Elevated Fecal Biomarkers of Colo‐Rectal Epithelial Cell Activity in Irritable Bowel Syndrome

ABSTRACTBackgroundIrritable bowel syndrome (IBS) is a common functional gastro‐intestinal disorder characterized by discomfort with constipation and/or diarrhea with unclear pathophysiology. We aimed to determine the activities of colorectal eosinophils, neutrophils and epithelial cells by biomarkers in feces reflecting these activities.MethodsFecal samples were collected from 185 patients with IBS before and after 8 weeks of placebo or mesalazine treatment and from 40 healthy subjects. Calprotectin, eosinophil derived neurotoxin (EDN), eosinophil cationic protein (ECP), human neutrophil lipocalin (HNL) (pab/765) or dimer, human phospholipase BII‐precursor (HPLBII‐P) and myeloperoxidase (MPO) were measured by ELISA. Symptom scores were evaluated by diaries.ResultsHPLBII‐P, HNL (pab/765) and EDN, proteins secreted by intestinal epithelial cells, were elevated in IBS patients as compared to healthy subjects (p < 0.0001–p = 0.008). In contrast, the neutrophil proteins calprotectin, MPO and HNL dimer were unaltered. The eosinophilic protein ECP was lower in IBS (p = 0.001). HNL (pab/765) (p = 0.01) and EDN (p = 0.004) increased in IBS patients after mesalazine treatment. Colo‐rectal mucosa showed strong staining of HPLBII‐P and western blotting of fecal extracts showed the presence of mainly monomeric, epithelial‐associated HNL.ConclusionsThe absence of signs of involvement of neutrophils and eosinophils in IBS suggests that activity of local epithelial cells rather than inflammation may be a major determinant of the disease. The measurements of EDN, HNL (pab/765), and HPLBII‐P may serve as potential fecal biomarkers in the study and monitoring of IBS.

AtPHR1 and SlPHL1 positively regulate phosphorus remobilization in response to phosphate starvation by directly facilitating the transcription of genes encoding S-like ribonucleases in Arabidopsis and tomato, respectively

Phosphorus (P) is a crucial element for plant growth and development, with inorganic phosphate (Pi) being the primary form absorbed by plant roots. However, soil often experiences Pi deficiency, leading to early leaf senescence and hastening the P re-mobilization from senescent leaves to young tissues. The PHOSPHATE STARVATION RESPONSE (PHR) proteins are widely acknowledged as pivotal regulatory factors in response to low Pi, and they may be involved in the regulation of leaf senescence and P remobilization triggered by Pi deprivation. Nevertheless, the mechanisms underlying the regulation of these processes require further elucidation. In this study, it was observed that overexpression of AtPHR1 accelerates leaf senescence, facilitates P transfer, and upregulates the expression of S-like RNase-encoding genes such as RNS1, RNS2, and RNS5. Subsequent investigations utilizing yeast one-hybrid (Y1H), electrophoretic mobility shift assay (EMSA), and transient transcription expression assay (TTEA) revealed direct binding of AtPHR1 to the P1BS motifs within the promoters of these genes, thereby activating their transcription. Similarly, the overexpression of tomato PHR protein SlPHL1 also increased the transcript levels of the S-like RNase-encoding genes including LE and LX. Moreover, experiments using Y1H, EMSA, and TTEA confirmed that SlPHL1 directly binds to the promoters of these two genes through P1BS motifs and subsequently activates their transcription. Additionally, introducing SlPHL1 into Arabidopsis led to an enhanced P turnover and RNA degradation, along with an increase in the expression of RNS1 and RNS2. Further investigations revealed that similar to AtPHR1, SlPHL1 is capable of directly activating the transcription of RNS1 and RNS2. Taken together, these findings suggest that AtPHR1 and SlPHL1 play a pivotal role in facilitating P remobilization by directly stimulating the transcription of S-like RNase-encoding genes. This mechanism appears to be conserved across Arabidopsis and tomato, highlighting the importance of PHRs in optimizing plant P utilization.

Effect of benralizumab treatment on the airway microbiome in COPD

BackgroundOne-third of patients with COPD have an eosinophilic inflammatory phenotype. Benralizumab is an afucosylated humanised monoclonal antibody that targets the interleukin-5 receptor α subunit, leading to rapid and near-complete eosinophil depletionviaantibody-dependent cellular cytotoxicity. We hypothesised that benralizumab-targeted immune modulation could have an impact on the airway microbiome in COPD. The objective of the present study was to investigate the effect of benralizumab treatment on inflammation and the sputum microbiome in COPD.MethodsSputum samples from 94 COPD patients enrolled to the GALATHEA trial (NCT02138916) and randomised to receive placebo (33), benralizumab at 100 mg (29) or 30 mg (32) over 52 weeks were analysed at baseline, week 24 and at end of treatment (week 56). Sputum microbiota taxonomic profiles and diversity indices, generated from paired-end Illumina sequencing targeting the 16S rRNA gene, were used for comparative analyses. Linear mixed model analyses were applied to blood and sputum cell counts and eosinophil mediators for within- and between-treatment group analyses.ResultsParticipants treated with 100 and 30 mg benralizumab, respectively, showed a significant reduction from baseline in both blood and sputum eosinophil counts (blood: p=1.2e-10 and p=8.8e-10; sputum p=0.03 and p=0.004) and eosinophil-derived serum mediators (eosinophil cationic protein: p<3e-09 and p<2e-08; eosinophil-derived neurotoxin: p<8e-12 and p<2e-09). No significant changes in the composition or diversity of the sputum microbiome were observed.ConclusionsIn this study, the airway microbiome measured in sputum was unaffected by a targeted reduction of eosinophilic inflammation with benralizumab treatment.
