We report an unusual case of a 30-year-old man with a primary mature cystic teratoma in the right ventricle. This case constitutes the third case of primary intramyocardial teratoma in an adult and the first that describes its benign form. In this report, we discuss surgical implications of such a rare cardiac tumor.

The reform movement in healthcare is occurring in response to financial pressures on American business. The trend toward managed care with competition-based discounting and capitation contracts is likely to continue even if Congress does not enact healthcare reform legislation. Efficient management of resources with acceptable outcomes and customer satisfaction is becoming increasingly crucial. As cost-shifting options are reduced, accurate knowledge and management of resource consumption costs become critical to enable hospitals to assume the financial risks associated with competing in any future capitation area. Hospitals must consider altering their approach to utilization management and business as usual to survive these changes.

External cardiac pacing in right ventricular infarction

A patient with acute inferior wall myocardial infarction presented with clinical evidence of right ventricular infarction complicated by bradycardia and cardiogenic shock. Pharmacologic interventions produced no change in heart rate or blood pressure, and a transvenous pacemaker failed to capture the infarcting right ventricle. An external transthoracic pacemaker immediately increased the heart rate with a marked hemodynamic improvement. In the setting of right ventricular infarction, external pacing may be more effective than transvenous pacing, perhaps due to its ability to pace the left ventricle.
