The purpose of this study is to determine the ideal dose of spinal morphine for use in Cesarean section. Spinal anesthesia (single injection in the lower back to numb patients from the waist down) is commonly used in Cesarean section to provide numbness and pain relief during the surgery, and adding morphine to the spinal anesthetic provides long lasting pain relief for up to 24 hours after surgery. The ideal dose of spinal morphine, when given with other types of pain medications such as nonsteroidal anti-inflammatories and acetaminophen, has not been determined. In addition, spinal morphine can have side effects such as nausea and itching, so using a lower dose of morphine may decrease these side effects while providing the same amount of postoperative pain relief. Study participants will be divided into two groups. Group 1 will receive the standard dose of spinal morphine (0.1mg) while Group 2 will receive a lower dose of spinal morphine (0.05mg). Both groups will receive the standard dose of spinal bupivacaine (numbing medication) and spinal fentanyl (short acting pain medication). The additional pain medications (IV Toradol and oral acetaminophen) will be given to both groups after surgery. Pain control and morphine side effects will be compared between the two groups in order to determine the best dose of spinal morphine for cesarean section.

A Prospective Double Blind Randomized Comparison of Methadone, Fentanyl, and Morphine for Post-laparotomy Epidural Analgesia.

The current study aim is to compare between the opioids morphine, fentanyl and methadone in terms of efficacy and side effects profile when administered epidurally for pain management after laparotomies. Intravenous morphine will also be used. The outcomes of that method will be compared to those obtained using epidurally administered opioids, as this is another commonly used postoperative pain relief method.

Dietary inclusion of micelle silymarin enhances egg production, quality, and lowers blood cholesterol in Hy‐line brown laying hens

AbstractThis study aimed to evaluate the effect of micelle silymarin (MS) supplementation on productive performance, egg quality, and blood biochemical parameters in laying hens. A total of 384 (Hy‐Line brown, 28‐week‐old) laying hens were randomly distributed into one of four dietary treatment groups (eight replication per treatment; 12 hens per replicate) for a 12‐week long feeding trial. Hens were fed a basal diet with the addition of 0%, 0.02%, 0.04%, and 0.06% of MS respectively. The results expressed that egg production percentage and egg weight were linearly (p < 0.05) enhanced at Weeks 0–6, 7–12, and 0–12, when 0%, 0.02%, 0.04%, and 0.06% of MS were added to laying hen diets. The feed conversion ratio and feed intake improved (p < 0.05) linearly at Weeks 0–6, 7–12, and 0–12 with increasing levels of MS supplementation in laying hens. The eggshell thickness and eggshell strength were linearly (p < 0.05) improved at weeks 4, 8, and 12 with an MS‐supplemented diet. Additionally, eggshell colour, Haugh unit, and albumin height had no significant (p > 0.05) difference throughout the experimental period with different levels of MS‐supplemented diet. The yolk colour of eggs showed significant (p < 0.05) differences at weeks 8 and 12 with levels of the MS‐supplemented diet. However, at the end of the trial, the blood profile indicated that cholesterol levels decreased (p < 0.05) linearly, and triglyceride levels showed a tendency to decrease (p < 0.10) with MS supplementation. In conclusion, increasing the level of MS addition in the laying hen diet improved the egg production percentage and egg quality, reducing cholesterol levels in Hy‐Line brown hens. However, this study indicates that MS can be added to the diet of laying hens up to 0.06% for improved egg production and egg quality.

Pb contaminated soil from a lead-acid battery plant immobilized by municipal sludge and raw clay

Soil heavy metal pollution poses a serious threat to the eco-environment. Municipal sludge-based passivators and clay minerals have been widely applied to immobilize heavy metal contamination in soils. However, little is known about the immobilization effect and mechanisms of raw municipal sludge and clay in reducing the mobility and bioavailability of heavy metals in soils. Here, municipal sludge, raw clay and mixtures of thereof were used to remediate Pb-contaminated soil from a Pb-acid battery factory. The remediation performance was evaluated through acid leaching, sequential extraction, and plant assay. Results showed that the leachable Pb content in the soil decreased from 5.0 mg kg^-1 to 4.8, 4.8 and 4.4 mg kg^-1 after 30 d of remediation with MS and RC added at equal weights to give total dosage of 20, 40 wt% and 60 wt %, respectively. The leachable Pb further decreased to 1.7, 2.0 and 1.7 mg kg^-1 after 180 d of remediation. Speciation analysis of the soil Pb indicated that the exchangeable and Fe-Mn oxide-bound Pb were transformed into residual Pb in the early stage of remediation, and the carbonate-bound Pb and organic matter-bound Pb were transformed into residual Pb in the later stage of remediation. As a result, Pb accumulation in mung beans decreased by 78.5%, 81.1% and 83.4% after 180 days of remediation. These results indicate that the leaching toxicity and phytotoxicity of Pb in remediated soils were significantly reduced, presenting a better and low-cost method for soil remediation.

Efficacy of micrographic surgery versus conventional excision in reducing recurrence for basal cell carcinoma and squamous cell carcinoma: A systematic review and meta‐analysis

AbstractThe standard of care for basal cell carcinoma (BCC) and squamous cell carcinoma (SCC) involves excision by conventional surgery (CS) with a predefined safety margin of resection or micrographic surgery (MS) with microscopic margin control. Previous studies have reported the superiority of MS in reducing recurrences for high‐risk BCC and SCC. This systematic review aimed to assess MS and CS recurrence rates by including randomized clinical trials (RCTs) and cohort studies. A systematic review and meta‐analysis were conducted for related studies in PubMed, LILACS, Embase, Scopus, Web of Science, CINHAL and Cochrane until May 2023. RCTs and cohorts involving patients with BCC or SCC submitted to MS and CS were included. Risk of bias assessment followed Cochrane‐recommended tools for RCTs and cohorts, and certainty of evidence followed the GRADE approach. Pooled estimates were used to determine the relative risk (RR) and absolute risk difference (RD) using a random‐effects model. Seventeen studies were included, two RCTs and fifteen cohorts. There were 82 recurrences in 3050 tumours submitted to MS, with an overall recurrence rate of 3.1% (95% CI 2.0%–4.7%). For CS, there were 209 recurrences in 3453 tumours, with a recurrence rate of 5.3% (95% CI 2.9%–9.3%). The combined estimate of RR was 0.48 (95% CI 0.36–0.63), without heterogeneity nor evidence of publication bias (p > 0.3). The RD resulted in 2.9% (95% CI 1.0%–4.9%; NNT = 35). Regarding subgroup analysis, the RR for BBC was 0.37 (95% CI 0.25–0.54), and RD was 3.7% (95% CI 0.8%–6.5%; NNT = 28). For SCC, RR was 0.57 (95% CI 0.29–1.13), and RD was 1.9% (95% CI 0.8%–4.7%; NNT = 53). Among primary tumours, RR was 0.39 (95% CI 0.28–0.54), and for recurrent tumours was 0.67 (95% CI 0.30–1.50). There is moderate evidence based on two RCTs, and low evidence based on 15 cohort studies that MS is superior to CS in reducing recurrences of BCCs and primary tumours. The development of protocols that maximize the cost‐effectiveness of each method in different clinical scenarios is paramount.
