Introduction

An erythematous eruption commonly associated with drug reactions or infection and characterized by inflammatory nodules that are usually tender, multiple, and bilateral. These nodules are located predominantly on the shins with less common occurrence on the thighs and forearms. They undergo characteristic color changes ending in temporary bruise-like areas. This condition usually subsides in 3-6 weeks without scarring or atrophy.

主要研究目的健康受试者空腹或餐后状态下，单次口服山东淄博新达制药有限公司生产的受试制剂布洛芬颗粒（规格：0.2g）或Kaken Pharmaceutical Co., Ltd.生产的参比制剂布洛芬颗粒（规格：20%1g，商品名：Brufen ®），分别考察空腹或餐后状态下受试制剂与参比制剂在健康受试者体内的药代动力学参数，评价两制剂的生物等效性。次要研究目的考察受试制剂和参比制剂在健康受试者中的安全性。

Main study objectives Healthy subjects were given a single oral dose of the test preparation ibuprofen granules (specification: 0.2g) produced by Shandong Zibo Xinda Pharmaceutical Co., Ltd. or the reference preparation ibuprofen granules (specification: 20%1g, trade name: Brufen ®) produced by Kaken Pharmaceutical Co., Ltd. in the fasting or postprandial state, respectively, to investigate the pharmacokinetic parameters of the test preparation and the reference preparation in the healthy subjects in the fasting or postprandial state, and to evaluate the bioequivalence of the two preparations. Secondary study objectives Investigate the safety of the test preparation and the reference preparation in healthy subjects.

The ibuprofen granules (specification: 200 mg/g) developed by Jilin Kaiman Pharmaceutical Co., Ltd. were used as the test preparation, and the ibuprofen granules (trade name: Brufen®, specification: 200 mg/g) of the licensee Kaken Pharmaceutical Co., Ltd were used as the reference preparation. The pharmacokinetic parameters and relative bioavailability of the two preparations after single administration in the fasting and postprandial states were investigated to evaluate whether the two preparations were bioequivalent. At the same time, the safety and tolerability of the two preparations in healthy humans were evaluated.

Methotrexate and Prednisolone Study in Erythema Nodosum Leprosum (MaPS in ENL

Erythema Nodosum Leprosum (ENL) is a painful, debilitating complication of leprosy. Patients often require high doses of corticosteroids for prolonged periods. Thalidomide is expensive and not available in most countries. The use of corticosteroids for long periods is associated with adverse effects and mortality. It is a priority to identify alternative agents to treat ENL. Methotrexate (MTX) is a cheap, widely used medication which has been reported to be effective in ENL resistant to steroids and thalidomide.

Mycobacterium tuberculosis in hepatic hilum as a cause of erythema induratum of Bazin

Erythema induratum of Bazin (EIB) is a rare manifestation of cutaneous tuberculosis, typically associated with active tuberculosis infections. We present the case of a 75-year-old immunocompetent Spanish woman who developed nodular lesions on her lower limbs. Initial differential diagnoses included sporotrichosis, erythema nodosum, Sweet's syndrome, sarcoidosis, and tuberculosis. The cutaneous and hepatic hilum biopsy revealed necrotizing granulomatous lymphadenitis but only the last one tested positive for M. tuberculosis; no evidence of the pathogen was detected in the other clinical samples. The patient was diagnosed with EIB associated with tuberculosis in the hepatic hilum and successfully treated with a standard anti-tuberculosis regimen. This case underscores the importance of considering hepatic involvement in extrapulmonary tuberculosis and the diagnostic challenges it may pose in immunocompetent patients.

Perforating ENL: A variant of type 2 lepra reaction

Lepra reactions are acute episodic inflammatory reactions that occur during illness due to abrupt changes in the body's immunological response against Mycobacterium leprae. These are of two types, type 1 and type 2. Type 2 reaction is also called Erythema Nodosum Leprosum (ENL). The common skin lesion in ENL is a red, tender, raised lesion. Subcutaneous nodules can also appear. The skin lesions may present as vesicles, pustules, or bulla. They can also develop ulcers and become necrotic. Some patients may experience skin lesions that resemble those seen in erythema multiforme. A rare desquamative rash may occur, which needs to be differentiated from a drug-related rash. Healing lesions may leave scars that may become inflamed during ENL flare-ups. In rare instances, there may be no skin lesions suggesting ENL. Reactive perforating type of erythema nodosum leprosum (ENL) is a rare type 2 lepra reaction occurrence, observed in lepromatous leprosy and borderline lepromatous cases. It is linked with the involvement of several organs, so if diagnosis and treatment are delayed, it can result in complications and a grim prognosis.

Löfgren syndrome, characteristics of Japanese cases: A case and a review of the literature

ABSTRACTLöfgren syndrome (LS) is a sarcoidosis subtype characterised by an acute disease course, bilateral hilar lymphadenopathy (BHL), erythema nodosum (EN), and ankle arthritis. LS in Caucasians appears to be self-limiting; however, our patients require glucocorticoid (GC) treatment. Here, we present a case of LS and review the literature to identify the characteristics of Japanese patients with LS. A 66-year-old woman was referred to Kobe City Medical Center General Hospital; she initially presented with an acute onset of low-grade fever and ankle arthritis, followed by EN. Skin biopsy revealed a noncaseating granuloma, and a chest computed tomography scan displayed BHL; she was diagnosed with LS. Her arthralgia ameliorated spontaneously, but the erythema persisted, necessitating GC treatment. A literature review revealed that the Japanese LS patients showed more fever, were more frequently treated with GCs, and more patients seemed to relapse, which may be explained by the absence of human leucocyte antigen–DR isotype 3, a good prognostic allele in Caucasians. Japanese LS may cause severe symptoms after development because of the differences in human leucocyte antigens from foreign countries. For early diagnosis, it is important to evaluate EN and BHL in patients with polyarthritis involving ankle arthralgia.
