machine learning classification algorithms were tested under several data processing regimes. Using an iterative approach, consisting of an initial model selection followed by a feature selection method, the performance was improved. Models were evaluated using weighted F1 scores and Brier scores to select the best model with the highest confidence.ResultsThe best performance is achieved using a random forest (RF) approach, resulting in good average F1 scores of 0.731. A model was selected based on a balanced assessment considering the accuracy of the prediction (F1 = 0.751) and the confidence of the model (Brier score = 0.38).ConclusionTo simulate a use-case, the accumulation of the prediction error over time was assessed. It was determined that an improvement in accuracy is expected if this model was to be deployed in practice.
