In 2024, Merck and Moderna also initiated a two-part Phase 2/3 randomized clinical trial evaluating mRNA-4157 (V940) in combination with KEYTRUDA as neoadjuvant and adjuvant treatment in patients with resectable locally advanced Stage II-IV (M0) cutaneous squamous cell carcinoma (INTerpath-007, NCT06295809 ), a Phase 2 randomized clinical trial evaluating mRNA-4157 (V940) in combination with KEYTRUDA as adjuvant treatment in patients with intermediate-high-risk, high-risk, or M1 no evidence of disease renal cell carcinoma (INTerpath-004, NCT06307431 ), and a Phase 2 randomized clinical trial evaluating mRNA-4157 (V940) in combination with KEYTRUDA as adjuvant treatment in patients with high-risk muscle-invasive urothelial carcinoma post-radical resection (INTerpath-005, NCT06305767 ). About mRNA-4157 (V940) mRNA-4157 (V940) is a novel investigational messenger RNA (mRNA)-based individualized neoantigen therapy (INT) consisting of a synthetic mRNA coding for up to 34 neoantigens that is designed and produced based on the unique mutational signature of the DNA sequence of the patient’s tumor. Upon administration into the body, the algorithmically derived and RNA-encoded neoantigen sequences are endogenously translated and undergo natural cellular antigen processing and presentation, a key step in adaptive immunity. Individualized neoantigen therapies are designed to train and activate an antitumor immune response by generating specific T-cell responses based on the unique mutational signature of a patient’s tumor. KEYTRUDA is an immunotherapy that works by increasing the ability of the body’s immune system to help detect and fight tumor cells. As previously announced from the Phase 2b KEYNOTE-942/mRNA-4157-P201 trial evaluating patients with high-risk stage III/IV melanoma, combining mRNA-4157 (V940) with KEYTRUDA may provide a meaningful benefit over KEYTRUDA alone. About INTerpath-009 ( NCT06623422 ) INTerpath-009 is a global, randomized, double-blind Phase 3 trial evaluating 680 patients with resected (R0 or R1) Stage II, IIIA, IIIB (N2) NSCLC who did not achieve a pCR after neoadjuvant KEYTRUDA plus platinum-based chemotherapy. Following surgical resection, participants 18 years and older will be randomized 1:1 to receive either V940 (mRNA-4157) (1 mg every three weeks for up to nine doses) and KEYTRUDA (400 mg every six weeks up to seven cycles) or placebo (1 mg every three weeks for up to nine doses) and KEYTRUDA (400 mg every six weeks up to seven cycles). The primary endpoint is disease-free survival (DFS), defined as the time from randomization to any recurrence (local, locoregional, regional, or distant), occurrence of new primary NSCLC, as assessed by the investigator, or death due to any cause, whichever occurs first. The secondary endpoints are overall survival (OS), distant metastasis-free survival (DMFS), DFS2, lung cancer-specific survival (LCSS), safety, and quality of life. Bristol Myers Squibb and Ideaya Biosciences both released detailed snapshots of early-stage data for cancer drugs targeting solid tumors with a certain mutation known as MTAP deletion. There are no approved therapies specifically for MTAP deletions, though these tumors can be found in a wide range of cancer types, such as urothelial, lung, pancreatic and others. Either drug would allow patients an additional option if they end up resistant to other therapies like checkpoint inhibitors. While the companies are separately working to treat the same kinds of cancer mutations, their drugs act differently. Bristol Myers’ program, called BMS-986504, is a next-generation PRMT5 inhibitor that affects the binding of PRMT5 and MTA (the “PRMT5-MTA complex”) in an attempt to avoid off-target toxicity from earlier-in-class drugs. Ideaya’s therapy, codenamed IDE397, is a MAT2A inhibitor and is designed on the concept of synthetic lethality, where tumors with certain genetic aberrations become dependent on another cellular pathway. By blocking MAT2A, IDE397 can effectively treat patients with MTAP-deletion solid tumors, Ideaya says. Bristol Myers said Friday that, as of May 15, it enrolled 125 patients, 107 of whom were evaluable for efficacy. Of those patients, 21 achieved a partial response, good for an ORR of 20%. There were no complete responses, and the median duration of response was 4.8 months, with a median follow-up of 5.8 months. BMS measured seven different dose levels, but did not break down efficacy by each specific dose. The highest ORR came in mesothelioma, with three of seven patients achieving a partial response for 43%. Median duration of