This is a research study to find out if Qutenza 8% capsaicin topical system is safe and effective when treating subjects with lower back pain (LBP) that is caused by damage at or near the nerve's root in the lower back leg (lumbosacral radiculopathy) which is pain that can move all the way down the back of the leg. The pain may also start outside of the spinal cord, in the peripheral nerves and may also be felt all the way down the back of the leg (neuropathic LBP). Qutenza 8% capsaicin, the study drug, is currently FDA approved to treat nerve pain after a shingles outbreak in addition to a type of nerve pain in the feet associated with diabetes. In this study a maximum of four patches per visit (sized 14cm x 20 cm) will be used to deliver the Qutenza 8% capsaicin to the participants skin.
If a subject meets the qualifications for this study, in addition to their standard of care for their LBP, they can expect to have a total of 5 visits in a 12 month period. Each visit will require subjects to fill out several surveys and receive treatment patches for their LBP (your doctor will decide if participants will need to be retreated at each visit based on your symptoms). The study visits are estimated to take 90 minutes upwards to 120 minutes.

Capsaicin in Digital Osteoarthritis Versus Control : a Randomized Study

The objective of this multicentric, randomized controlled double-blind clinical trial is to demonstrate the efficacy of transdermal application of capsaicin in patients with painful digital osteoarthritis with a neuropathic pain component.
Participants will receive either a transdermal patch of capsaicin 179 mg (8%) or the control treatment (capsaicin 0.04%).
Researchers will compare the intensity of pain in the fingers at day 60 in the capsaicin 8% group versus capsaicin 0.04% (control arm)

Colorimetric and SERS dual-mode sensing of capsaicin based on Au@PtNPs with enhanced peroxidase-like activity for gutter oil discrimination

The development of a capsaicin detection system that integrates facile preliminary evaluation with high sensitivity and accuracy across a wide detection range is crucial for the discrimination of gutter oil and the protection of human health. In this study, we constructed a novel colorimetric/SERS dual-mode detection system utilizing peroxidase-like Au@Pt nanoparticles (NPs) that exhibit SERS-activity. The Au@Pt NPs feature a spherical gold nanoparticle core with synapse-like platinum nanoparticles distributed intermittently on their surface, this structure enhances the catalytic activity of the PtNPs and the SERS activity of the AuNPs, making them ideal for designing colorimetric/SERS dual-mode probes. Based on the colorimetric assay, we achieved a capsaicin detection range from 1 to 10,000 μM, with a limit of detection (LOD) of 3.48 μM, as determined by absorption spectroscopy analysis. Additionally, a linear calibration curve for capsaicin ranging from 10^-9 to 10^-5 M was established using the SERS method, yielding an LOD of 7.38 nM. By integrating the advantages of facile detection through colorimetric analysis with the high-sensitivity trace assay provided by SERS, this proposed dual-mode detection system demonstrates excellent sensitivity, reliability, selectivity, and wide detection range compared to previous probes and methods. Consequently, this method can serve as an efficient tool for monitoring oil quality.

Deep eutectic solvent-induced coacervation in micellar solution of alkyl polyglucoside surfactant: Supramolecular solvent formation and application in food analysis

The number of deep eutectic solvents were investigated to induce coacervation and cause phase separation in the micellar solution of the alkyl polyglucoside C₈-C₁₀. Three fatty acids were selected as hydrogen bond donors namely hexanoic acid, heptanoic acid, and octanoic acid, while quaternary ammonium salts and a monoterpenoid thymol were used as hydrogen bond acceptors to obtain deep eutectic solvents. The precursors of the deep eutectic solvents could be incorporated into the micelle structure of alkyl polyglucoside C₈-C₁₀ and modulate its properties, improving the interaction with the target compounds and promoting phase separation due to the increasing size of the micellar aggregates. The synergy of green surfactant and deep eutectic solvent has been demonstrated in the determination of capsaicinoids as secondary metabolites retaining the pungency of chili peppers and, thus, of spicy foods. Extraction recoveries were above 75 % for target analytes. The limits of detection were found to be 1.7 μg g ^-1 for capsaicin and dihydrocapsaicin. The relative recoveries were in the range of 76 -130 %. The research presented is the first example of synergy between alkyl polyglucoside and deep eutectic solvent. The proposed extraction system has the potential to be used in many other analytical tasks, especially in the analysis of solid and heterogeneous samples. Not only fatty acid-based deep eutectic solvents but other based on alcohols, monoterpenoids, etc. can also be used for coacervation. Thus, wide range of already studied deep eutectic solvents could be investigated for this purpose.

