Methyl bromide is a pre-plant soil fumigant that is widely used to control nematodes, insects, and fungi in farmlands. Methyl bromide enters the human body through dermal absorption or inhalation and can damage the respiratory, nervous, circulatory, urinary, and other systems. A 62-year-old man who had inhaled a large amount of methyl bromide was admitted to our department. He presented with respiratory failure and pink foamy sputum. He was started on dexamethasone, alanyl glutamine, sulbactam, furosemide, vitamin B1, mouse nerve growth factor, and other treatments, and mechanical ventilation and continuous venovenous hemodiafiltration (CVVHF) were performed daily. He subsequently developed coagulopathy because of the CVVHF, for which protamine, recombinant human brain natriuretic peptide, and albumin were administered intravenously. Notably, the patient developed sustained anuria and eventually died owing to multiple organ failure; specifically, failure of the brain, heart, lungs, and kidneys. This report presents the diagnosis, clinical course, management, and prognosis of a patient who was treated at our hospital for severe methyl bromide poisoning.

[Treatment of sudden deafness with type 2 diabetesmellitus by post-auricular subperiosteal injection of methylprednisolone].

Objective:To investigate the therapeutic effect of post-auricular subperiosteal injection of methylprednisolone combined with ginkgo biloba injection and mouse nerve growth factor in sudden deafness with type 2 diabetesmellitus. To understand the effect of post-auricular subperiosteal injection of methylprednisolone on type 2 diabetesmellitus complicated with sudden deafness and its effect on blood-glucose level, and to explore a more effective and safe method for treating this special group of sudden deafness. Method: Total 84 patients suffered from type 2 diabetesmellitus with sudden deafness and treated in Shaanxi Provincial People's Hospital from March 2017 to June 2018 were randomly divided into experimental group(post-auricular subperiosteal injection of methylprednisolone+ginkgo biloba injection, mouse nerve growth factor)(n=48) and a control group(ginkgo biloba injection, mouse nerve growth factor)(n=36). Each group was divided into high frequency, low frequency and flat and total deafness group. Then to compare the clinical efficacy of the two groups and the difference of the fluctuation of blood glucose and HbA_1c before treatment and 2 hours after treatment. Result:①The effective rate of treatment in the experimental group was 79.17%, higher than that in the control group(58.33%), P<0.05. The results indicated that the effect of post-auricular subperiosteal injection of methylprednisolone combined with ginkgo biloba injection and mouse nerve growth factor was effective in sudden deafness with type 2 diabetesmellitus. ②There was no significant difference in the fluctuation of blood glucose and HbA_1c before treatment and 2 hours after treatment in the experimental group, and it indicated that this treatment can effectively avoid systemic side effects of steroid and ensure the safety of the treatment. Conclusion:Post-auricular subperiosteal injection of methylprednisolone combined with ginkgo biloba injection and mouse nerve growth factor is effective in patients with sudden deafness with type 2 diabetesmellitus, furthermore, blood glucose will not be affected by the using of steroid, and it will aviod systemic hormonal side effects.

[To study the effect of intratympanic injection with mouse nerve growth factor and methylprednisolone on sudden deafness].

Objective:To observe the efficacy of intratympanic injection with mouse nerve growth factor and methylprednisolone on the treatment of sudden deafness.Method:One hundred and forty-nine patients diagnosed as sudden deafness were randomly divided into the control group and the observation group. The control group was treated with the conventional medication and methylprednisolone(0.5 ml)+0.9%NaCl(0.5 ml), once every three days for two weeks. The observation group was added with mouse nerve growth factor(0.5 ml) once every three days for two weeks.Result:The total effective rate in the observation group of hearing, tinnitus and vertigo were respective 93.4%, 77.8%,85.2%, which were higher than 80.8%, 63.0%, 67.2%.in the control group.And the differences between them had statistical significance(P< 0.05).Conclusion:Effect of the intratympanic injection with mouse nerve growth factor and methylprednisolone on the treatment of sudden deafness is obvious, which can effectively improve hearing level, and relieve tinnitus and vertigo.
