Various median nerve anatomical variations have been reported in the literature.One of these is the bifid median nerve.Due to its relatively higher cross-sectional area, it can facilitate compression of the median nerve in the carpal tunnel.Therefore, bifid median nerve is relatively common in patients with CTS.Some studies have shown that the bifid median nerve has an effect on surgical failure in the treatment of carpal tunnel syndrome.Apart from surgical treatment, perineural injection with corticosteroids is a treatment method that has been used for a long time in the treatment of mild and moderate carpal tunnel syndrome.It is controversial whether bifid median nerve has a negative effect on treatment effectiveness.When looking at the literature, there is no previous study on this subject.With the introduction of ultrasound into our daily practice, the frequency of ultrasound-guided perineural injections has increased.Ultrasound allowed us to see variations in the median nerve before injection.The aim of the study is to compare the effectiveness of steroid injection in patients with CTS with bifid median nerve and patients with CTS with non-variant median nerve.It is also to investigate whether this anatomical variation has a negative effect on the treatment.

Comparison of Post-Inflammatory Pigment Alteration After Psoriasis Treatment (PIPA - Dermavant)

The purpose of this prospective study is to evaluate the degree of post-inflammatory pigmentation alteration on legs of study subjects treated with two different psoriasis treatments. Betamethasone dipropionate 0.05% cream is a high potency steroid that is commonly used to alleviate the inflammation of psoriasis. Tapinarof is another medication approved for psoriasis; tapinarof activates aryl hydrocarbon receptors to downregulate inflammatory cytokines (such as interleukin [IL]-17) which play a large role in psoriasis. Study subjects will be randomized to receive one of these medications and will be followed to monitor for PIPA. Photographs will be taken to help evaluate visual changes. The Taylor Hyperpigmentation scale will be used to grade the area and severity of hyperpigmentation and hypopigmentation; severity of erythema, burning, peeling, and dryness will also be recorded. This will allow us to understand the effects of two standard-of-care psoriasis medications to better treat patients.

Glassy Drug Microneedle Array Design: Drug Glass-Forming Ability and Stability

Glassy microneedles, composed only of drug, provide an intradermal alternative to oral or parenteral drug delivery. Compared to microneedles composed of drug-polymer solid dispersions, they offer higher drug loading while possessing mechanical strength for skin penetration. However, their microneedle structure and associated mechanical strength are reliant on the component glass stability. This study investigates relationships between the glass stability of drug-only microneedles and drug glass-forming ability (GFA), determined by differential scanning calorimetry (DSC) analysis. The glass stability of microneedles fabricated from six drugs was evaluated at 2-8 °C under nitrogen, 25 °C/60% relative humidity (RH), and 40 °C/75% RH. Drug glass stability was determined by visual assessment of microneedle appearance, together with DSC and powder X-ray diffraction analysis of the drug melt cooled outside the microneedle molds. Glassy microneedle structure was retained for all drugs stored at 2-8 °C under nitrogen for 3 months. Drug GFA classes informed glass stability under dry (nitrogen) environments at temperatures below their glass transition temperature. Under controlled humidity conditions, all glass microneedles crystallized, except for itraconazole. Drug GFA did not inform microneedle glass stability when exposed to water vapor during storage due to water absorption and glass plasticization. Itraconazole's glass stability was attributed to the interaction of absorbed water with liquid crystalline phases present in the itraconazole glass. The results highlight how glassy microneedle stability is informed by storage below T_g and glass interaction with moisture vapor. Results also demonstrate how the skin penetration efficiency of glassy microneedles is maintained during storage by selecting stabilizing storage conditions.

Chromatographic retention assisted in-silico prediction of anticancer potential of glucocorticoids on cancer cell lines

Glucocorticoids (GCs) are hallmarks of anti-inflammatory activity. They are used as adjuvant therapy in oncology medications to alleviate some of the associated side effects. Although recent research has indicated that GCs have favorable anticancer potential, some scientific evidence suggests a pro-proliferation impact of GCs on cancer cells. This may create a scientific confusion on the utility of GCs and the choice of which GC enhances the anticancer potential and mitigates the negative effect. Accurate in-silico prediction and correct ranking of biological activity may be limited by the impact of the physicochemical interaction between small molecules and biological membranes. Chromatographic retention is inherently dependent on the physicochemical properties of the test molecule. It can scale the relative hydrophobicity and tentatively indicate the membrane permeability. In this study, the relationship between the in-silico binding affinity, the chromatographic retention and the in-vitro anticancer activity was investigated. Fifteen GCs were chromatographically separated on an Inertsil® C18 (4.6 * 250 mm; 5 μm) HPLC column. The binding affinity of the test GCs was determined on three receptors involved in cancer cell proliferation (topoisomerase II (TOPII), glucocorticoid receptor (GR) and ATP-binding cassette (ABCG2)). The antiproliferative potential of the test steroids was determined as per their IC₅₀. The correlation between chromatographic retention and the binding affinity to the observed IC₅₀ was investigated by multiple linear regression (MLR). Results revealed that some GCs exhibited a remarkably favorable inhibitory potential against cancer cell lines over normal cell lines. Our data indicated a significant correlation between the retention times of different GCs and the determined binding affinity, especially to the GR (r² = 0.677; p = 0.011) and the estrone sulphate (ESS) binding site of the ABCG2 (r² = 0.643; p = 0.018). Concurrently, the retention times were well correlated to the observed IC₅₀ on the colorectal cancer cell lines (r² = 0.580; p = 0.038) and the hypopharyngeal cell lines (r² = 0.638; p = 0.019). Significant MLR models (n = 4) correlating the retention times of the tested steroids and the binding affinity to the observed IC₅₀ were created. The MLR model correlating the retention times and the ABCG2_ESS binding affinity to the IC₅₀ on lung cancer was the most significant (p = 0.004). The accuracy of the model was 107.12 ± 29.18 % indicating good IC₅₀ prediction abilities. In conclusion, chromatographic retention can be employed as a low-cost and simple auxiliary tool for improving the in-silico prediction of the in-vitro activities of small molecules.

