Downstaging of Hepatocellular Carcinoma with Macrovascular Invasion by Radiotherapy (SBRT or Y90) and Atezolizumab Plus Bevacizumab Followed by Liver Transplantation - a Prospective Trial

The goal of this clinical trial is to learn if locoregional therapy and immunotherapy can be used together to help patients with hepatocellular carcinoma (HCC) and macrovascular invasion achieve liver transplantation.
The main questions it aims to answer are:
* How many patients will achieve transplant with this treatment strategy?
* What will the 5-year survival and recurrence-free survival rates be for these patients?
Participants will:
* Undergo a biopsy of the tumor.
* Receive locoregional therapy (SBRT or Y90) followed by immunotherapy (atezolizumab and bevacizumab) 2 to 6 weeks later, for a maximum of 9 months.
* Be referred for a liver transplant and undergo the procedure if deemed eligible and safe.
* If applicable, be followed for five years post-transplant with regular data collection.

Implementation of dosimetry in molecularradiotherapy in Spain. Results from a Spanish survey

The implementation of the dosimetry and optimization requirements for radiopharmaceuticals of therapy, established in the Euratom Safety Standards Directive (EC Directive 2013/59/Euratom), is a major challenge in European countries.The aim of this study is to evaluate the current practice in mol. radiotherapy (MRT) in Spain.An electronic questionnaire has been distributed to Spanish Nuclear Medicine Departments (NMDs) in order to have an overview of the current situation with regard to the optimization of radiopharmaceutical therapies and the anal. of the resources available in hospitals.This survey received 24 replies, which represents 16 % of the total number of Spanish NMDs geog. distributed over 17 Spanish Autonomous communities.A Quality Assurance Program (QAP) has been developed in 96 % of Spanish NMDs that replied to the survey, and 86 % of these QAPs include the criteria for justifying medical exposure.Regarding staff knowledge of the provisions of Directive 2013/59/Euratom, 70 % of the responses were pos.A high level of compliance with the Summary of Product Characteristics for each therapy was observed in relation to the officially approved conditions of use of radiopharmaceutical therapies.Planning of the activity to administer with physician involvement is used only for non-standardized procedures (70 %).However, non-standardized procedures do not seem to be common practice.The use of activities based on fixed-activity protocols is more than 82 % for all therapies, except for the radiopharmaceutical I-131 MIBG (56 %).As regards the assessment of the resources available to carry out dosimetry-guided treatments, only 4 % considered them to be adequate.Patient-specific dosimetry has not yet been implemented in Spanish NMDs as a daily routine due to the lack of resources, training and specific regulatory instructions to perform it.

Analysis and prediction of the impact of Fukushima nuclear polluted water on the Chinese sea area

Discharge of Fukushima nuclear polluted water (NPW) into the ocean has aroused a new round of public concern. Based on the previous research findings and the data released by Japan, this work discusses transport of NPW in the North Pacific Ocean, the time required for NPW to reach the Chinese Sea area, variations in activity concentrations, and radiation effects on the marine organisms to predict the NPW's impact on the Chinese sea area. It reveals that the amount of NPW are transported to the Chinese sea area under the combined action of mode waters and mesoscale eddies with a transit period of 2-3 years. The corresponding amount is dominated by the strength of Kuroshio-Kuroshio Extension system. The increases of the ¹³⁷Cs, ⁹⁰Sr and ³H in the Chinese sea area will be indistinctive and covered in the fluctuations of their backgrounds. The other nuclides (⁶⁰Co, ⁹⁰Y, ⁹⁹Tc, ¹²⁵Sb, ^125mTe, ¹²⁹I) derived from NPW will be diluted to be negligible. Such low level of radioactivity is insufficient to cause‌ radiation effects ‌on‌ marine organisms. Given the limitations of this prediction, it is imperative to implement regular monitoring and validate the impacts through actual observational data.
