Shared and unique genes and pathways between neuropathic and inflammatory pain assays

BACKGROUNDCurrent studies mostly concentrate on behavioral differences and have not yet systematically elucidated the molecular distinctions among various chronic pain models.METHODSTo identify the similarities and differences in gene expression among mice of three kinds of pain models, i.e., spared nerve injury (SNI) model, chronic constriction injury of the sciatic nerve (CCI) model, and the complete Freund's adjuvant-induced chronic inflammatory pain (CFA) model. The lumbar enlargement segments (L5-L6) were collected. Total mRNA was extracted for RNA sequencing. The differentially expressed genes were analyzed by bioinformatics, including GO analysis, KEGG analysis, and PPI network to explore the functions.RESULTSCommonalities and significant variations in gene expression were observed among the three pain models. Compared with Sham, there were 60 shared differential genes among the three models, which were mainly involved in oxidative phosphorylation-related biological process (e.g., mt-Nd1). Compared with CCI, SNI upregulated genes were associated with inflammation response (e.g., Ifi204, Ifi27), while downregulated genes were linked to microtubule-based movement (e.g., Dnah7b, Hcmn1); When compared with SNI, CFA upregulated genes were related to axon development (e. g., Oprm1, Gucy1a2, Syn3), whereas downregulated genes were associated with oxidative phosphorylation (e. g., Rpl41, Rpl21); In contrast to CCI, CFA upregulated genes pertained to axon development (e. g., Zbtb16), while downregulated genes were connected to oxidative phosphorylation (e. g., Cyp3a13).CONCLUSIONSThe three widely employed chronic pain models exhibit both similarities and distinctions, and genes that vary across all three models may serve as potential targets for chronic pain research.

Occurrence of Monoclonal Band in Alpha Region on Serum Electrophoresis in Multiple Myeloma: A Diagnostic Conundrum

The presence of an M-band in serum and/or urine electrophoresis is a crucial diagnostic indicator for multiple myeloma and other plasma cell dyscrasias. Conventionally, on serum protein electrophoresis (SPEP), the M-band is observed as a sharp spike (M-spike), typically in the gamma region, occasionally in the beta region, and rarely in the alpha region. However, the infrequency of M-bands in the alpha region raises the possibility of overlooking them when coexisting with normal bands. This study underscores the significance of considering the potential discovery of M-bands in unusual sites, particularly in the alpha-2 region. This retrospective study spanned three years and involved conducting serum and urine protein electrophoresis followed by serum and urine immunofixation electrophoresis on a semi-automated platform (Helena Biosciences, Europe) using agarose gel and serum protein 6-band format kits. We identified five cases of multiple myeloma presenting with a monoclonal band in the alpha-2 region on SPEP and UPEP. Subsequent immunofixation electrophoresis confirmed these bands to be monoclonal bands in the lambda lane. In suspected cases of multiple myeloma, even in the absence of an M-band in the gamma region, heightened prominence in the alpha or beta region warrants further investigation through immunofixation electrophoresis (IFE).

Soluble Guanylate Cyclase Dysfunction and Nitric Oxide Pathway in Chronic Rhinosinusitis With Nasal Polyps: Predictive Markers for Postoperative Recurrence

Background Elevated nitric oxide (NO) levels have been linked to a heightened risk of recurrence in patients with chronic rhinosinusitis with nasal polyps (CRSwNP). However, the precise influence of NO on CRSwNP recurrence remains unclear. Objective This study seeks to elucidate the relationship between NO levels and the risk of CRSwNP recurrence. Methods A protein chip array analysis was conducted to identify differentially expressed inflammatory mediators in the nasal tissues between patients with CRSwNP and healthy controls (HC). Differentially expressed proteins were analyzed, and bioinformatics analysis was used to predict the potential functions and pathways of these proteins. Western blotting (WB) and immunohistochemistry were employed to validate the candidate proteins in 2 independent cohorts. Receiver-operating characteristic (ROC) curves were employed to assess the abilities of target proteins for predicting the postoperative recurrence of CRSwNP. Results Twelve differentially expressed proteins were identified between the CRSwNP and HC groups. Notably, differentially expressed proteins exhibited high expression of the biological process term “positive regulation of nitric oxide-mediated signal transduction” ( P < .05). WB and immunohistochemistry results demonstrated that guanylate cyclase 1 soluble subunit alpha 1 (GUCY1A1), GUCY1A2, nitric oxide synthase 1 adaptor protein, epidermal growth factor receptor, and insulin were found to be upregulated in the CRSwNP group compared to the HC group ( P < .05). Moreover, elevated levels of GUCY1A2 and GUCY1A1 were observed to be associated with an increased risk of CRSwNP recurrence ( P < .05), and ROC curve analysis confirmed their effectiveness as predictors for postoperative recurrence ( P < .05). Conclusion Our findings revealed that CRSwNP exhibited a distinct tissue protein profile, with soluble guanylate cyclase dysfunction and the nitric oxide pathway implicated in the underlying pathological mechanisms. The discovery-validation results suggested that GUCY1A1 and GUCY1A2 were promising predictors for postoperative recurrence in patients with CRSwNP.