Evaluation of psoriasis patients with long-term topical corticosteroids for their risk of developing adrenal insufficiency, Cushing’s syndrome and osteoporosis

PURPOSEIn this study, we will investigate the possible side effects of psoriasis patients using long-term topical corticosteroids (TCS) such as adrenal insufficiency, Cushing's Syndrome (CS) and osteoporosis and determine how these side effects develop.MATERIAL AND METHODSForty-nine patients were included in the study. The patients were divided into two groups based on the potency of the topical steroid they took and the patients' ACTH, cortisol and bone densitometer values were evaluated.RESULTSThere was no significant difference between the two groups regarding the development of surrenal insufficiency, CS and osteoporosis. One patient in group 1 and 4 patients in group 2 were evaluated as iatrogenic CS. ACTH stimulation tests of these patients in group 2 showed consistent results with adrenal insufficiency, while no adrenal insufficiency was detected in the patient in Group 1. Patients who used more than 50g of superpotent topical steroids per week compared to patients who used 50g of superpotent topical steroids per week. It was identified that patients who used more than 50g of superpotent topical steroids had significantly lower cortisol levels, with a negatively significant correlation between cortisol level and the amount of topical steroid use (p < .01).Osteoporosis was detected in 3 patients in group 1 and 8 patients in Group 2. Because of the low number of patients between two groups, statistical analysis could not be performed to determine the risk factors.CONCLUSIONSOur study is the first study that we know of that investigated these three side effects. We have shown that the development of CS, adrenal insufficiency and osteoporosis in patients who use topical steroids for a long time depends on the weekly TCS dosage and the risk increases when it exceeds the threshold of 50 grams per week. therefore, our recommendation would be to avoid long-term use of superpotent steroids and to choose from the medium-potent group if it is to be used.

LEO and Reig Jofre partner to improve access to psoriasis therapy in Europe

The commercial agreement will focus on strengthening the promotion of Enstilar, an aerosol spray foam containing the vitamin D analogue calcipotriol and betamethasone dipropionate, for mild-to-moderate psoriasis within the scope of primary care across Spain. Approximately 125 million people worldwide are affected by some form of psoriasis, a chronic inflammatory condition that typically affects the skin, nails and joints. Up to 90% of patients have plaque psoriasis, characterised by distinct round or oval plaques typically covered by silvery-white scales. LEO’s fixed-dose combination therapy is designed to improve anti-psoriatic response in patients and has been recognised by Farmaindustria, the national association of the pharmaceutical industry in Spain, as an “incremental innovation” due to its advanced foam-based formulation. The collaboration between LEO and Reig Jofre will help extend the treatment’s reach, improve patient access and enhance support for healthcare professionals. “This agreement marks a significant milestone in positioning Reig Jofre and LEO Pharma as key players in medical dermatology in Spain,” Reig Jofre outlined in a statement, adding that the partnership “ensures that healthcare professionals receive the support, proximity and information needed to provide the best possible care to their patients”. LEO has already entered into a number of partnerships this year. Last month, it announced a collaboration with non-profit organisation DEBRA Research aimed at accelerating the development of treatments for epidermolysis bullosa, a group of rare and painful inherited skin disorders that cause the skin to become very fragile and tear or blister very easily. Though financial details of the alliance were not disclosed, the partners said they are hoping to “enhance scouting capabilities and provide additional capacity to accelerate the development of life-changing therapies for epidermolysis bullosa”. The company also partnered with Gilead Sciences in January to advance its small molecule oral STAT6 programme for inflammatory diseases. The deal, worth $1.7bn, gave Gilead global rights to develop, manufacture and commercialise the oral STAT6 programme. Meanwhile, LEO will hold exclusive global rights to STAT6 topical formulations in dermatology, and will have the option to co-commercialise oral programmes for dermatology indications outside the US.

They treat allergic inflammation caused by indoor and seasonal allergens as well as insect stings. Severe allergic reactions are treated with oral corticosteroids, according to the Mayo Clinic. This type of treatment is commonly given as allergy shots (subcutaneous immunotherapy, or SCIT), but is also available in a form given under the tongue (sublingual immunotherapy, or SLIT). With SCIT, injections are given in your doctor’s office where they can monitor for adverse reactions. Treatment starts with the “build-up phase,” which is once or twice a week, and decreases to a maintenance dose which is once every two to four weeks. Sublingual immunotherapy treatment, like subcutaneous immunotherapy, also exposes you to allergens over a period of time, with the goal of building immunity to the allergen. SLIT tablets are available by prescription and taken daily. Immunotherapy is used to treat seasonal and indoor allergies, as well as insect stings. Side effects include watery eyes, sneezing, mild asthma symptoms, a rash at the injection site, tiredness and headaches.
