Efficacy of BCG Revaccination Compared With Oral Chemoprophylaxis in Household Contacts Aged 6-18 Years for Prevention of Tuberculosis Disease - A Phase III Open Labelled Randomised Controlled Trial

Bacille Calmette Guerin (BCG) vaccine is one of the most used vaccines of the world, to reduce the risks of natural tuberculous infection. The efficacy of BCG vaccination in newborns is well known and has a documented protective effect against meningitis and disseminated TB in children. However, there is considerable uncertainty on BCG revaccination. It is known that BCG revaccination enhances immune responses, but it is yet to be established if BCG revaccination can help prevent TB disease in household contacts.
The primary aim of this study is to assess the efficacy of BCG revaccination compared to oral chemoprophylaxis in preventing TB disease in house hold contacts aged 6-18 years.
The study is designed as a multicentre randomised controlled trial with two groups of healthy household contacts aged 6-18 years receiving either the BCG vaccine or oral chemoprophylaxis. They will be followed up for 24 months to compare the incidence of TB disease in each arm.

Protective Role of Vitamin D Against Development of Active Tuberculosis in Close Household Contacts of Pulmonary Tuberculosis Patients: A Prospective Cohort Study

Vitamin-D is known to promote innate immune responses by acting as a cofactor of VDR for induction of antimicrobial peptides like cathelicidin. Close household contacts of pulmonary tuberculosis patients are at high risk of active infection, Therefore, possible role of vitamin-D in TB prevention through cathelicidin production was studied in high-risk household contacts (HHCs) of pulmonary tuberculosis (PTB) patients. 20 HHCs of PTB patients were recruited and followed up for one year. Levels of vitamin-D (25(OH)D) and its associated molecules were evaluated at 3-months intervals for one year or until the development of active TB. 25(OH)D was measured using chemiluminescence method. Serum VDR and cathelicidin levels were measured by ELISA and VDR mRNA expression by qPCR. Throughout the study period mean range of serum 25(OH)D levels was 20.51 ± 5.12 ng/ml. VDR and cathelicidin levels however showed significant decline after six months suggesting decrease in bacterial exposure. None of the HHCs developed active infection even with high exposure to 2 + to 3 + AFB positive index cases. Mantoux positive household contacts had high levels of VDR and cathelicidin, suggestive of an early or latent phase of infection, did not develop active TB plausibly due to maintenance of adequate serum levels of vitamin-D. Optimal levels of 25(OH)D and its associated molecules during early stages of infection may serve as protective factor against development of active TB. Cohort of HHCs with severely deficient vitamin-D levels (10 ng/ml) could be followed up for a better risk assessment.

Occupational and Environmental Exposure Influences the Inflammatory (Pro-and Anti-) Status in Benign Prostate Hyperplasia and Prostate Carcinoma Patients: A Retrospective Analysis

Multiple diseases and disorders are connected with occupational and environmental exposure risk. It is also well-established that chemicals and chemical mixtures have an influence on the immune cells of humans. This is an important field of research that has been pursued extensively in relation to autoimmune illnesses, allergy/asthma, and lung cancer, but Prostate Carcinoma has received rare reports. Chronic chemical exposure is known to produce inflammation, which is one of the most prominent characteristics of all malignancies. Changes in the ratio of pro-inflammatory to anti-inflammatory molecules are thought to be a key factor in the emergence of inflammation. Prostate gland cells express the pro-inflammatory cytokine interleukin-18 (IL-18), which is a major facilitator of immunological responses. Conversely, interleukin-10 (IL-10) is an anti-inflammatory cytokine that is linked to immune responses and inhibits the development of an inflammatory environment. Our goal is to investigate the inflammatory status of IL-18 (pro-) and IL-10 (anti-) in a variety of occupationally exposed populations in patients with Benign Prostate Hyperplasia (BPH) and patients with Prostate Carcinoma. The present study was conducted with 664 subjects, comprising 285 Prostate Carcinoma patients, 94 BPH patients and 285 controls. The subjects of BPH and Prostate Carcinoma were screened and confirmed on the basis of Prostate Serum Antigen (PSA) and pathological biopsy. All subjects were categorized as per their occupational exposure into various groups. The pro-inflammatory and anti-inflammatory Interleukins (IL-18 and IL-10) and serum PSA levels were analysed by using corresponding quantitative ELISA kits. The results showed that as compared to control participants, the serum PSA levels were higher in the Prostate Carcinoma and BPH groups. When mean levels of IL-18 were compared between various occupational groups, Tanners (tanning industry), Agriculture, and Ordnance workers had significantly higher levels (P < 0.05) of IL-18 than sedentary workers. The pro-inflammatory cytokine (IL-18) levels were also found to be aggravated in Prostate Carcinoma compared to BPH and controls. According to the findings of the current study, the levels of inflammatory cytokines (IL-18 and IL-10) in various occupational groups of BPH, Prostate Carcinoma, and controls were altered. Long-term occupational exposure may have a negative influence on inflammation levels and the immune system; therefore, preventative measures should be explored for improved health.

