Clinical efficacy of simoctocog alfa versus extended half‐life recombinant FVIII concentrates in hemophilia A patients undergoing personalized prophylaxis using a matching‐adjusted indirect comparison method

Abstract:

Objectives:

We aimed to indirectly compare the efficacy of personalized prophylaxis with simoctocog alfa (Nuwiq®) versus three extended half‐life (EHL) recombinant FVIII (rFVIII) concentrates.

Methods:

Treatment effects were compared using matching‐adjusted indirect comparisons after matching individual patient‐level baseline characteristics for simoctocog alfa (pharmacokinetic [PK]‐guided personalized prophylaxis) against published aggregate personalized prophylaxis data for efmoroctocog alfa, damoctocog alfa pegol, and rurioctocog alfa pegol.

Results:

A higher percentage (p < .001) of patients with zero bleeds was found with simoctocog alfa compared with efmoroctocog alfa (75% vs. 45%), damoctocog alfa pegol (77% vs. 38%), and rurioctocog alfa pegol (target trough level 1%–3%; 78% vs. 42%). Similar efficacy was found comparing simoctocog alfa against rurioctocog alfa pegol 8%–12% (77% vs. 62%). The mean total annualized bleeding rate was lower (p < .001) with simoctocog alfa than damoctocog alfa pegol (1.5 vs. 4.9). Consistent with approved dosing, the mean FVIII weekly dose was higher (p < .001) for simoctocog alfa than efmoroctocog alfa, damoctocog alfa pegol, or rurioctocog alfa pegol 1%–3%, but lower (p < .001) than rurioctocog alfa pegol 8%–12%.

Conclusions:

Indirect comparisons demonstrated that PK‐guided, personalized prophylaxis with simoctocog alfa can lead to higher zero bleed rates compared with personalized EHL rFVIII concentrate regimens, albeit with higher weekly doses, and a lower percentage of patients treated twice weekly or less.

INTRODUCTION:

Simoctocog alfa (Nuwiq®) is a 4th generation recombinant FVIII with proven efficacy for the prevention and treatment of bleeding episodes (BEs) in previously treated patients with severe haemophilia A. The NuProtect study assessed the immunogenicity, efficacy and safety of simoctocog alfa in 108 previously untreated patients (PUPs). The incidence of high-titre inhibitors was 16.2% and no patients with non-null F8 mutations developed inhibitors.

AIM:

To report the efficacy and safety results from the NuProtect study.

METHODS:

PUPs received simoctocog alfa for prophylaxis, treatment of BEs, or as surgical prophylaxis. The efficacy of prophylaxis (during inhibitor-free periods) was assessed using annualised bleeding rates (ABRs). The efficacy in treating BEs and in surgical prophylaxis was assessed using a 4-point scale. Adverse events were recorded throughout the study.

RESULTS:

Of 108 PUPs treated with simoctocog alfa, 103 received at least one prophylactic dose and 50 received continuous prophylaxis for at least 24 weeks. In patients on continuous prophylaxis, the median ABR was 0 (mean 0.5) for spontaneous BEs and 2.5 (mean 3.6) for all BEs. In 85 patients who had BEs, efficacy of BE treatment was excellent or good for 92.9% (747/804) of rated BEs; 92.3% of BEs were treated with 1 or 2 infusions. The efficacy of surgical prophylaxis was excellent or good for 94.7% (18/19) of rated procedures. There were no safety concerns and no thromboembolic events.

CONCLUSION:

Simoctocog alfa was efficacious and well tolerated as prophylaxis, surgical prophylaxis and for the treatment of BEs in PUPs with severe haemophilia A.

BACKGROUND:

The NuProtect study reported data on the immunogenicity, efficacy and tolerability of simoctocog alfa (Nuwiq^® ) in 108 previously untreated patients with severe haemophilia A planned to be treated for ≥100 exposure days or up to 5 years. The NuProtect-Extension study collected long-term prophylaxis data in children with severe haemophilia A.

METHODS:

Patients who completed the NuProtect study according to the protocol were eligible for the NuProtect-Extension study, a prospective, multinational, non-controlled, Phase 3b study.

RESULTS:

Of 48 patients who entered the extension study, 47 (median age 2.8 years) received prophylaxis with simoctocog alfa for a median of 24 months, with 82%-88% on a twice-weekly or less regimen. No patient developed FVIII inhibitors during the extension study. The median (IQR) annualized bleeding rate (ABR) during prophylaxis was 0 (0-0.5) for spontaneous bleeding episodes (BEs) and 1.00 (0-1.95) for all BEs. ABRs estimated using a negative binomial model were .28 (95% CI: .15, .53) for spontaneous and 1.62 (95% CI: 1.09, 2.42) for all BEs. During the median follow-up of 24 months, 34 (72%) patients had zero spontaneous BEs and 46 (98%) had zero spontaneous joint BEs. Efficacy in treating BEs was excellent or good for 78.2% of rated BEs, and efficacy of surgical prophylaxis was excellent for two rated surgeries. No treatment-related adverse events were reported.

