Targeted nanoliposomes for precision rheumatoid arthritis therapy: a review on mechanisms and in vivo potential

Rheumatoid arthritis (RA) is an inflammatory immune-triggered disease that causes synovitis, cartilage degradation, and joint injury. In nanotechnology, conventional liposomes were extensively investigated for RA. However, they frequently undergo rapid clearance, reducing circulation time and therapeutic efficacy. Additionally, their stability in the bloodstream is often compromised, resulting in premature drug release. The current review explores the potential of targeted liposomal-based nanosystems in the treatment of RA. It highlights the pathophysiology of RA, explores selective targeting sites, and elucidates diverse mechanisms of novel liposomal types and their applications. Furthermore, the targeting strategies of pH-sensitive, flexible, surface-modified, PEGylated, acoustic, ROS-mediated, and biofunctionalized liposomes are addressed. Targeted nanoliposomes showed potential in precisely delivering drugs to CD44, SR-A, FR-β, FLS, and toll-like receptors through the high affinity of ligands. In vitro studies interpreted stable release profiles and improved stability. Ex vivo studies on skin demonstrated that ultradeformable and glycerol-conjugated liposomes enhanced drug penetrability. In vivo experiments for liposomal types in the arthritis rat model depicted remarkable efficacy in reducing joint swelling, pro-inflammatory cytokines, and synovial hyperplasia. In conclusion, these targeted liposomes represented a significant leap forward in drug delivery, offering effective therapeutic options for RA. In the future, integrating these advanced liposomes with artificial intelligence, immunotherapy, and precision medicine holds great promise.

Combined Molecular Subclass and Immune Phenotype Correlate to Atezolizumab Plus Radiation Therapy Response in Invasive Bladder Cancer: BPT-ART Phase 2 Study

PURPOSEBladder preservation therapy in combination with atezolizumab and radiation therapy trial, which was a multicenter, open-label, single-arm phase 2 study, showed a promisingly high interim clinical complete response (cCR) rate of 84.4% (38/45). In the present study, we aimed to identify potential tissue biomarkers for achieving cCR using bladder preservation therapy in combination with atezolizumab and radiation therapy.METHODS AND MATERIALSWe used tumor tissue samples of the bladder and blood samples collected from patients at baseline to analyze the tumor immune microenvironment at baseline using an integrated approach of immunophenotyping, genomic, and tumor-infiltrating lymphocyte (TIL) profiling.RESULTSImmune phenotype analysis revealed that cCR rates of patients with the desert phenotype were as similarly high as patients with excluded/inflamed phenotypes (73.3% [11/15] vs 93.3% [14/15], P = .33) despite lower programmed death-ligand 1 expression levels in the desert phenotype. Genomic and TIL profiling then revealed that increased CD8+ and CD204+ TIL infiltration, high CD8:forkhead box protein P3 ratios in the stroma of the excluded/inflamed phenotypes, and gene alterations, such as CDK12, GNAS, NOTCH2, and AR1D1A, were associated with a high cCR rate (93.3%). Furthermore, the characteristics of these dual TILs, CD8-forkhead box protein P3 ratios, and gene alterations (especially FGFR3) bifurcated the desert phenotype into 2 subgroups with different cCR rates (100% [11/11] and 0% [0/4]).CONCLUSIONSThese potential subgroups, defined by combined molecular subclass and immune phenotype, could lead to the identification of good responders to atezolizumab plus radiation therapy for invasive bladder cancer. However, given the small cohort size and limited number of tumor samples, these findings should be viewed as hypothesis-generating and require further validation in larger studies.

Structural analysis of substrate recognition loop flexibility in D-arabinose dehydrogenase from Candida auris

Candida auris is an emerging fungal pathogen that poses a significant threat to global health due to its multidrug resistance and ability to persist in healthcare settings. A key factor contributing to its survival and virulence is its capacity to combat oxidative stress, a process primarily driven by oxidative stress-related enzymes. One such enzyme, d-arabinose dehydrogenase from C. auris (CaAldO), plays a crucial role in the biosynthesis of d-erythroascorbic acid (EASC), an essential antioxidant that shields fungal cells from oxidative damage. CaAldO catalyzes the oxidation of d-arabinose to D-arabinono-1,5-lactone, a key precursor in EASC synthesis, thereby enhancing the oxidative stress resistance of C. auris. To elucidate its structural features, we determined the high-resolution crystal structure of CaAldO at 1.95 Å. Its cofactor-binding pocket is formed by four loop regions within the TIM-barrel fold. Notably, Loops A and C in the substrate-binding pocket exhibit significant flexibility, facilitating the transition between the open and closed conformations of the cofactor-binding pocket of CaAldO. A structural comparison of CaAldO with its homolog ScAra1 revealed notable differences in the length and conformation of the substrate recognition loops, as well as variations in the cofactor-binding pocket. These findings enhance our understanding of the unique structural properties of CaAldO and offer insights into developing novel antifungal strategies targeting C. auris.