response was not reached. In NSCLC, six of 20 patients reached a partial response (30% ORR) with a median duration of response of 6.8 months. Pancreatic ductal adenocarcinoma had the lowest response rate, with three of 30 patients achieving partial response (10%). Treatment-related side effects were observed in 74% of the 125-patient sample, including 12% who saw grade 3 or higher adverse events. The most common side effects were nausea, vomiting, fatigue, diarrhea and decreased appetite. Additionally, 2% of patients discontinued treatment due to side effects. Ideaya’s data build on a July readout from an 18-patient sample. In the new dataset, there are 27 patients evaluable for efficacy as of an Aug. 22 cutoff. Nine of the 27 patients responded to the therapy, including one complete response and eight partial responses (33% ORR, down from 39% in July). Enrolled patients had either urothelial cancer, squamous NSCLC or adenocarcinoma NSCLC with MTAP deletions. The median duration of response is still immature, Ideaya said, as is median progression-free survival. The median duration of treatment has also not been reached. Treatment-related side effects were observed in 54% of patients, including 18% who saw grade 3 or higher adverse events (though none of those observed in more than 5% of patients). The most common side effects were peripheral neuropathy (none at grade 3 or higher), decreased appetite, blood creatinine increase, nausea, fatigue and asthenia. In addition to the monotherapy, Ideaya is also testing IDE397 with BMS-986504 and an experimental PRMT5 inhibitor from Amgen.

ENA 2024 Clinical Data Update – IDE397 Phase 2 Expansion in Subjects with MTAP-Deletion UC and NSCLC The company observed encouraging clinical activity at the 30 mg once-a-day (QD) Recommended Phase 2 Dose (RP2D) in its Phase 1 clinical trial evaluating its potential first-in-class MAT2A inhibitor IDE397 in heavily pre-treated MTAP-deletion UC and NSCLC patients. The patients evaluated had a median of two (2) to three (3) prior lines-of-therapy, ranging from one (1) to seven (7). The reported Phase 1 clinical expansion data are based on twenty-seven (27) evaluable MTAP-deletion patients, including ten (10) UC, nine (9) adenocarcinoma NSCLC, and eight (8) squamous (sq) NSCLC patients at the expansion dose of 30 mg QD of IDE397. The clinical efficacy and tolerability data are preliminary and based on investigator review from an unlocked database as of the data analysis cutoff date of August 22, 2024. The clinical data update in the twenty-seven (27) evaluable patients by RECIST 1.1 include: ~33% Overall Response Rate (ORR). One (1) complete response (CR) and eight (8) partial responses (PRs) by RECIST 1.1 evaluation out of twenty-seven (27) evaluable patients. Nine (9) of nine (9) responses have been confirmed by RECIST 1.1, including four (4) UC patients, of which one was a CR, three (3) squamous NSCLC patients, and two (2) adenocarcinoma NSCLC patients. Two patients confirmed after the data cutoff date. In the earlier reported July 8, 2024, IDE397 webcast program update, five (5) confirmed responses were reported out of eighteen (18) evaluable patients. There were zero (0) non-evaluable patients reported as of the data analysis. One (1) CR, eight (8) PRs, and sixteen (16) stable disease (SD) by RECIST 1.1 evaluation out of twenty-seven (27) evaluable patients Preliminary durability assessment: Fifteen (15) of twenty-seven (27) patients still on treatment. Seven (7) of nine (9) RECIST 1.1 responses remain on treatment. Median duration of treatment (DOT) has not been reached and is greater than 6.2 months and median time to response (TTR) is ~2.7 months. The median duration of response and median progression free survival data is still immature. Three (3) UC patients on treatment greater than 250 days, four (4) squamous NSCLC patients on treatment greater than 200 days, and three (3) adenocarcinoma NSCLC patients on treatment greater than 200 days ~81% ctDNA Molecular Response Rate (MRR). Seventeen (17) of twenty (21) patients with 50% or greater ctDNA reduction, and ~33% (7 of 21) with deep 90% or greater ctDNA reduction. All MRs (17 of 17) were rapid occurring at the first ctDNA sample analysis. There were several quality control failures of patient samples that led to unavailability for MR analysis Favorable adverse event (AE) profile. Approximately 18% grade 3 or higher drug-related AEs and no drug-related serious adverse events (SAEs) observed at the IDE397 30mg once-a-day expansion dose. No drug-related AEs leading to discontinuations were observed.