Circulating miRNA-21 Levels in Breast Cancer Patients Before and After Chemotherapy and Its Association with Clinical Improvement

Breast cancer is the most frequent type of cancer in women, many patients experience recurrences and metastasis. miR-21 (microRNA-21) as biomarker is under investigation for breast cancer. At present, there is very limited information available regarding effect of chemotherapy on miR-21 expression in breast cancer and its correlation with the clinical improvement. Hence, this study was planned to evaluate the effect of chemotherapy on miR-21 in metastatic breast cancer and its relationship with the clinical outcome. Females, aged-18-90 years diagnosed with Invasive Ductal Carcinoma of breast and candidate of neoadjuvant chemotherapy including Adriamycin (60 mg/m²), Cyclophosphamide (600 mg/m²) with or without Taxane (75-175 mg/m²) were included in the study. Before and after 42 days of staring of chemotherapy sample was collected for circulatory miR-21 and RECIST 1.1 criteria was applied to assess the clinical status. Blood samples for routine clinical biomarkers including liver function test and renal function tests was also collected. miR-21 expression before and after chemotherapy was assessed using standard method based on real time PCR. Expression of miR-21, RECIST criteria and other liver and kidney related biomarkers were compared before and after chemotherapy. After neoadjuvant chemotherapy expression of miR-21 was significantly increased by 5.65-fold. There was significant improvement in clinical scores based on RECIST criteria (0.046). No significant correlation was observed between miR-21 expression and difference in RECIST score (r = - 0.122, p = 0.570). Neoadjuvant chemotherapy causes clinical improvement in breast cancer patients however it is not correlated with the miR-21 expression which significantly increased after chemotherapy.

AIIMS Delhi unveils new oncology AI for early cancer detection more AI briefs from India

