An Artificial Intelligence Model for Accurate Diagnosis of Renal Tumors Based on Multi-phase Contrast-enhanced CT and Laboratory Tests: A Model Development and Multi-center Evaluation Study

This multi-center retrospective study aims to develop a multimodal artificial intelligence diagnostic model using preoperative contrast-enhanced CT images and routine laboratory parameters from patients with renal tumors. Primary Renal Neuroendocrine Tumor Presenting as Ectopic ACTH Syndrome

AbstractEctopic adrenocorticotropin syndrome (EAS) is rarely caused by genitourinary neuroendocrine tumors (NETs). We describe an unusual case of renal NET presenting with severe EAS. A 32-year-old woman had presented to endocrine clinic with a 2-month history of proximal muscle weakness, skin hyperpigmentation, amenorrhea, and weight gain. She was hypertensive (160/90 mm Hg) with facial puffiness, hirsutism, and obesity. Investigations suggested high 8 Am cortisol, 24-hour urine free cortisol, and high adrenocorticotropin level. She required inpatient admission because of worsening symptoms, and work-up revealed severe hypokalemia and hyperglycemia requiring intravenous (IV) potassium chloride and IV insulin. Radiology revealed normal sella on magnetic resonance imaging and a mass in the upper pole of the right kidney along with bilateral adrenal enlargement on computed tomography. The patient underwent surgical resection of the right renal mass. The initial histopathology revealed renal oncocytoma, with no evidence of renal cell carcinoma, which was revisited with additional immunochemistry. Final histopathology confirmed primary renal well-differentiated NET. The abruptness and severity of symptoms favored aggressive neoplasm but histopathology confirmed an extremely rare example of primary renal NET and ruled out malignancy. This case highlights the presentation of severe EAS in a patient with extremely rare renal NET.

Management strategies and oncological follow-up of a bicentric French cohort of biopsied renal oncocytomas

OBJECTIVERenal oncocytoma is a benign renal tumour whose management remains heterogeneous and poorly codified. The aim of this study was to describe the management strategies and evolution of a cohort of biopsied renal oncocytomas.MATERIALS AND METHODSWe retrospectively reviewed renal oncocytomas biopsied in 2 French academic centres between 2009 and 2016. Demographic, anatomopathological, radiological and operative (if operated) variables were collected and compared between patients operated on and those monitored in order to identify those associated with the management strategy and clinical course.RESULTSFifty-three patients were included with a mean follow-up of 29.6±32.3 months. Patients operated on immediately were younger (mean 57±11.5 years vs 68±9.8 years; P=0.007), less comorbid (Charlson 2.8 vs 4; P=0.01) with a higher RENAL score (9 vs 6; P<0.001) and a larger tumour size (mean 58±27 vs 27±13mm; P=0.002). Of the 28 patients monitored, 23 (82%) were still being monitored at the end of the study. The median duration of monitoring was 17.56 months. For the cohort as a whole, the mean tumour growth rate was 2.6mm/year (±2.5).CONCLUSIONThe management of oncytomas was heterogeneous. Tumour size, patient age and his comorbidities, RENAL score and tumour growth rate were the criteria associated with surgical treatment.LEVEL OF EVIDENCE: 4

What Is New in Pathologic Diagnosis and Classification of the Common Renal Cell Neoplasms?

Diagnostic challenges remain among the common renal cell carcinoma (RCC) subtypes. High-grade clear cell RCC may have deceptive patterns, for example BAP1-deficient tumors. Subtyping type 1 and 2 papillary RCC is no longer recommended, as former type 2 tumors may now be contain other diagnostic entities, such as FH-deficient RCC, MITF family RCC, or others. Clear cell papillary tumor is no longer considered carcinoma due to its highly favorable behavior. However, imperfect examples are best considered clear cell RCC. Oncocytic renal neoplasm of low malignant potential has been proposed as a borderline category in the absence of overt malignant features.
