Atypical facial pain (AFP) is a chronic condition characterized by persistent facial pain without clear clinical signs, making diagnosis and treatment difficult. Common pharmacological treatments include antidepressants, anticonvulsants and neuromodulators, but their effectiveness remains uncertain, necessitating a systematic review to guide clinical practice.

MATERIALS AND METHODS:

Following PRISMA 2020 guidelines, randomized controlled trials (RCTs) evaluating pharmacological treatments for AFP in adults were included. A comprehensive search of five databases without date restrictions was performed. Data on pain reduction, quality of life, and adverse events were extracted. The Cochrane Risk of Bias (RoB 2.0) tool assessed bias, and evidence quality was evaluated using the Critical Appraisal Skills Programme tool.

RESULTS:

Out of 196 studies identified, 10 RCTs met the inclusion criteria. Pharmacological responses varied significantly across studies. Antidepressants such as dothiepin and clomipramine demonstrated significant pain reduction, whereas venlafaxine showed limited efficacy. However, onabotulinum toxin A and sumatriptan exhibited inconsistent or minimal effects on AFP pain intensity. Adverse events were reported across multiple treatments, ranging from mild side effects such as dry mouth and nausea to severe reactions like diplopia and facial asymmetry, which impacted adherence. Despite some positive outcomes, the heterogeneity in study methodologies, outcome measures, and follow-up durations limited direct comparisons between interventions.

CONCLUSION:

This systematic review highlights the mixed efficacy of pharmacological treatments for AFP, with certain medications demonstrating superior pain relief in specific patient subgroups. Given the variability in response and adverse events, a multimodal approach combining pharmacological and non-pharmacological therapies may offer the most effective management strategy. Future research should focus on standardized treatment protocols, long-term efficacy, and personalized treatment plans to optimize patient care.

Comparative Efficacy and Safety of SSRIs and TCAs in the Management of Major Depressive Disorder: A Prospective Observational Study

Background::

Psychiatric disorders form an important public health priority and major causes of morbidity. Major Depressive Disorder (MDD) is a severe psychiatric illness characterized by persistent sadness, loss of interest, lack of energy, disturbed sleep, and suicidality with lifetime prevalence rate for MDD is 16.2%. Treatment options for depression include medication, primarily antidepressants, psychotherapy which includes cognitive-behavioral therapy (CBT), interpersonal therapy (IPT), and electroconvulsive therapy.

Method::

A prospective observational study was conducted in psychiatric department of VIMSAR, Burla taking 50 patients diagnosed with MDD for a period of 24 months. Efficacy and safety between Selective Serotonin Reuptake Inhibitors (SSRIs) and Tricyclic Antidepressants (TCAs) were evaluated using Clinical Global Impression-Severity (CGI-S) scale, Clinical Global Impression-Improvement (CGI-I) scale, and Montgomery-Asberg Depression Rating Scale (MADRS) scores.

Results::

Dosulepin/Dothiepin (TCA) was most frequently prescribed antidepressant followed by escitalopram (SSRI). The efficacy of SSRIs is equivalent to that of TCAs. There was no significant difference among the antidepressants about reductions in the total scores on CGI-S, CGI-I and MADRS scales. Dry mouth and weight gain were the most frequently reported adverse drug reactions (ADRs).

Conclusion::

Antidepressants used for therapy of MDD in our study were effective in clinical improvement, better compliance on long term basis except for side effect profile.

Sustainability at scale: implementing a green solvent approach for Grignard reactions in the synthesis of key intermediates for Active Pharmaceutical Ingredients (APIs)

The operational feasibility of greener ethereal solvents viz., 2-Methyltetrahydrofuran (2-MeTHF) and Cyclopentyl Me ether (CPME) for Grignard reactions in the scale-up processes of ten API intermediates was investigated.Initially, the solubility and stability of four different Grignard reagents that are used in these processes were examined at various molar concentrations in greener solvents.Then, Grignard reactions were performed, while optimizing the reaction conditions in 2-MeTHF, CPME, and conventional solvents THF (THF), THF/toluene.Evaluation of the reaction performances directs that 2-MeTHF is a better alternative solvent for these API intermediates over CPME. 2-MeTHF was efficiently recycled from the reactions and reused in subsequent batches without compromising reaction performance, which significantly reduces environmental footprints.These investigations bolster the adoption of scalable, sustainable processes in the synthesis of ten API-key intermediates.
