Evaluation of the Efficacy, Tolerance of the Treatment, and Patient Satisfaction During the Management of an Arrested Pregnancy in the First Trimester According to the New Toulouse University Hospital Protocol

The clinical practice recommendations highlight the fact that the patient's choice concerning the management of her aborted pregnancy in the first trimester should be privileged, after clear and appropriate information on the available treatments, in order to improve his experience and reduce the negative psychological symptoms that can generate an alteration in the quality of life, in the aftermath of the loss of pregnancy.

Access to medical abortion and abortion care in Japan

Objectives: On April 26, 2023, the Japanese Ministry of Health, Labour and Welfare approved the use of the medical abortion pill package (called the MEFEEGO Pack in Japan) on the condition that the patient undergoes either "hospitalization or outpatient observation at a facility with beds." The purpose of this study is to examine the immediate uptake of medical abortion and the current situation of peri-abortion care in Japan. Methods: The study design was an institution-level national survey. Questionnaires were sent to 3941 facilities to collect information on abortions and abortion consultations performed between May and October 2023. Results: Responses were received from 2096 facilities (53.2 % response rate). A total of 28,346 abortions were performed before the ninth week of pregnancy, of which the MEFEEGO Pack was used in 435 cases (1.5 %), with no serious complications. The proportion of facilities providing peri-abortion care services in all abortion cases was 10.9 % for mental health support, 3.3 % for counseling information, 50.6 % for support to prevent repeat abortions, and 73 % for contraceptive information. These services were more commonly provided in hospitals with a higher number of obstetrician-gynecologists and abortions, and where interprofessional collaboration was present. Physicians were the main providers of peri-abortion care, while midwives and nurses provided these services at only about 20 % of facilities. Conclusion: Greater interprofessional collaboration and increased involvement of midwives and nurses are necessary to prepare for a possible increase in the use of medical abortion in Japan.

“It gives you autonomy over your own choices”: A qualitative study of Canadian abortion patients’ experiences with mifepristone and misoprostol

OBJECTIVES:

The gold standard of medication abortion, mifepristone and misoprostol, became available in Canada in 2017 as a combination pack under the trade name Mifegymiso®. We aimed to document people's experiences deciding to use and accessing medication abortion and generate insights for how information and services could be improved.

METHODS:

We conducted semi-structured, in-depth interviews with 64 people who had used the mifepristone/misoprostol regimen to induce an abortion in seven different Canadian provinces. We used ATLAS.ti to manage our data, which we analyzed for content and themes using inductive and deductive techniques.

RESULTS:

The overwhelming majority of participants reflected positively on their experience with mifepristone/misoprostol for early abortion. Most people reported that medication abortion offered increased autonomy, privacy, and convenience compared with instrumentation procedures and especially liked being able to have the abortion in a comfortable and familiar environment. The small number of participants who reflected more negatively on their experiences generally did not feel well informed about what to expect. Several participants reported that the media served as a key information source for finding and accessing services.

CONCLUSIONS:

Although our sample of Canadian abortion patients appear to be highly satisfied with the mifepristone and misoprostol regimen, there are several avenues by which medication abortion care could be improved. Providing more detailed information about pain management and the products of conception and lifting up the voices and stories of those who have used mifepristone and misoprostol could help patients manage expectations and would likely be welcomed.

IMPLICATIONS:

Changes to the Mifegymiso® product monograph since its introduction seem to have addressed a number of barriers described by early users of mifepristone and misoprostol. Continued engagement with clinicians to ensure that the process is not overly medicalized appears warranted.

Implementation of mifepristone medical abortion in Canada: pilot and feasibility testing of a survey to assess facilitators and barriers

Abstract:

Background:

Direct primary care provision of first-trimester medical abortion could potentially address inequitable abortion access in Canada. However, when Health Canada approved the combination medication Mifegymiso® (mifepristone 200 mg/misoprostol 800 mcg) for medical abortion in July 2015, we hypothesized that the restrictions to distribution, prescribing, and dispensing would impede the uptake of this evidence-based innovation in primary care. We developed and pilot-tested a survey related to policy and practice facilitators and barriers to assess successful initiation and ongoing clinical provision of medical abortion service by physicians undertaking mifepristone training. Additionally, we explored expert, stakeholder, and physician perceptions of the impact of facilitators and barriers on abortion services throughout Canada.

Methods:

In phase 1, we developed a survey using 2 theoretical frameworks: Greenhalgh’s conceptual model for the Diffusion of Innovations in health service organizations (which we operationalized) and Godin’s framework to assess the impact of professional development on the uptake of new practices operationalized in Légaré’s validated questionnaire. We finalized questions in phase 2 using the modified Delphi methodology. The survey was then tested by an expert panel of 25 nationally representative physician participants and 4 clinical content experts. Qualitative analysis of transcripts enriched and validated the content by identifying these potential barriers: physicians dispensing the medication, mandatory training to become a prescriber, burdens for patients, lack of remuneration for mifepristone provision, and services available in my community. To assess the usability and reliability of the online survey, in phase 3, we pilot-tested the survey for feasibility.

Results:

We developed and tested a 61-item Mifepristone Implementation Survey suitable to study the facilitators and barriers to implementation of mifepristone first-trimester medical abortion practice by physicians in Canada.

Conclusions:

Our team operationalized Greenhalgh’s theoretical framework for Diffusion of Innovations in health systems to explore factors influencing the implementation of first-trimester medical abortion provision. This process may be useful for those evaluating other health system innovations. Identification of facilitators and barriers to implementation of mifepristone practice in Canada and knowledge translation has the potential to inform regulatory and health system changes to support and scale up facilitators and mitigate barriers to equitable medical abortion provision.

Women’s Health Advocates Call on Province of New Brunswick and Premier Higgs to act urgently following the closure announcement of Clinic 554

OTTAWA, Feb. 02, 2024 (GLOBE NEWSWIRE) -- Following the announcement of the closure of Clinic 554 in Fredericton, the Society of Obstetricians and Gynaecologists of Canada (SOGC) calls on the Province of New Brunswick and Premier Higgs to take immediate measures to ensure that all women in New Brunswick have equitable access to surgical abortion. The closure of Fredericton-based Clinic 554 means that women will now only be able to access these services in two cities in the province. Only three hospitals in New Brunswick offer this service. A 2023 research study from the University of New Brunswick found that there are many barriers to accessing services, including general information gaps, payment models, and regional disparities. These long-standing concerns have been exacerbated by this recent announcement. “The health and safety of Canadian women, which includes equitable access to reproductive health care, should be a priority of all levels of government; it should not depend on a woman’s postal code. While Mifegymiso offers another choice, it may not be a suitable option for all individuals due to medical, geographic, or personal concerns. The closure of Clinic 554 limits women’s choice and access to an important medical service in that region of our country.
