Priming of labor with tafoxiparin in nulliparous term pregnant women with unripe cervix—Proof‐of‐concept and dose‐finding results from a randomized controlled trial

Abstract:

Introduction:

New therapies are needed to improve the risk–benefit profile of intervention for labor induction. Tafoxiparin, a depolymerized form of heparin with no anticoagulant effects, has demonstrated potential to increase cervical ripening and influence the onset and outcome of labor. We assessed the effects of tafoxiparin in women with an unripe cervix scheduled for labor induction.

Material and Methods:

Phase 2 randomized double‐blind placebo‐controlled study (PPL17; EudraCT 2019‐000620‐17), conducted in two parts. The first part compared tafoxiparin 300 mg vs. placebo, while the second part assessed tafoxiparin 150 and 75 mg. Study treatment was administered once daily up to 7 days or until onset of labor or ripened cervix. Study participants were nulliparous women at term with an unripe cervix and planned labor induction. The primary endpoint was the cervical ripening rate during treatment, as measured by Bishop score.

Results:

Of 348 randomized women, 347 received study treatment. The primary analysis of Bishop score showed statistically significant increases in cervical ripening with tafoxiparin. The difference from placebo was most pronounced in the 300 mg group (mean slope 0.86 vs. 0.62, p = 0.006). Spontaneous onset of labor occurred in 39.8%–42.4% of women treated with tafoxiparin versus 28.4% with placebo. The rate of instrumental vaginal deliveries (vacuum extractions) was lowest in the tafoxiparin 300 mg group (10.0% vs. 24.1% with placebo; p = 0.013). Few serious adverse events were reported, and there was no hypercontractility related to tafoxiparin.

Conclusions:

Tafoxiparin was effective in cervical ripening, facilitating spontaneous onset of labor and reducing the need for instrumental delivery. Benefits were most pronounced with the 300 mg dose, and a favorable safety profile was observed. These results suggest that tafoxiparin may be suitable for obstetrical use in outpatient and home‐based settings.

Sorption-mediated organohalide respiration biokinetics of tetrachloroethene

Combining microbial bioaugmentation and activated carbon sorbent amendment is an emerging approach for in situ remediation of recalcitrant organohalide-contaminated sediments. However, how sorbent surfaces impact the organohalide respiration rate of the attached microorganisms needs to be better understood to predict the efficacy of the technology and design optimized engineering solutions. Here, we carried out microcosm studies to measure organohalide respiration rates of tetrachloroethene by different bioaugmented solids. We demonstrate that biofilms of the halorespiring bacterium Dehalobium chlorocoercia strain DF-1, attached to activated carbons, graphite, and silica, maintain comparable biokinetics to their planktonic counterparts. The established model, developed with a combination of biokinetics, sorption kinetics, and competitive sorption effect, adequately described the aqueous concentrations of chloroethenes when considering sorption mass transfer. The bioavailability of B-vitamins is reduced through sorption to some sorbents, which can limit DF-1 biokinetics, indicating the importance of ensuring a sufficient concentration of the nutrients in the presence of sorbents. Additionally, the sorption of chloroethenes by the two activated carbons did not reduce net molar transformation even though aqueous concentration was suppressed, indicating potential inter-dynamics of concentrated chloroethenes between sorption and microbial dechlorination. Our work illustrates the importance of accounting for sorption effects in models better to characterize the bioremediation effectiveness of organochlorides via bioaugmented sorbents.

INTRODUCTION:

Delayed onset and/or protracted labor progression is a relatively common obstetrical condition. The hypothesis that low molecular weight heparins may induce positive effects on uterine tissues in promoting and shortening delivery time was advanced in the last few years through several retrospective studies in pregnant women treated for thrombotic disorders. Tafoxiparin, a depolymerized form of heparin with a molecular structure created to eliminate the anticoagulant effects of heparins, is under investigation as a potential tool to promote labor activation and progression.

AREAS COVERED:

The authors conducted a critical review on current pharmacological and clinical evidence of tafoxiparin effects on labor.

EXPERT OPINION:

To date, available results may indicate potential benefits of using tafoxiparin for cervical ripening and labor augmentation, but several aspects need further clarifications and investigations. Clinical evidence is still weak to generalize the results of the trials and support the introduction of the drug into daily medical practice because few studies specifically focussing on this topic were published until now. In addition, many queries are left to be explored with future research, including optimal timing and doses, better route of administration, patient and physician acceptability, target pregnant populations, and cost-effectiveness aspects.

Karolinska Development AB (Nasdaq Stockholm: KDEV) announces today that the company, based on an external valuation, will increase the book value of its holding in the portfolio company Dilafor. The background is that Dilafor’s drug candidate tafoxiparin has shown a significantly positive impact on cervix ripening in a recently conducted phase 2b study in first-time mothers receiving treatment to induce labor. The external valuation, which has been risk-adjusted by Karolinska Development, will preliminarily have a positive effect of approximately SEK 450 million on the booked fair value of the holding in Dilafor that is indirectly owned via KDev Investments AB. About a quarter of all pregnant women are subject to labor induction. More than half of these inductions fail, which leads to protracted labor that entail an increased risk of complications for both mother and child. In a previous phase 2a study, subcutaneous administration of Dilafor’s drug candidate tafoxiparin has shown a significant positive effect with a shortened time to delivery and an enhanced ripening of the cervix in patients induced into labor. Dilafor has now enrolled the first subject in a phase 2b study to investigate in a larger group whether treatment with subcutaneously administered tafoxiparin can soften the cervix and improve the outcome of labor induction, and thereby shortening the time to delivery. The Phase 2b study is a multi-center, double blind, placebo-controlled proof of concept study in term-pregnant first-time mothers with an unripe cervix that are planned for labor induction. The pregnant women will be randomized to either subcutaneous injection of tafoxiparin or placebo once daily up to one week prior to scheduled labor induction. The treatment is then followed by induction according to clinical practice, which is usually balloon catheter or hormonal treatment. The target is to enroll 170 pregnant women in the study that is planned to be performed at delivery clinics in two countries in Europe.