Renal nerve afferents drive preferential renal sympathoexcitation in response to acute renal ischemia/reperfusion in rats

Renal nerve activity is composed of afferent (sensory) and efferent (sympathetic) nerve activity. Ischemia/reperfusion (IR) of the kidney increases renal sympathetic nerve activity (rSNA) and depresses renal function. As the role of renal afferent fibers in acute renal IR is unclear, we tested the hypothesis that renal IR increases rSNA triggered by renal afferent nerves responding to acute ischemia. Two experimental series were performed in adult male Wistar rats. IR was induced by total obstruction of blood flow to the left kidney by clamping the renal artery for 60 min and reperfusion for 120 min. We recorded MAP, HR, rSNA, and splanchnic sympathetic vasomotor activity (sSNA) in 8 normal IR rats and 6 left kidney deafferented IR rats (IR ARD). Renal deafferentation was performed using capsaicin administration to the left renal nerve 2 weeks before the experiments. Blood samples were collected before ischemia and at the end of reperfusion for total and differential leukocyte counts. Renal ischemia significantly increased rSNA 23 % (20 min: 0,07 ± 0,04mVs P < 0.05) but not sSNA. The increase in rSNA was triggered by activation of renal afferent fibers, since IR significantly reduced rSNA in the IR ARD group maximal decrease in frequency 22 % (180 min: -62 ± 29Δspikes/s) and in amplitude 41 % (-0,29 ± 0, 12mVs, P < 0.05) and induced hypotension and bradycardia. However, no significant difference was observed between groups in blood leukocyte profile, but a significant reduction in renal IL-6 was found in IR ARD, suggesting a reduction in renal inflammation in deafferented IR rats. The results show that renal afferent nerves trigger a preferential increase in rSNA and inflammation in the kidney during acute IR.

Not Much Works To Soothe Low Back Pain, Study Says

FRIDAY, March 21, 2025 -- There are a ton of suggestions out there for soothing an aching lower back -- acupuncture, heat, cannabinoids, muscle relaxants, TENS electrical pulses, dry cupping, opioids, massage and more. But not much actually eases back pain, including those suggestions, according to a new evidence review. Only painkillers like aspirin or ibuprofen -- non-steroidal anti-inflammatory drugs (NSAIDs) -- have been shown to be moderately effective in treating short-term lower back pain based on available medical evidence, researchers concluded. There’s also some evidence that five other treatments might help long-term low back pain, researchers said -- exercise , spinal manipulation, taping, antidepressants and warming creams containing ingredients like capsaicin . Overall, only 10% of treatments for low back pain had any medical evidence supporting their potential effectiveness, and even they didn’t work that great, concluded the review in the journal BMJ Evidence-Based Medicine . “Surprisingly, we found only around one in 10 was effective and most provided pain relief that was only marginally better than placebo – in other words, our review did not find reliable evidence of large effects for any of the included treatments,” Aidan Cashin , deputy director of the Center for Pain IMPACT at Neuroscience Research Australia, said in a news release. Non-surgical treatments are considered first-line care for back pain, Cashin said. Researchers performed the evidence review to see just how well these myriad therapies work. For the review, they analyzed findings from 301 clinical trials examining 56 treatments or treatment combinations for low back pain. Results showed that no sure-fire cures for back pain are currently available. Even ones with some evidence behind them can’t be counted on to work well for everyone. “We found for acute low back pain, NSAIDs may be effective. For chronic low back pain, exercise, spinal manipulation, taping, antidepressants and TRPV1 agonists (warming creams) may be effective,” Cashin said. “However, those effects were small.” Researchers also specifically highlighted five treatments that don’t work, based on the evidence. These included three for short-term low back pain – exercise, steroid injections and acetaminophen – and two for chronic back pain, antibiotics and anesthetics. For the rest of the treatments included in the research, there was simply not enough evidence to rule one way or the other, researchers concluded. More research is needed to prove whether or not these other back pain cures might have something to them. “We need further high-quality, placebo-controlled trials to understand the efficacy of treatments and remove the uncertainty for both patients and clinical teams,” Cashin said. “Placebo-controlled studies are the gold standard for evaluating the effects of treatments, with a clear need to better understand which treatments work for people with low back pain,” he added. In the meantime, Cashin recommends that people with back pain talk to their medical team to understand the treatments available and find one that works best for them. The Phase III trial AV001 aims to evaluate QUTENZA® in post-surgical neuropathic pain (PSNP), a debilitating complication of surgery occurring after approximately 10 percent of all surgical procedures1, thus affecting more than 3 million people with surgical procedures per year in the U.S.2 QUTENZA® is a topical system, non-systemic, non-opioid pain treatment that is currently approved in the US for the treatment of neuropathic pain associated with postherpetic neuralgia (PHN) and neuropathic pain associated with diabetic peripheral neuropathy (DPN) of the feet in adults.