The All India Institute Of Medical Science in New Delhi recently unveiled its new AI solution that assists with the early detection of cancer. Developed with the Centre for Development of Advanced Computing, Pune under the Ministry of Electronics and Information Technology, the AI system runs on deep learning models capable of analysing complex medical data with "unprecedented accuracy and efficiency". It was trained using a data set of 500,000 radiological and histopathological images from 1,500 cases of breast and ovarian cancers, the two most common types of cancer. It has now been applied by the Department of Gynaecology to validate cases of breast and ovarian cancer. AIIMS has made the AI solution available to cancer hospitals and research centres for implementation, with deployment already made to five undisclosed district hospitals. The Neurology Department at Aster CMI Hospital has created an AI tool for screening Carpal Tunnel Syndrome (CTS). The AI diagnostic solution can identify the median nerve in ultrasound videos and detect CTS with 95% accuracy, the hospital claimed in a statement. Singapore-headquartered Qritive was recently signed by three diagnostic centres and hospitals in India to deliver AI-driven pathology management solutions. According to a press release, Metropolis Healthcare , Rajiv Gandhi Cancer Institute, and CŌRE Diagnostics will be adopting Qritive's Pantheon image management system and AI solutions as part of their respective pathology modernisation strategies. "The integration of Qritive’s AI-powered platform into our workflows signifies a revolutionary step in enhancing precision, timeliness, and personalised diagnosis," Metropolis Healthcare CEO Surendran Chemmenkottil commented. "This collaboration will strengthen our diagnostic precision, resulting in improved patient outcomes, and reaffirm our dedication to delivering excellence in clinical care. The collaboration also has the potential to lead to the development of new AI services for the diagnosis of rare diseases," said Dinesh Chauhan, CEO of CŌRE Diagnostics. "With efficient identification of concerning areas and precise grading, integrating these tools has enhanced our workflow and diagnostic precision, offering significant benefits to our practice," Dr Gurudutt Gupta, head of the Pathology Department at Rajiv Gandhi Cancer Institute Research Centre, also said about their adoption of Qritive's technology. Amrita Hospital in Faridabab announced that the head of its Radiation Oncology department has received a grant from the government to develop a robot-assisted ultrasound system for image-guided therapies and hyperthermia for cancer treatment. Explaining his project, which has received support from the Indian Council of Medical Research, Dr Bhaskar Viswanathan said, "It is a non-invasive technique that uses focused ultrasound waves and has a huge potential in cancer treatment like tumour ablation, mild hyperthermia for radiotherapy and chemotherapy, and motion management during radiation delivery." "This cutting-edge technology will allow precise localisation of lesions, enabling targeted and effective delivery of therapeutic radiation and avoiding damage to the surrounding tissue and organs," he added. Dr Viswanathan will also collaborate with the Indian Institute of Technology, Hyderabad and Eranki Labs for this three-year p roject. Ambulance service Medulance has launched what could be the first 5G-enabled medical facility offering for corporates. Launched in a corporate office in Gurgaon, city southwest of India's capital New Delhi, the Smart Medical Room features diagnostic tools, telemedicine capabilities, and real-time monitoring systems. Enabling rapid communication and data transfer, it reduces emergency response time to "under 4 minutes," the company claimed in a statement. "[W]e are not only treating basic parameters inside the medical room like CPR or Basic Life Support [but] we’re also going to provide teleconferencing with the doctor from this medical room and we will ensure that they can see the patient’s vitals remotely – this means blood pressure, sugar levels, and ECGs can be simultaneously shown to a doctor while the patient is on [their] way to a hospital," Medulance co-founder Pranav Bajaj further explained.

Seoul National University Hospital to lead South Korea's national strategic technologies R&D and more briefs

Seoul National University Hospital has been designated as a national strategic technology-specialised research institute by the South Korean Ministry of Health and Welfare. As such, it is expected to lead the country's effort to promote national strategic technologies as mandated by the National Strategic Technology Promotion Act, which was passed in February last year. It will conduct R&D and human resource training and be involved in international cooperation in the biotechnology field, focusing on digital health and health data analysis and utilisation. Indian drone tech Skye Air Mobility has bagged contracts for drone delivery projects in seven public medical institutions in India. Skye Air will deploy its drone to deliver life-saving medicines and blood in remote villages in a few minutes, as well as quickly transfer blood and pathological samples between hospitals. Tertiary hospital Kokilaben Dhirubhai Ambani Hospital (KDAH) in Mumbai has recently beefed up the cybersecurity of its IT infrastructure with solutions from Check Point amid the rise of "sophisticated" cyber threats in India. It has replaced its manual data aggregation process with Check Point's Maestro Hyperscale Network Security, an AI-powered centralised system to streamline workflows and optimise resources. Maestro has also provided visibility into open ports to enable independent remediations. KDAH also implemented the Check Point Harmony Endpoint to enhance endpoint security. It also plans to later introduce Check Point Email & Collaboration to strengthen its email security. "Since onboarding Check Point, we’ve had virtually no security issues," KDAH CIO Srinivasan Raman testified . Cardiology DTx company Lupin from India has partnered with the American College of Cardiology to offer what could be the world's first professional certificate on digital therapeutics. Based on a press release, ACC will grant the "digital pioneer" certificates to participating cardiologists who are using Lupin's remote cardiac rehabilitation and digital heart failure solutions. Merck has launched its first digital hub outside the United States and Europe. The Merck Digital Hub in Singapore, which was launched with the support of the Singapore Economic Development Board, is expected to promote digital innovation and talent across healthcare, semiconductor, and AI. It will be leveraging data collaboration ecosystems, Syntropy and Athinia, which help organisations integrate and curate their data while maintaining high-quality standards set by their sector.
