FRIDAY, Jan. 12, 2024 -- For patients undergoing surveillance following diagnosis and treatment of ductal carcinoma in situ (DCIS), very few second breast cancers are detected by clinical breast examination by a physician, according to a study published online Dec. 28 in the Journal of the National Comprehensive Cancer Network . Bethany T. Waites, M.D., from the Kaiser Permanente San Francisco Medical Center, and colleagues performed a retrospective cohort study of patients with DCIS treated between Jan. 1, 2008, and Dec. 31, 2011, to assess the modes of detection of second cancers. The study cohort included 1,550 women (median age at diagnosis, 59 years), who were followed for a median of 10 years. During follow-up, 179 women (11.5 percent) were diagnosed with a second breast cancer: 43.0, 54.8, and 2.2 percent were ipsilateral, contralateral, and presented with distant metastases, respectively. Of the second cancers, 61.5, 36.3, and 2.2 percent were invasive, DCIS, and Paget disease, respectively. The researchers found that second breast cancers were imaging-detected, patient-detected, physician-detected, and detected incidentally on imaging or pathology from procedures unrelated to oncologic care in 74.3, 20.1, 2.2, and 3.4 percent of cases, respectively. "Our findings highlight the importance of mammogram screening and patient education regarding self-detection, and can inform future National Comprehensive Cancer Network recommendations for DCIS survivorship care," the authors write. Teriparatide injection can lessen the chance of broken bones (fractures) in the spine and other bones in postmenopausal women with osteoporosis. increase the bone mass in men with primary or hypogonadal osteoporosis who are at high risk for having broken bones (fractures) or who cannot use other osteoporosis treatments. treat both men and women with osteoporosis due to use of glucocorticoid medicines, such as prednisone, for several months, who are at high risk for having broken bones (fractures) or who cannot use other osteoporosis treatments. IMPORTANT SAFETY INFORMATION What is the most important information I should know about teriparatide injection? Possible bone cancer. During drug testing, the medicine in Teriparatide injection caused some rats to develop a bone cancer called osteosarcoma. Studies in people have not shown that Teriparatide injection increases your chance of getting osteosarcoma. There is little information about the chance of getting osteosarcoma in patients using Teriparatide injection beyond 2 years. Who should not use Teriparatide injection? Do not use Teriparatide injection if you: are allergic to any of the ingredients in teriparatide injection. Symptoms of a serious allergic reaction of Teriparatide injection may include swelling of the face, lips, tongue or throat that may cause difficulty in breathing or swallowing. Call your healthcare provider right away or get emergency medical help if you get any of these symptoms. What should I tell my healthcare provider before using teriparatide injection? Before you use Teriparatide injection, tell your healthcare provider about all of your medical conditions, including if you: have a certain bone disease called Paget’s disease or other bone disease. have bone cancer or have had a history of bone cancer. are a young adult whose bones are still growing. have had radiation therapy. are affected with a condition that runs in your family that can increase your chance of getting cancer in your bones. have or have had too much calcium in your blood (hypercalcemia). have or have had a skin condition with painful sores or wounds caused by too much calcium. have or have had kidney stones. take medicines that contain digoxin. are pregnant or plan to become pregnant. It is not known if Teriparatide injection will harm your unborn baby. are breastfeeding or plan to breastfeed. It is not known if Teriparatide passes into your breastmilk. Increased calcium in your blood. Tell your healthcare provider if you have nausea, vomiting, constipation, low energy, or muscle weakness. These may be signs there is too much calcium in your blood. Worsening of your kidney stones. If you have or have had kidney stones your healthcare provider may check the calcium levels in your urine while you use Teriparatide injection to see if there is worsening of this condition. Decrease in blood pressure when you change positions. Some people may feel dizzy, get a fast heartbeat, or feel light-headed right after the first few doses of Teriparatide injection. This usually happens within 4 hours of taking Teriparatide injection and goes away within a few hours. Teeth could be capable of preserving antibodies for hundreds of years, allowing scientists to investigate the history of infectious human diseases, a new study has found. Antibodies are proteins produced by the immune system as a natural response to infectious organisms like viruses and bacteria. Their job is to recognise those microbes so that the immune system can attack them and clear them from the body. In the new paper, published by iScience, antibodies extracted from 800 year-old medieval human teeth were found to be stable and still able to recognise viral proteins. The study, led by Professor Robert Layfield and research technician Barry Shaw from the School of Life Sciences, University of Nottingham, in collaboration with Professor Anisur Rahman and Dr Thomas McDonnell from the Department of Medicine at University College London, expands the study of ancient proteins, referred to as palaeoproteomics, potentially allowing experts to analyse how human antibody responses developed through history. Palaeoproteomics can reach back into deep time with ancient proteins already successfully recovered and identified after preservation in 1.7-million year old dental enamel from an ancient rhinoceros and an ostrich eggshell more than 6.5 million years old. In this new study, the authors also found preliminary evidence that, like the medieval human teeth, mammoth bones nearly 40,000 years old appear to preserve stable antibodies. This science has previously been applied by the Nottingham team to the analysis of other disease-associated proteins recovered from archaeological human bones and teeth to allow identification of an unusual ancient form of the skeletal disorder Paget's disease. Professor Layfield explained:"In discovery science we come to expect the unexpected, but the realisation that intact, functional antibodies can be purified from skeletal remains in the archaeological record was quite astonishing. Some ancient proteins were known to be stable, but these tend to be 'structural' proteins such as collagens and keratins, that are pretty inert." Professor Rahman added: "Antibodies are different because we are able to test whether they can still do their job of recognising viruses or bacteria even after hundreds of years. In this case we found that antibodies from medieval teeth were able to recognise Epstein-Barr virus, which causes glandular fever. In future it could be possible to look at how antibodies from ancient specimens react to diseases present during those periods, such as the Black Death."
