Midterm Outcomes of Shoulder Dislocations in Brachial Plexus Birth Injuries Treated With Injection Botulinum Toxin A

Background:Shoulder dislocation can be seen early in infancy and can be diagnosed by shoulder ultrasound. Botulinum toxin injection in the internal rotators of the shoulder can be used to as an adjunct to relieve the internal rotation contracture. The aim of this study was to evaluate the intermediate-term outcomes of shoulder dislocations treated with injection botulinum toxin and to identify the factors determining the outcomes of the procedure.Methods:Eighty-six children with ultrasound diagnosed shoulder dislocation underwent injection BTX-A between 2011 and 2022. Children who underwent nerve surgeries and the ones with inadequate follow-up were excluded. Sixty-two children were followed up for a mean duration of 36 months. Injection botulinum toxin A 2 U/kg body weight was injected each into the subscapularis and the pectoralis major. After manual stretching of contracted anterior soft tissue and shoulder capsule, a shoulder spica was applied for 3 weeks. Hospital for Sick Children Active Movement Scale (HSC-AMS) score and active and passive shoulder was recorded before and after the procedure and the requirement for further interventions was noted.Results:Outcomes were divided into 3 categories: group A—27 (44%) children who did not require any further surgery in our follow-up period; group B—27 (44%) needed further procedure for recurrence of IR contracture; and group C—8 (12%) developed external rotation contracture later. Children in group A had statistically significant higher preinjection HSC-AMS score for elbow flexion and shoulder abduction than the other groups. Children in group C had greater passive external rotation and weak elbow extension and wrist dorsiflexion as compared with the other 2 groups.Conclusions:Injection botulinum toxin A is effective in treating shoulder dislocations in children with early recruitment of C5 to 6 muscles, while children with involvement of C7 muscles may develop an external rotation contracture subsequently, hence this procedure should be avoided.Level of evidence:Level IV—therapeutic.

In Vivo Pressure Responses of the Cervical Cerebrospinal Fluid in a Porcine Model of Extension and Flexion Whiplash Exposures

PURPOSEThe mechanisms of whiplash injury remain poorly understood. One theory suggests that the characteristic inertial loading of the head and neck in motor vehicle collisions can produce injurious cerebrospinal fluid (CSF) pressure transients in the cervical spine. However, these in vivo CSF pressure responses have not yet been adequately characterized.METHODSThis study used a pig model to characterize the cervical CSF pressure responses to head kinematic inputs in extension (simulating low-speed rear-end collisions with no head restraint) and flexion (simulating low-speed frontal collisions). We also compared the pressure and pressure impulses at three spinal levels to determine if the pressure transient responses differ spatially. Four anesthetized pigs were instrumented with intrathecal pressure transducers placed at the C2, C5, and C7 levels. A servomotor system was programmed to actuate the head through specific trajectories to model two extension, and two flexion, whiplash exposures.RESULTSDuring the extension tests, mean peak pressure transients ranged from - 31.2 to 148.7 mmHg, whereas during the flexion tests, mean peak pressure transients ranged from - 50.8 to 126.9 mmHg. Peak individual responses ranged from - 71.1 to 244.8 mmHg across all tests. Pressure impulses reached a maximum of 6.77 mmHg·s. Peak pressure and pressure impulses were largest at the C5 and C7 levels during extension exposures and at the C2 level in flexion exposures.CONCLUSIONThe reported pressure and pressure impulse responses could be used to determine neural tissue tolerances relevant to whiplash injury and contribute to the development and validation of computational models of whiplash.

The transcriptome landscape of Kumrogarh, a unique rice landrace showing the simultaneous presence of Sub1 and SK loci for submergence tolerance

To decipher the molecular mechanism behind submergence tolerance in a typical rice genotype (var. Kumrogarh), leaf transcriptome analysis was performed on submerged plant tissue with 7 and 14 days of induced submergence, followed by cataloguing the differentially expressed transcripts. Subsequent bioinformatics analysis identified 5,267 differentially expressed genes (DEGs), of which 2,657 were upregulated and 2,610 were downregulated in four comparative combinations: T7-C7, T14-T7, T14-T7, and C14-C7. A group of 41 co-expressed genes was found across all sets, while 1427, 558 and 83 transcripts were uniquely expressed in the T7-C7, T14-T7, and C14-C7 combinations, respectively. Constructed Ven diagram showed that 1428, 65, and 44 transcripts were commonly expressed in the paired combinations "T7-C7" and "T14-T7", "C14-C7" and "T7-C7", and "C14-C7" and "T14-T7". Gene ontology study functionally categorized the DEGs into molecular functions, biological processes, and cellular components. Additionally, nine transcription factor families were identified, including MYB, WRKY, bZIP, bHLH, SET domain, NAC domain, C2H2 zinc finger, E2F, and HSF, along with a set of differentially regulated signalling genes. Twelve genes related to submergence adaptation were selected for final validation through quantitative real-time PCR-based expression analysis, which demonstrated a strong association with a coefficient (R ² = 0.716) after aligning with the RNA-Seq data. Derived results showed upregulation of gibberellin receptor GID1L2 (LOC_Os02g35940.1), ethylene-responsive element-binding protein (LOC_Os06g08360.1), glyceraldehyde-3-phosphate dehydrogenase (LOC_Os04g38600.1), decarboxylase (LOC_Os08g04540.1), sucrose synthase (LOC_Os03g22120.1), aldehyde dehydrogenase (LOC_Os12g07810.1), endonuclease/exonuclease/phosphatase family domain-containing protein (LOC_Os01g08780.1), polygalacturonase inhibitor 1 precursor (LOC_Os07g38130.1), transmembrane amino acid transporter protein (LOC_Os01g41420.1), and SAM-dependent carboxyl methyltransferase (LOC_Os02g48770.1). This study provides a comprehensive profile of leaf transcriptomics in a traditionally tall-type rice landrace containing both submergence-tolerant Sub1 and SK alleles, highlighting an area of research that remains largely unexplored. These remarkable findings have driven this investigation to decipher the interplay among these key genetic factors by hypothesizing a model leading to the development of a genetic network associated with improved survival under prolonged deep submergence of such a unique rice genotype.Supplementary InformationThe online version contains supplementary material available at 10.1007/s13205-025-04277-7.