CONCLUSION:

No FVIII inhibitors developed during long-term prophylaxis in the NuProtect-Extension study. Prophylaxis with simoctocog alfa was efficacious and well-tolerated, and is therefore an attractive long-term option for children with severe haemophilia A.

WIL-31, the largest study in VWD prophylaxis, demonstrated the safety and efficacy of prophylaxis with a plasma-derived von Willebrand factor/factor VIII concentrate (wilate®) in patients with VWD. The lead investigator, Dr Robert F. Sidonio Jr., will present sub-analyses of data from the study which explore differences in bleed sites across VWD types and age groups. Prophylaxis was well-tolerated across all age groups in the 22 VWD type 3 patients who were included in the analysis population. MOTIVATE is an ongoing, international, investigator-initiated study supported through funding from Octapharma AG, evaluating the management of haemophilia A patients with inhibitors. Dr Carmen Escuriola-Ettingshausen, one of the study's coordinating investigators, will present an interim analysis of the study. Haemophilic arthropathy is a serious complication of haemophilia A and early diagnosis is critical to minimise its long-term impact. A proof-of-concept study has identified a signature of several circulating small non-coding microRNAs, which may be used as biomarkers to assist in the diagnosis of haemophilic arthropathy. Future work will investigate microRNAs associated with earlier stages of the condition in a sub-study of the PROVE clinical trial. Altered microvascular endothelial functionality has been observed in haemophilia A patients. Indeed, impaired tubulogenesis, migration and permeability of endothelial cells from haemophilia A patients has been observed, compared to those from healthy patients. New data will be presented showing that Nuwiq® can bind to endothelial cell surfaces, improving their functionality. FVIII physically interacts with platelets following vascular injury to ensure efficient clot formation. Different FVIII modifications may impact platelet binding and downstream signalling. Octapharma to present new clinical and scientific data at ASH 2023 from ongoing research initiatives aimed at advancing care for people living with bleeding disorders

LACHEN, Switzerland, Nov. 27, 2023 /PRNewswire/ -- New clinical data from Octapharma's haematology portfolio will be presented during the scientific program at the 65th American Society of Hematology (ASH) Meeting and Exposition, which is being held on December 9–12, 2023, in San Diego, California. New data from the prospective clinical trial using wilate® for prophylaxis in patients with von Willebrand disease (VWD) will be presented. Furthermore, the design of a new phase IV clinical trial of bone and joint health in people with haemophilia A will be presented, in addition to updates from research collaborations between Octapharma and academic institutions. The WIL-31 study investigated the efficacy and safety of prophylaxis with wilate® in adults and children of any gender with all types of VWD. WIL-31 is the largest prospective prophylaxis study in VWD and the only one with a prospective on-demand run-in study as an intra-individual comparator. The results of the study showed that wilate® prophylaxis is highly effective at reducing the number of bleeds in children and adults and provides compelling evidence to support the use of regular prophylaxis in patients with severe VWD. Data from sub-analyses of the study will be presented at ASH 2023, focusing on the effectiveness of prophylaxis in patients with different types of bleeding and in different subsets of patients. Women with bleeding disorders such as VWD are at an increased risk for heavy menstrual bleeding, but they are underdiagnosed and undertreated. A sub-analysis of WIL-31 will be presented, demonstrating that wilate® prophylaxis was effective in reducing the incidence of heavy menstrual bleeding. An additional sub-analysis focused on the efficacy and safety of wilate® prophylaxis in children and adolescents will also be presented. People with haemophilia A often suffer from joint health issues and reduced bone strength. Prophylaxis with factor VIII replacement therapy has been associated with reductions in joint bleeding rates, joint deterioration, and degenerative joint disease. In contrast, there is a shortage of data on the long-term effects of prophylaxis with the non-factor therapy emicizumab. The new phase IV clinical trial PROVE aims to assess the long-term effects of prophylaxis with Nuwiq® or emicizumab on joint and bone health. The study design will be presented for the first time at ASH 2023. In addition to clinical studies, Octapharma is proud to support scientific investigations with academic research partners that further our understanding of different aspects of bleeding disorders. Results from various avenues of scientific investigation will be presented at ASH 2023. In PUPs with haemophilia A, the development of inhibitors to FVIII is the most significant treatment complication, and there is a need to better understand the mechanisms underlying inhibitor development. The NuProtect study with Nuwiq® was the largest prospective study of a single FVIII product in PUPs. Researchers from University College London, UK, will present new data from a sub-study of the NuProtect study, revealing differences in gene expression profiles between PUPs who developed inhibitors and those who did not. In collaboration with the ETH Zurich and Balgrist Campus Zurich, Switzerland, the effect of FVIII on bone remodelling has been evaluated in vivo, with the results indicating that FVIII and/or von Willebrand can further inhibit the formation of osteoclasts, cells that play a key role in bone remodelling. Platelets and endothelial cells play key roles in haemostasis. Research performed in collaborations with the ETH Zurich, Switzerland, and the University of Eastern Piedmont, Italy, revealed significant differences in the binding of different recombinant FVIII products to platelets and endothelial cells.
