Introduction

A chronic communicable infection which is a principal or polar form of LEPROSY. This disorder is caused by MYCOBACTERIUM LEPRAE and produces diffuse granulomatous skin lesions in the form of nodules, macules, or papules. The peripheral nerves are involved symmetrically and neural sequelae occur in the advanced stage.

Mycobacterium leprae infection with rash misdiagnosed as a flare-up of systemic lupus erythematosus: a case report

BACKGROUNDLeprosy (Hansen's disease) is a curable, chronic contact infectious disease caused by Mycobacterium leprae. It mainly affects human skin and nerves and can cause progressive and permanent damage to the skin, nerves, limbs, eyes and is of great concern to the medical community.CASE PRESENTATIONA 35-year-old Han Chinese female patient presented to our hospital with 11 years of recurrent erythema and pain in the limbs and face, which was aggravated by fever for 6 days. The patient had an 11-year history of systemic lupus erythematosus (SLE). Initially, lupus was considered to be active and was subsequently treated with methylprednisolone; however, the rash was not controlled. By completing pathohistological examination, immunohistochemistry, and antacid staining, according to the Ridley-Jopling classification of leprosy, confirmed a diagnosis of lepromatous leprosy.CONCLUSIONSThis report will help increase awareness of leprosy in patients with immune disorders to reduce the rate of misdiagnosis and mistreatment.

Perforating ENL: A variant of type 2 lepra reaction

Lepra reactions are acute episodic inflammatory reactions that occur during illness due to abrupt changes in the body's immunological response against Mycobacterium leprae. These are of two types, type 1 and type 2. Type 2 reaction is also called Erythema Nodosum Leprosum (ENL). The common skin lesion in ENL is a red, tender, raised lesion. Subcutaneous nodules can also appear. The skin lesions may present as vesicles, pustules, or bulla. They can also develop ulcers and become necrotic. Some patients may experience skin lesions that resemble those seen in erythema multiforme. A rare desquamative rash may occur, which needs to be differentiated from a drug-related rash. Healing lesions may leave scars that may become inflamed during ENL flare-ups. In rare instances, there may be no skin lesions suggesting ENL. Reactive perforating type of erythema nodosum leprosum (ENL) is a rare type 2 lepra reaction occurrence, observed in lepromatous leprosy and borderline lepromatous cases. It is linked with the involvement of several organs, so if diagnosis and treatment are delayed, it can result in complications and a grim prognosis.

ABSTRACT.This report documents the first known cases of lepromatous leprosy in patients with chronic mucocutaneous candidiasis (CMC) linked to a gain-of-function mutation in the STAT1 gene. Two related patients, a mother and daughter, who both suffer from CMC and lepromatous leprosy, carry a heterozygous STAT1 mutation (c.821G>A; p.R274Q). Both individuals exhibited similarly high levels of total and phosphorylated STAT1 in CD4+ T cells and decreased interleukin 17A transcripts. This mutation underscores a complex relationship between genetic susceptibility to infections and the necessity to evaluate each case individually.
